must be either: A. Open and closed on the same day.
B. Covered, sealed, and heated to prevent freezing.

I am writing to request a permit to construct new well and septic system be issued for
this proPerty. The initial request was made on July 5, 2006. Your department issued
two comments concerning the initial submittal on July 11, 2006, which I responded to
on July 14, 2005. On July 20, 2006 your department issued two additional comments. I
will address those comments in this letter in the same order they were presented.
Comment: The new site plan has test holes in different location.
Response: Two lot corners where found. The test holes were located off of these two lot
corners and the site plan revised accordingly.
Comment: The legal description we (MOA OD) have differs from yours. Can you check
this out?
Response: Based on the information obtained off the MOA Land Information web-site,
the legal description is Lot 18, T151~, RlW,
Design Elements
The proposed systems will serve a four-bedroom single-family house. Currently the lot is
undeveloped. The proposed well location is in the northeast corner of the lot, over 100
feet from the proposed septic system. There are no septic systems within 100 feet of the
proposed well location.
1. Soils. Two test holes were excavated and logged. See the attached soils logs. Each
test hole had a percolation test performed. Ground water was observed originally at a
depth of 12 feet in test hole one during the excavation and stabilized at a depth of 11

feet below ground level on dune 30, 2006. Test hole two was dry. Bedrock was not
encountered in the test holes. It is my opinion that the overall soils appearance of the
soils, an application rate of 1 gallons/day/square feet should be used using a
conventional treatment system.

4. Topography: The average topography in the area of the proposed septic system is
approximately 1 to 5 percent sloping to the northwest.
The proposed installation will not affect the future development of the surrounding or
existing lots. There are no wells or septic systems within 100 feet of the proposed septic
location.

Comment: Total Depth on site plan differs from the total depth listed elsewhere.
Response: Total depth on the site plan was clarified.
Comment: The site plan shall have a description of the topography area of excess slope,
and extent of slopes and surface drainage patterns within 100 feet of any part of the
system or portions of the lot intended for use for on-site wastewater disposal (AMC
15.65.030 D). Please provide this information on the site plan.
Response: The required infmmation was added to the site plan.
Design Elements
