1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska Wastewater
Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80).
3. The wastewater code requires inspections during the installation. The engineer must notify the Development Services
Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
Special Provisions:
1. A seive analysis is to be done prior to the construction of the septic field. Please submit
results with the As -built Inspection Report. If the results require a design change, construction of
the system will stop pending On -Site review and approval.
2. CO #1: The Engineer needs to do an additional test hole prior to the construction of the septic
field. Construction may proceed at your own risk before the 7 day water monitoring is complete.
Please submit stamped and signed results with the As -built Inspection Report. If the results
require a design change, construction of the system will stop pending On -Site review and approval.
3. CO #1: If the lot is not developed, any wells located on the lot are to be decommissioned.
4. Two well logs must be submitted to close this permit.

All construction must be in accordance with:
1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska Wastewater
Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80).
3. The wastewater code requires inspections during the installation. The engineer must notify the Development Services
Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
Special Provisions:
Submit a Well Decommissioning Log from a certified pump installer or well driller stating that the
well was decommissioned per code for all wells on the lot that will not be in use.
Special Provisions: -
A seive analysis is to be done prior to the construction of the septic field. Please submit results
with the As -built Inspection Report. If the results require a design change, construction of the
system will stop pending On -Site review and approval.
Special Provisions: CO #!: If the lot is not developed, any wells located on the lot are to be
decommissioned.
Special Provisions:
CO #I: The Engineer needs to do an additional test hole prior to the construction of the septic
field. Construction may proceed at your own risk before the 7 day water monitoring is complete.
Please submit stamped and signed results with the As -built Inspection Report. If the results
require a design change, construction of the system will stop pending On -Site review and
approval.

The owner has requested we modify our design for a well and septic permit on
the subject lot due to the lack of water in the first well location. The general slope
of this lot is from northeast to southwest at a grade of approximately 0-10%.
The well location has moved thus also moving the house and septic system
outside the proposed well radius. The new septic location is just outside the radii
of the testholes that were performed on May 29, 2012, which were submitted and
approved. If water is obtained in the new well location we will then excavate a
new test hole to confirm conditions for the septic system location. Water was not
noted at time of excavation or during monitoring. We propose to install 5' wide
shallow trench system.
Adjacent properties are served by public water. There is no surface water within
100' of the proposed system and there are no known curtain drains within 50'.
We do not expect there to be any adverse effect on adjacent lots by the

on the subject lot. The general slope of this lot is from northeast to southwest at a

We have designed the 4 -bedroom system utilizing the testholes that were
