Subject: Reactex septic system site visit summary
To whom it may concern:
ARM Services was onsite in January (as well as a follow up visit in April) responding to the subject
property due to an alarm condition observed at the control panel. Upon arrival, the system control
panel was found to be in Auto, and the alarm tight on. The audible alarm appeared to have been
previously disconnected and did not appear to be making sound. Please see the below findings and
recommendations for repair. An April update can be found at the end of the report.
OBSERVATIONS:
Septic Tank:
-Upon arrival, the septic tank appeared to be operating about 3ft surcharged (overfull), and the two
pumps were not activating properly. Both of the pumps in the 2nd compartment/lift station were
tested, and initially did not appear to be responding to testing. The discharge pump was pulled and
intake screen checked; this was found to be fairly clogged with hair and biomat. ARM cleared the pump
intake fitter and tested again. Once the discharge pump was placed into hand, effluent was observed
properly leaving the lift station towards the upflow fitter. The liquid level appeared to be dropping in
the lift station during manual pumping of the discharge pump. The intake filter was cleaned on the
recirculation pump as well, and this pump property responded to testing as well. However, once the
liquid level was dropped in the lift station (by pumping out through the up -flow filter), it was observed
that there was a large steady stream of effluent returning to the tank during recirculation pump
testing. Further investigation revealed that this was likely due to a hole that was observed around the
pressurized line leading from the recirculation pump to the trickling filter. It is reasonable to believe
that the pressurized line is fractured or broken out side of the tank, as no iqui was o served reaching
the tric ing fitter see a ow . i e ARM was not a e to visually inspect the possible hole in the
sidewall of the riser, given the age of the tank (26+ years) it is likely that the original hole for the
pressurized line has rusted out and is not properly seated.
-The "discharge timer on" float did not appear to be contacting the panel properly during testing.
Despite the liquid level being high enough to activate the timers, the Discharge pump timer in the
control panel did not appear to be activated. During testing, it was intermittently observed activating
the timer, but not reliably.
-All of the components of the lift station were observed to be heavily biomatted during the site visit.
Annual maintenance helps to keep the components clean and operating property.
Summary: The system did not appear to be operating as it was designed due to a likely broken pipe
underground, and was pumping untreated effluent up to the drainfield. Given the age of the tank, it
may be worth replacing the tank vs. repairing these deficiencies
Trickling Filter:
-There was no observed flow through the spray nozzle during ARM's site visit. This can likely be
attributed to the previously mentioned broken pressurized line, as the pressurized line services this
fitter. The spray nozzle appeared to be clear, based upon a visual inspection.
-The treatment media was observed to be fairtv heavily biomatted and brid�in�, and did not appear to
have been in use for some time (likely due to the above broken pipe). It was also observed that the
vertical separator for the media sheets had fallen down and was no longer spacing the sheets out
properly.
Upflow filter:
-The drainfietd was observed to be dry when ARM arrived onsite, and maintained as such throughout
the site visit.
-The upflow filter appeared to be allowing liquid to pass through it property, and was observed
transferring liquid to the drainfietd.

