From the time approval was granted there has been a change of owners and now the construction is to proceed. Attached is the original approval letter for a new Aerocell Treatment System with the 1,500 gallon treatment tank and the 2,000 gallon septic tank and a field of 300 ft2 mounded disposal field. All conditions in the prior approval still apply. On July 1, 2011 I called and electronic mailed to you the reapprove for this project. Please proceed as the replacement with the new septic tank of 2,000 gallons and the onsite disposal field of 300 ft2 per the design is needed. This approval does not imply the granting of additional authorizations nor obligate any state, federal or local regulatory body to grant required authorizations. Any person who disagrees with this decision may request an adjudicatory hearing in accordance with 18 AAC 15.195- 18 AAC 15.340 or an informal review by the Division Director in accordance with 18 AAC 15.185. Informal review requests must be delivered to the Division Director, 555 Cordova Street, Anchorage, Alaska 99501, within 15 days of receiving the decision. Guidance information on the informal review process may be found at http://www.dec state akus/commish/ReviewGuidance htm Adjudicatory hearings requests must be delivered to the Commissioner of the Department of Environmental Conservation, 410 Willoughby Avenue, Suite 303, P.O.
