The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

Engineer's Comments:
In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEO and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above: Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.

Dear Ms. Sloppy:
Thank you for the plans and specifications and engineers report in
request of well regulation waivers for the referenced project under
your letter of August 23, 1983. The construction of the referenced
project will consist of furnishing and installing an estimated 2,960
linear feet of 8 inch ductile iron sanitary sewer line and other
related appurtenances in-the Alpine Village and O'Brien Subdivisions.
The plans and specifications are approved for matters of concern to
this Department.
In accordance with Title 18 of the Alaska Administrative Code Chapter
72 (18 AAC 72.021.a), a Mr. William P. Hamm, P.E., submitted a report
for ADEC's review and consideration to waive established separation
distances to construct, maintain and operate portions of Alpine Village
LID No. 135 closer to drinking water sources. More particularly,
construction of the referenced project will require a waiver to encroach
upon the well radius of ten wells adjacent to this project. The wells
are located along.Bern, Zurich and O'Brien Streets between E. 72nd and
E. 74th Streets. In recognition of the report and the plans and
specifications for the proposed sewer line, the following separation
distances are approved instead of that required in Table A of 18 AAC
72.021.
Separation distances of 20 and 64 feet between a private well
on Lot 8, West Side of Bern Street {between E. 72nd and E. 74th
Streets) and proposed sewer line and nearest cleanout respectively.

The Department finds that the special manholes detailed in the plans
and specifications as an acceptable solution to the separation distance
problem between manhole and well. Within the special manholes, the
sewer line's ~anged pipe is mechanically sealed and gasketed to prevent
leakage of any sewage into the manhole. With the above in mind and by
reviewing the pertinent characteristics of piping materials to be used,
the pipe's most feasible alignment and the limited soil information,
this Department f~nds the above separation distances as adequate
protection of the private water sources.
Please forward copies of. the bid tabulations, the signed construction
contract and notice-to-proceed related to construction when they are
available.
