Description and Permit Application
Dear Onsite Services Engineer,
VEl Consultants was contracted to design a septic tank upgrade at the above referenced property.
The existing septic system serves a four bedroom house and consists of two septic tanks (750gal
and 1000gal) and an absorption trench. The 750 gallon septic tank, installed in 1971, has partially
collapsed and needs replacing. The proposed repairs will consist of abandoning the 750 gal. tank
in place and installation of a new 1,250 gallon septic tank and piping to connect to the existing
septic system.
The existing septic tank and proposed replacement tank are over 10' from any property line or
building foundation and over 100' from any surface water. There are no water mains or service
lines in the subdivision. Immediate neighboring septic systems are all more than 100' away, and no
private wells within 100' or community wells within 200' of the existing septic system. We request
that you approve the attached permit application for construction of the septic tank repairs.

Dear Onsite Services Engineer,
VEI Consultants received comments from your office for the subject permit dated June 6,
2011. We have addressed the remarks and are submitting additional information and
response to each comment as follows:
l)
To the best of our knowledge, the original well installed in 1971 and deepened in
1976 was decommissioned by filling the casing with a cement slurry and capping
below the ground surface. Presently the old well is not visible from the surface
and it is assumed proper decommissioning of the well was accomplished when the
well casing was cut below the surface. Attached is a statement from the owner
describing the circumstances regarding the old well. According to the owner,
decommissioning of the old well was part of the conditions for sale of the property
by the Alaska Credit Union in 1990 to the current owner and was accomplished by
the Bank prior to sale; however, documentation regarding the old well was not
provided to the owner.

A permanent Residential Septic System Easement has been acquired allowing the
drainfield to be located on lot 11. A copy of the recorded easement is attached. A
revised site plan showing the easement is also attached.
3)
VEI performed a leak test on the existing 1000 gallon septic tank on June 8th. Test
results indicate the tank to be sound and without perforations. Following is a
description of the observations made: The tank was found to be full at the start of
the test with static water level measured at a depth of 8feet-2inches below the top
of the standpipe. The top of the tank was measured at a depth of 7feet-4inches.

The static water level measured at the start of the test corresponds to the
approximate invert elevation of the tank outlet as determined by field
measurements. The septic system was then unused and observed for a period of 8
hours. No change in water level was observed during this time. At the end of the
observation period, water was added to the tank through the standpipe at a rate or
4gpm for 30 minutes. No change in water level was observed after water was
added. Our observations indicate the septic tank maintains a water level equal to
the invert of the tank outlet.
We request that with this additional information the On-Site Sewer Application Permit be

When we purchased the property in Jan 1990 from the Alaska USA Credit Union, part of
the conditions for sale was that the old abandoned well that was next to the house be
properly decommissioned by filling with a cement slurry and capped below the surface.

CONSTRUCTION PRACTICES AND MATERIAL SPECIFICATIONS
LEGAL DESCRIPTION: McKinley Heights Subdivision, Block 2, Lot 1, 2 &Il
GENERAL:
1. The scope of this project includes abandoning in place an existing septic tank and
installation of a 1250 gallon septic tank to serve a 4 bedroom single family
residence located on the referenced property.
2. Construction shall be in accordance with the municipal permit with any special
provisions or conditions, and all applicable Municipal Wastewater Disposal
Regulations.
3. Contractor shall verify the septic tank is a minimum 100' away from any private
wells in the area.
4. The contractor shall be responsible for obtaining any necessary underground utility
locates.
5. Contractors installing wastewater disposal systems must be certified by the
Municipality of Anchorage On-Site Water and Wastewater Department for
systems installations. Owners installing their own systems must also receive prior
approval from the Municipal On-Site Water and Wastewater Department.
SEPTIC TANK INSTALLATION:
1. Septic Tank shall be a minimum of 1,250 gallon tank of MOA approved
construction with allowable burry depth of 10 feet. The contractor will ensure that
tank installation conforms to manufacturers specifications for depth of bury,
backfill type, and compaction requirements. Minimum depth of cover shall be 4
feet.
2. Septic Tanks installed with less than 4 foot of cover shall be insulated. Minimum
cover with insulation shall be 2 feet.
3. The sewer piping shall be 4 inches laid at a 2% minimum and insulated with 2"
insulation if shallower than 3ft, with 2ft minimum bury.
4. Construction shall include two 4 inch cleanouts for pumping access to the septic
tank. All stand pipes on the septic tank shall extend to a minimum of 12 inches
above final grade
5. In the line between the tank and the absorption system there shall be two adjacent
cleanouts. The cleanouts shall be located on undisturbed soil not more than ten feet

