Box 196650 Anchorage, AK 99519-6650 Subject: New Septic Permit - T12N R3W SEC 15 LOT 68 The owner has requested we proceed forward to obtain a septic permit on the subject lot to serve the 4-bedroom house. The house is served by a Class C well. The general slope of this lot is from east to west at a grade of approximately 2-5% and flat over the septic area. We have designed the 4-bedroom system utilizing the testholes that were excavated on October 24, 2011. We propose to install one 64' long deep trench. Water was not encountered at excavation or at monitoring. There are no public or private wells within 200' of our proposed system location except as noted. There is no surface water within 100' of the proposed system and there are no known curtain drains within 50'. The assessment of the condition of the well and septic applies only to the conditions as or the day tested. The flow and absorption rates may change due to subsurface conditions that may not be observed from the surface, changes inland use, local soil characteristics, groundwater levels that may fluctuate during the year and the water usage of the family being served by the system. The operational life of all well and septic systems are subject to these various and dynamic characteristics and are outside the control of the evaluator of the well and septic system. That a water well exists on Lot 69 that supplies water to Lots 68 and 69. 2. Owner hereby establishes an easement on Lot 69 as set forth on the attached Exhibit A for ingress and egress to inspect, make repairs, and such other actions as are reasonably necessary to assure that the water well is properly maintained. 3. Current owners and any future owners of the subject properties shall share the water well located on Lot 69, including, but not limited to, maintenance and repair of the water well and the pumps, pipes, water lines, and maintenance of all of the shared Easement and Water Well Agreement. Page I of 3 components of the water system, excluding those fixtures which are specific to the individual two dwellings. Each Lot shall be assessed and responsible for an undivided one-half share of said costs. 4. Owners of Lot 68 and Lot 69 cannot improve or modify the shared well without the consent of both owners currently utilizing the shared well. In the event both Owners agree, any costs of improvement or modification shall be shared equally between said Owners. 5. The cost of electrical service to the well pump shall be paid by the owner of Lot 68. 6. In the event either Lot Owner elects not to use the water well and provides 30 days written notice of same to the other Lot Owner, said party shall no longer be responsible for the maintenance or repairs on the well. The remaining Owner shall then be responsible for all costs, improvements, repairs, etc. of the water well. In the event the remaining Owner is the owner of Lot 68, said Owner shall notify in writing the Owner of Lot 69 of any necessary modifications or improvements, as the well is situated on Lot 69. 7. In the event the Owner of Lot 68 relinquishes said Owner's right to the shared well and drills a new well, said Owner of Lot 68 must relinquish any rights to enter into Lot 69 and the easement for shared water well as described herein shall be terminated and shall be of no further force or effect. 8. This AI,n-eement shall run with Lots 68 and 69 and shall be binding upon and shall accrue to the benefit of the respective owners, their heirs, successors and assigns unless terminated as described herein.
