must be either: A. Open and closed on the same day.
B. Covered, sealed, and heated to prevent freezing.
5. The following special provisions.
WHEN THE WELL HAS BEEN COMPLETED AND SATISFACTORY WATER PRODUCTION HAS BEEN
DETERMINED, THE WELL DRILLER SHALL PLACE AN APPROVED SANITARY SEAL ON THE WELL HEAD AND
NO PITLESS ADAPTER OR PUMP SHALL BE INSTALLED UNTIL AN APPROVED WASTEWATER DISPOSAL
SYSTEM HAS BEEN CONSTRUCTED.

To whom it may concern:
The existing Lot 2, USS 3042 is to be subdivided into 3 lots. Our client is proposing to build on
proposed Lot 2A, Stewart subdivision. The property is to be served by a private well and public
sewer. The well is to be drilled in the approximate location shown on the attached site plan. Also
shown on the attached site plan are possible well locations for Lot 2B and Lot 2C, both lots are to
be served by public sewer. We request that your department issue a well permit for proposed Lot
2A, Stewart subdivision.
I am unaware of any adverse impacts this installation would have on adjacent wells or septic

These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for

Ref: Sewer Line to Water Line Separation Distance for USS 3042 Lot 2A,
To whom it may concern:
It is not possible to verify the separation distance between the private sewer line, and the water
service line with out the lines being professionally located. Per the AWWU connect card for the
property and the location of the well it is likely that the sewer line is 25 feet away from the well.
It appears that the separation distance between the water amd sewer service lines is unknown.
Water sample results indicate nitrate levels at 0.518 mg/L is well below the maximum level of
