If you have drilled the well, a well log must be sent to this office for documentation of the installation and to close the permit. If a licensed Professional Engineer has inspected the installation of the on-site wastewater system, the original as-built inspection report must be sent to this office for review, approval and documentation. All inspection reports must be submitted within 30 days of construction completion. A new permit must be obtained from this office for a well and/or on-site wastewater system NOT installed by the expiration date. August 5, 1998 Municipality of Anchorage Department of Heath & Human Services 825 "L" Street Anchorage, AK 99502-0650 Subject: Lot 1, Block 2, Prospect Heights Subdivision No. 4 Septic System Design Impacts to Adjacent Properties Dear Onsite Services Engineer: The owner of Lot 1, Block 2, Prospect Heights Subdivision No. 4 intends to construct a 4 bedroom home on the lot. We are therefore requesting a well and septic system permit be issued for the lot. The attached site plan and backup documentation identify the size and location of the new septic system and well proposed for the lot. It also shows the location of the 100' protective radius. Three testholes were placed on the lot at the locations shown. In all cases bedrock was encountered at depths between 5.5' and 6'. The material above the bedrock, however, percolates well and there is no evidence of groundwater. We are therefore proposing placement of a recirculating septic tank with an upflow filter. This system requires an absorption bed of 150 square feet based on a percolation rate less than 30 minutes per inch. We are designing a 300 square foot bed for additional capacity. Soil materials on the lot consist of peat and organic material for the initial 1' to 1.5'. This material is underlain be silty sand and gravel to depths of 5' and 6'. More than 2' of unsaturated percable soil is available for the absorption bed. Our design shows placement of the distribution piping at the ground surface. All organic material will be removed and replaced with coarse sand. The bottom of the bed will be a minimum of 4' to 5' above the bedrock layer. The surface of the lot slopes from east to west at varying grades. Lot 1; Block 2, Propsect Heights No. 4 August 5, 1998 Page Two The slope from the road is approximately 10%, but flattens out in the area of the absorption bed. The slope then increases to 15% to 20% to the west boundary of the lot. No conflicts will exist between the new system and wells or septic systems in the area. If the system is constructed as designed the following statements apply: The system, if constructed as designed, will have no adverse impact on the wells in the area or those to be constructed in the future. The system, if constructed as designed, will have no adverse impact on existing septic systems in the area or those to be constructed in the future. The system, if constructed as designed, will have no adverse impact on reserved space, either surface or subsurface, on any lots located in the area.