RECOMMENDATIONS:
-A proper maintenance and cleaning should be completed on all of the components of the lift station to
ensure proper treatment and operation.
-The Discharge pump timer on float should be replaced. (Due to the age of the system, it is possible
that other electrical deficiencies may be present and uncovered during float replacement.)
-The alarm should be reconnected to provide the code required outdoor alarm.
-The pressurized tine should be dug up and repaired, and hole in the sidewall of the tank seated to
prevent unwanted groundwater and surface water from entering the tank.
-Given the age of the tank and conditions found while onsite, it may be prudent to replace the
septic tank instead of completing repairs. Due to the tanks age, the sidewall of the tank may
not be able to be seated during the repair. Coupled with the conditions of the other components
in the system, the best use of funds may,he to upgrade the system to an Wgraded septic
system. Septic tanks have a lifespan of 20-30 years, and given an install ate of 1998, it is likely
that the tank has other holes. The system is quickly approaching the 30 year mark, at which the
point the tank would have to be replaced when the house is sold. Tank/system replacement
would supersede all other recommendations.
-The treatment media should be cleaned and treated to clear the biomat and bring the system back up
to health.
-The treatment pod's vertical separator should be installed correctly to maintain spacing between
sheets.
-To protect the drainfietd, it is recommended that proper maintenance, repairs, or replacements are
completed on the other components to prevent damage. It is reasonable to believe that the system
may continue operate for a short time in its current configuration, but if left long term the drainfield
may show signs of failure and require attention. Due to the probable broken pressurized line, there is
no secondary treatment happening to the effluent, and the system is pumping poorly treated effluent
to the drainfield. Poorly treated effluent can drastically shorten the lifespan of a drainfield.
APRIL UPDATE:
-ARM responded to a backup of the system, the cause of which was likely due to a tripped breaker. The
system was pumped down via pump truck to provide relief and the ability to inspect the system. During
the inspection, it was revealed that the recirculation pump was no longer properly responding to
testing, and appeared to be in need of replacement. Additionally, the foundation cleanout was
observed to be holding some liquid, so ARM snaked the line but did not find any obstructions. Further
investigation into the septic as built showed that the other cleanout next to the house was a part of a
venting system for the treatment pod, and not a part of the sewer line as thought while onsite. Other
site conditions were found to be as observed during the January site visit. Budgetary repair/
replacement costs include replacement of the sewer line from the house.
For a rough budgetary estimate, it is likely that all of the aforementioned repairs could cost $12 000-
18,000 (budgetary, subtract $3,500 if sewer tine from house is no rep ace wit repairs). Th_ !s option
wou still leave a 26+ year old tank serving the system, possibly leaking
and causing diminished
treatment. I spoke with JeffGarness over at Garness Engineering, and it is is opinion that a tank
upgrade wilt also require a treatment pod upgrade as welt, as the MOA will not approve continued use
of the existing treatment pod with a new tank install. Approximate cost for upgraded treatment system
and tank would run $45,000-$55,000 (price is depending on components required for the property, does
not include engineering or landscaping costs. These can be added to the quote if necessary once a path
forward is chosen and pre -bid site visits are made). We would recommend using Garness Engineering
for the design and permitting. As noted above, tank replacement would supersede any repairs.

ADVANCED WASTEWATER TREATMENT SYSTEM
MAINTENANCE AND REPAIR AGREEMENT
THIS MAINTENANCE AND REPAIR AGREEMENT, herein the "AGREEMENT" made and
entered into as of this Day of of 20 by and between
Ka w- n wa here' the "OWNER," and the Municipality of
Anchorage, herein the "MUNICIPALITY", in accordance with Anchorage Municipal Code
(AMC) 15.65.365. In consideration of the mutual covenants contained herein, the parties to this
Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS),

2. Maintenance, Repairs and Alterations.
(Owner is required to read, understand and initial each section)
- Throughout the term of this Agreement, the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.
f ` It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This
includes an annual maintenance fee (typically $400 to $600).
C-L Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance,
repairs or permitted alterations to the system.
�� Owner acknowledges that regular maintenance of an AWWTS reduces the potential
failure of the system, which could include sewage backup and costly repairs or drainfield
replacement.

�-- Owner acknowledges that the Municipality may request records of maintenance and
repairs from the manufacturer's representative or maintenance provider.
Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be
assessed in accordance with AMC 14.60.030.
Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS. The Municipality will give at least 24-hour notice.
1C Owner agrees that any sale or transfer of title of the property will not occur without a new
Certificate of On -Site Systems Approval.
Owner agrees that the AWWTS installation and maintenance requirements as provided
by the AWWTS vendor/installer and approved by the Municipality are the governing
guidelines for the construction, maintenance and repair of the Owner's AWWTS.
Owner agrees to maintain remote monitoring of the AWWTS as required by the
AWWTS approval.
3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions, nor in any way affect
the validity of the Agreement or any part hereof, or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.

It has come to our attention that it will not be possible to raise the sand height in the re -built drainfield to
an elevation that is 12 inches higher than the elevation of the existing top of sand. The limitation has to
do with the fact that the discharge line coming out of the existing upflow filter is fixed. If we remove the
sample port after the upflow filter, we can raise the height of the drainfield a maximum of four (4) inches.
I would argue that the height of the existing drainfield has adequate separation (4 feet) to seasonal high
groundwater because the design was based upon groundwater monitoring perforrr�ed by Lou Butera, RE,
during May of 1995. Butera monitored groundwater in TH #1 between 5/17/1995 and 5/26/19995 and it
varied between 5.8 feet and 6.0 feet below grade. The proposed top of sand in the re -built drainfield will
be 1.6 feet below the top of original grade. In short, the proposed top of sand will be greater than 4 feet
above the seasonal high groundwater table. An additional factor of safety in the design is provided by the
fact that effluent from the RUF system is passed through over 3 feet of sand in the existing upflow filter
before being discharged to the drainfield.
We are requesting that the MOA amend the permit to allow for the drainfield top of sand elevation (in the
rebuilt drainfield) to be raised to an elevation that is only 4 inches higher than the top of sand in the
existing drainfield.

