Dear Mr. Crewdson:
This letter is to request a conditional approval for the on-site septic system at the above-described
property constructed under the permit number OSP 121174. The owner of the property plans to continue
building the house and garage foundations this year or next. At that time the owner will also complete
the construction of the sewer lines to the tank and the respective house and garage foundation cleanouts
(one each) of the sewer system.
Anticipated future submittals after above-described construction are listed below.
1. Scaled plan view drawing with swing ties of all standpipes, monitoring tube, and the on-site well to
Jp1Y Q`(i corners of the house and garage.
hUl 2. Profile drawing showing foundation cleanouts, finish grade elevations, and sewer pipes to the tank
'Odp� with invert elevations.
3. Driller's Well Log.
4. Pump Installation Log.
5. Well water quality laboratory report.
Attachments to support this request for conditional approval are listed below.
I. The inspection report for the septic system.
2. As -built drawings with swing ties from the constructed on-site system cleanouts and monitoring
tube to two well heads. One well was completed per Permit OSPI21174 on this lot and the other
well is on neighboring Lot 3 Block 3 Roseberry Park Estates.
3. Profile drawings of the septic tank and drainage field.

Septic System Design Permit and Well Permit Application
To On-site Services Engineers:
We respectfully request your review and approval to construct the subject well and septic
system. Attached are:
1. A Site Plan and Septic Design Plan (Figures I and 2) with stamp and original
signature of engineer.
2. Design and calculations with stamp and original signature of engineer.
3. Results of soils, percolation, and water table test with stamp and original signature
of engineer.
This septic system design is for 5 bedrooms to be served by a private well. The owner
intends to construct a new 4 -bedroom home and an accompanying garage/shop with
lavatory facilities. We are proposing installation of a 1500 -gallon septic tank and a 5 -
wide trench type drain field. The attached Site Plan and Septic Design Plans, Figures I
and 2, respectively, include the existing adjacent property well and the proposed well for
this lot as well as the location of all soils and percolation test pits we performed for this
design application. No other wells exist within 200 feet of our proposed septic system
design.
We excavated two test holes and performed two percolation tests whose 30 -foot radii
encompass both a primary site and an alternate site. We also excavated a test pit to 14
feet deep to identify soils, verify no bedrock, and to place a 4 -inch perforated vertical
PVC tube for ground water monitoring. The deep pit serves both the primary site and
alternate site with respect to questions of groundwater and bedrock existence.
The topography is generally sloping north to south to approximately 10 percent. This lot
is partially covered by gravel fill from 1 foot to approximately 4 feet deep. All our test
holes were placed in native undisturbed soil outside the fill limits.
Soils below an approximately 1 -foot thick surface organic layer consist mostly of sands
and gravel, occasional cobbles and small boulders, and a little silt. Please see the
attached soils logs and percolation test results.

Surface waters are more than 100 feet from the proposed and alternate design locations.
Figure 1 indicates the creek that runs northeast to southwest through the subdivision by a
solid line with lengths and bearings. Various line types in the 100 scale drawing indicate
building setback, septic system setback, and creek maintenance and protective easements,
as well as adjacent property lines.
This installation creates no adverse effects on adjacent wells or septic systems.
