Narrative & Permit Application for North Slope #2 lot 1 Block 7
Dear Mr. Cross:
This lot is steeply sloping to the SW at a grade averaging 50%, the leachfield is located on the
only part of the property considered suitable for construction which had a natural slope of
approx. 30% and was leveled to create a building and leachfield area prior to our test hole
construction. The area from the top of the fill to the toe of fill is at the angle of material repose
or approx. 100%. This should not be considered a grade break as it was manmade. Our proposed
leachfield is located entirely in the natural soil below the organic layer. Considering the original
natural conditions I would say that the leachfield is being constructed on a 30-35% slope which
then increases to 50% south of the field. Due to the soil characteristics and past experience with
this type of leachfield installation I do not believe that the construction of this leachfield would
create a situation where effluent would daylight. There are no earth or roadway cuts located
below the leachfield. There are no other better sites on the lot. The proposed well and septic
system will have very limited impact on adjacent properties for the following reasons:
1. The surrounding lots are very large, allowing sufficient room for septic sites.
2. Immediate neighboring septic systems are all +30' distance.
3. Reserve space is adequate, due to absorption capacity and lot size.
4. Drainage will not be affected and is not a maj or consideration in our design.

SPECIFICATIONS FOR ON-SITE SEPTIC SYSTEM
LEGAL: North Slope #2 Lot 1 Block 7

A. GENERAL
1. The well and septic plan are for a single family residence only.
2. The drawing and or site plan shall be a part of this specification.
3. All materials and workmanship shall meet the Anchorage Department of Health and State
Department of Environmental Conservation requirements.
4. All soil tests are advisory to the design and are to be verified or modified in the field by the engineer.
5. All excavations and depths are advisory and are to be verified in the field by the contractor to meet
Municipality of Anchorage, Department of Environmental Conservation requirements.
6. It is the responsibility of the owner to obtain all necessary permits or easements and to locate any
adjacent multi-family wells.
7. The excavation is to be exactly in the area shown on the site plan, any deviation requires engineer
approval.
8. It is always recommended that a surveyor locate the nearest lot line position and the location of any
easements.
9. Any remaining open test hole excavations shall be filled.
B. SEPTIC TANK
1. Septic tank shall be a minimum of 1000 gallons, and of MOA approved design..
C. TRENCH
1. The trench is to be located on excavated bench to maintain uniform total depth of the trench bottom.
2. The bottom of the trench shall be level, plus or minus 1.5".
3. The total depth of the trench excavation is not to exceed 8' at any point.
4. .The effluent line within the trench shall be laid level within 0.03'.
5. The trench gravel is to be covered with typar fabric material.
6. Soil or combination of soil and extruded board insulation to a depth of 3' or equivalent is to be placed
over the leachfield.
7. The area over the trench is to be finish graded to prevent ponding of surface water runoff.
8. The septic tank and leachfield must not be closer than 10ft to any existing private well, 150' to any
Class "C" well, or 200 feet to any community well.
RECOMMENDED LEACHFIELD DIMENSIONS:
TOTAL DEPTH = 8' GRAVEL DEPTH = 5' under pipe, 2" over pipe
TRENCH LENGTH = 38' TRENCH WIDTH = 3'

tank is 20 years. Typical replacement costs range from $6,000 to $9,000.
In addition, this tank was not pumped due to winter road conditions that prevented

Due to icy weather conditions pumping was unable to be completed. We recommend to the buyer Kris
Nedwick to have the tank pumped in the spring at his earliest convenience. Liquid in tank is at a normal
operating level and if the buyer agrees to pump the tank we have no reason to believe that this would raise
any public health issues or operational concerns with the septic system.

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family On-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.
4. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation,
based on procedures outlined in the Certificate of On-Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the information
obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water
supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes,
ordinances, and regulations in effect at the time of installation.

Engineer's Comments: This investigation was completed in compliance with ADEC and MOA regulations. The
assessment of the condition of the well and septic applies only to the conditions as of the day tested. The flow
and absorption rates may change due to subsurface conditions that may not be observed from the surface,
changes inland use, local soil characteristics, groundwater levels that may fluctuate during the year and the
water usage of the family being served by the system. The operational life of all well and septic systems are
subject to these various and dynamic characteristics and are outside the control of the
evaluator of the well and septic system.