The existing 4 bedroom house is served by a private well and an Orenco "Reactex" septic system. The
drainfield is in a state of failure and needs to be upgraded. We are proposing to rebuild the existing
drainfield and re -utilize the existing 2000 gallon S.T.E.P. tank and existing trickling filter. The proposed
re -built bed will be 10 feet wide and 25 feet long (same as previous install).
1. SOILS: See the 1995 Louis Butera, P.E. soil log (attached) which shows the soil classifications,
groundwater monitoring, and the percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There is a road ditch to the south of the property which runs along Mannytell
Avenue. The proposed re -built drainfield will be installed greater then 100' from this water source..
4. TOPOGRAPHY: As can be seen on the attached design drawing the topography around the
drainfield is relatively flat. In short, there are no slope concerns.
Ground water monitoring (G. W.M.) from 1995 was at 5.8 feet below grade. After the contaminated sand
is removed, MOA sand will be imported and built to an elevation equal to the existing top of sand.
We are unaware of any adverse impacts this installation and granting of the waiver request would have on
adjacent wells or septic systems. If you have any questions, please contact us at 337-6179. Thank you

RECIIiCULATING UPFLOW FILTER (RUF) SYSTEM
To whom it may concern:
1. GENERAL: The existing 4 bedroom home is served by a private septic system and well.
The existing bed system is encroaching groundwater and surface water and must be upgraded. A
septic upgrade permit (SW950249) was issued by your Department in August of 1995, however
the proposed upgrade was never installed. The owner has now requested our firm to redesign a
septic system which would allow for a smaller leachfield than the previous design. We propose to
install a Recirculating Upflow Filter (RUF) system as designed by Anchorage Tank & Welding.
In May 1995 three test holes were excavated and soil percolation tests performed by Eagle River
Engineering Services. The original soil logs are on file in your department (see attached copies).
The soils are summarized as follows:
2. SOIL CONDITIONS: Per Municipal records three test holes were excavated on May 16,
1995 to an average depth of 8 feet. he soils profile for TH #1 consists of 2.5 feet of organics
(mainly fill and topsoil) followed by a GM material. Groundwater was encountered during the
excavation at a depth of 6 feet, and was monitored at the 5.8 foot depth ten days later. A
percolation test was performed between the depths of 3.0 and 4:0 feet and the absorption rate was
measured at approximately 9 minutes/inch (see attached soil logs).
3. DRAINFIELD: The intent is to install a Recirculating Upflow Filter (RUF) system that will
allow the use of a small drainfield in the area of TH 41. The size of the drainfield will be based
upon the previously established criteria for the RUF systems, which dictates that soils percolating
between 1 & 30 minutes/inch have an allowable appli4kion rate of 4 gpd/ft2 and soils percolating
between 30 &. 60 minutes/inch have an allowable application rate of 2 gpd/0. Given the perk

rate of 9.3 minutes/inch the 4 gpd/ft2 application rate would apply. We are proposing to install
10 foot wide bed that is 25 feet long and has an effective depth of 6 inches. This corresponds to
an absorption area of 250 ft2, or an application rate of 2.4 gpd/ft2 (assuming 600 gpd total flow).
4. ORENCO PACKAGE SYSTEM: The STEP tank with the trickling and upflow filters will
be manufactured by Anchorage Tank & Welding to meet the latest design criteria established by
Orenco Systems, Inc. The point of contact at Anchorage Tank is Lowell McNutt. Since you are
familiar with the operation of the system I won't elaborate. As with their standard STEP tank, it
is equipped with a high water alarm per M.O.A requirements.
5. SURFACE WATER: There is a surface water source within the road ditch which flows east
to west along Mannytell Avenue. It has been previously determined by the Municipality that the
water which flows within this ditch is definitely surface water. The proposed upgrade is designed
to be installed greater than 100 ft. from this water source.
6. TOPOGRAPHY: The slope of the ground at the proposed upgrade site is basically flat (less
than 2%). There are no slopes greater than 25% within 50 feet of the proposed bed.
7. CLOSING: I am open to any suggestions from your department that would be an
improvement to the proposed system. I am unaware of any negative impacts that this installation
would impose on adjacent wells, or septic systems. If you have any questions, please call me a

SAMPLING AGREEMENT

The parties agree that Susan Oswalt & Associates (contractor) will provide monthly sampling
services for the Upflow Filter System installed on Lot 6, Block G, Timberlux Subdivision as
required by the Municipality of Anchorage, Department of Health & Human Services, during
the designated testing period.
At presentt the contractor will take samples from the outfall of the nitrate treatment apparatus;
however, changes to this matrix may occur. If any changes to the sampling arc required by
the Municipality, this agreement. will remain in force and samples will be collected as
required.
It is understood that the required testing period will be six (6) months. It is also understood
that although the cost of laboratory charges for this service will be borne by the
Municipality, the actual sampling labor costs will be borne by the property owner signing
this agreement. The cost of sampling will be at the rate of $45/hour, Additional services
provided for this system will be charged at the same rate.
The property owner agrees to keep sampling areas clear of snow and to provide reasonable
access to these areas, including access to any control panel within the dwelling. The
contractor will make every effort to give adequate notice to the property owner prior to
sampling.
This agreement may be terminated by either party with a minimum of ten (10) days' notice.

