2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska Wastewater
Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80).
3. The wastewater code requires inspections during the installation. The engineer must notify the Development Services
Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
Special Provisions: Permit includes slope waiver OSPl11209, allowing the drainfield to be 35 feet
from a 40% maximum slope.
Special Provisions: Inspections shall be in accordance with AMC 15.65.150.

Mr. Jay Crewdson:
I respectively request that the subject wastewater absorption field receive a waiver for slope distance.
As shown on the design drawings, the FIVE-WIDE trench is located approximately 25-feet from a slope
that exceeds 25%. Our interest is to keep the drainfield in the same location, thereby allowing a greater
area for the future reserve. The waiver request is based on the following:
· Based on the attached profile drawings, the treated wastewater will not surface if it percolates
at 25% or more.
· Downhill slope is on the same property

To whom it may concern:
The proposed 4 bedroom house on this lot will be served by a private well and septic system. A test hole
was dug on the property and percolation tests were performed. The water table was not encountered
and was not recorded during the 7-day monitoring period beginning on 7/26/11. We are proposing that
a 1250 gallon STEP tank and a FIVE-WIDE drainfield be installed on the property. Notes regarding the
design are summarized as follows:
1. Soils: See the attached soil profile log which shows the soil classifications, groundwater
monitoring, and percolation test results. Based on the field results, an application rate of 1.2
gallons/day/SF is recommended.
2. Five Wide design:
a. Percolation Rate: 2 minutes/inch
b. Proposed Application rate: 1.2 gallons/day/ft2
c. Number of Bedrooms:4
d. Design Flow: 600 gallons/day
e. Absorption Area: 500 SF
f. Total depth: 5 FT
g. Effective depth: 3.0 FT
h. Width:5 ft
i. Reduction Factor: 0.58
j. Minimum Length: 58 ft
k. Effective Absorption Area: 290 FT
Surface Waters: there are no surface waters located within 100 feet of the proposed septic
system.
Topography: The drainfield will be located 50' from the cut bank to the east and approximately
25 feet northeast from a slope greater than 25%. A waiver request is included with this design
submittal.