from the tank. The first cleanout in the line shall be to clean the line toward the
absorption system and the second cleanout will be oriented to allow cleaning
toward the septic tank.
The septic tank shall be sufficiently bedded with proper material and compacting
to prevent settling or shifting of the tank.
The contractor shall abandon the old tank in compliance with the Uniform
Plumbing Code.
MINIMUM MATERIAL SPECIFICATIONS:
1. A MOA approved septic tank manufacturer must construct any septic tank
proposed for installation.
2. Insulation shall be at least 2 inch thick extruded direct burial polystyrene (Dow
Chemical Company Styrofoam HI, or qual)
3. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder,
Fernco, or equal)
4. The following pipe materials are MOA approved for use in septic system
installations:

the inspecting engineer.
INSPECTIONS:
Typically there will be 2 inspections required during the replacement of a septic tank.
These inspections will occur as follows:
1. The first inspection must be conducted after setting of the septic tank and
installation of piping, but before the placement of any backfill.
2. The final inspection is to occur upon final grading of the property.
The inspecting engineer is to be contacted at least 24 hours prior to the start of
construction. The engineer shall be notified ½ day prior to a required inspection.

The plans are approved, conditioned on adherence to the following items:
1. screened rock back fill in deep and wide trenches (3/4" - 3").
2. screened rock back fill in absorption bed (3/4" - 1 1/2").
3. sufficient area plotted on as-built for replacement system.
This letter constitutes the permit required by A.S. 46.03.720(a) for
plan approval of sewerage systems.
It should be remembered that final approval will necessitate the submission
of Engineer As-built Plans to include photographs as indicated on page 13
of enclosure. This will consequently mandate that a professional engineer
conduct basic inspection of this project so as to be able to sign off on
as-built pIans. Arrangement for this inspection work is the responsibility
of 'the developer.

I certify that I have checked, verified, or conformed to all MOA and HAA guidelines in effect on the date of this inspection.
During a recent Health Authority Approval on-site inspection and

well productivity required by this department (AMC 15.55) is 150

per minute. Although the subject well currently meets the
minimum well production requirements, all Parties concerned are
advised that the production capacity of the well may fluctuate
and that water conservation measures may be required during
periods of low production capacity.
Measures which can be taken to minimize the impact of a low
production well are:
1. Installation of a storage tank (300 to 500 gallon) to
serve as a reserve source during times of maximum water
usage.
2. Restriction of noncritical water uses such as, washing
cars, lawn and garden watering, etc.
3. Installation of water saving devices on faucets,
showers and toilets.
4. Conservative use of laundry facilities and dishwashers.
This advisory must be attached to all copies of the subject
Health Authority Approval.

property
A sewer system adequacy test was performed on the system located
on the referenced property, as you requested. The septic tank was
pumped and verified to have a capacity of approximately 750 gallons.
The seepage 'pit was charged with 1000 gallons of water and after
a period of 24 hours, approximately 143 gallons had percolated out
of the c~ib.
It can be concluded from this test that the septic tank is adequate
for a two bedroom residence and the seepage pit is currently adequate
for approximately one bedroom. ~~%~.to in.~rm-youthat both