We consider the drainage channel located on the above referenced property to be attributable to
snow melt, and therefore does not meet the definition of surface water which would require a
100' setback. The upgrade design utilizes a bed with a 2' sand filter and corrects a failing trench
system located in the groundwater table. We are going to recommend to the owner that an air
supply system be added six inches below the bed gravel in the sand layer to facilitate an aerobic
process. Please process the permit as soon as possible to allow construction this fall.

Narrative & Septic Upgrade Application
Dear Mr. Roth:
We are applying for a permit to construct an upgrade leachfield for the above referenced property.
This property has experienced continual problems due to changing neighborhood drainage
patterns.
We logged three test holes in the available septic area and found an acceptable receiving gravel
soil layer in two representative test holes, that may be usable for a conventional system with a
sand filter layer.
The permit request is for an imported sand filter layer over gravel with a varying water table depth
of one to two feet below native gravel soil as recorded during the high water level period during
break-up this season. The final separation distance to the water table will be at four feet vertical
distance utilizing sand fill. This is outlined on the attached profile drawings. Effluent delivery
will be pressurized and will utilize the OSI intermittent sand filter type control unit for a timed
dosing. The system is located on what we believe is the only area of the lot to have acceptable
soil In this position, we are located 60 feet from a seasonal drainage path that runs in the road

SPECIFICATIONS FOR ON-SITE SEPTIC SYSTEM
LEGAL: Tbnberlux #3 Lot 6, Block G
06/23/95
A. GENERAL.
1. The septic uprade plan is for a single family residence only.
2. The drawing and or site plan shall be a part of this specification.
3. All materials and workmanship shall meet the Anchorage Department of Health
requirements.
4. All soil tests are advisory to the design and are to be verified or modified in the
field by the engineer.
5. All excavations and depths are advisory and are to be verified in the field by the
contractor to meet Municipality of Anchorage requirements.
6. It is the responsibility of the owner to obtain all necessary permits or easements
and to locate any adjacent multi -family wells.
7. The excavation is to be exactly in the area shown on the site plan, any deviation
requires engineer approval.
8. It is always recommended that a surveyor locate the nearest lot line position and
the location of any easements.
B. SEPTIC TANK WITH LIFT STATION
1. The septic tank shall be an Anchorage Tank 1,500 gallon septic tank with integral
OSI lift station pump model OSI-20-HHF and intermittant sand filter type
controller, SICTETMRO with programmable timer.
C. BED
1. The bed is to follow the natural land contour to maintain uniform total depth of the
bed bottom.
2. The bottom of the bed shall be level, plus or minus 1.5".
3. A 2' approved filter sand layer shall be required at a total depth of 6'.
3. The total depth of the gravel layer is not to exceed 4' at any point.
4. The effluent line is to be a 1-1/4" PVC line with 1/8" holes drilled 27-3/8" OC
within the leachfield, placed face up with orifice shields placed over holes. Line
from tank to field is to be 1-1/4" HDPE.
5. The bed gravel is to be covered with typar fabric material.
6. Soil or combination of soil and extruded board insulation to a depth of 3' or
equivalent is to be placed over the bed. Mounded side slopes not to exceed 3:1.
7_ The area over the bed is to be finish graded to prevent ponding of surface water
runoff.
8. The septic tank and leachfield must not be closer than 100' to any existing private
well, 150' to any Class "C" well, or 200 feet to any community well.

and must be returned to this department within 30 days of the well completion.

Dear Lowell:
On 11/22/99 1 prepared a letter which addressed the recirculating trickling filter (RTF) on the
subject property. It was noted at that time that the liquid level in the bed type drainfield was
about 10 inches deep and that a plumbing leak was evident. Since that time the plumbing leak
was repaired, and the homeowners began to exercise a more 'normal" frequency of washing
clothes. On 12/14/99 I checked the liquid level in the bed an found it to be around 1.75 inches
deep (only 250 square feet of surface are in the bed). This corresponds to only about 90 gallons
of effluent in the pore space of the drainrock. Given the hard usage that is placed on this system,
the operating level is reasonable. In short, this system now appears to be operating normally.
