Box 196650 Anchorage, Alaska 99519 Subject: Rabbit Creek View & Heights, Block 8H, Lot 14A Well and Septic System Permit Request Ladies and Gentlemen: I am writing to request a permit to install a well and septic system be issued for this property. The proposed systems will serve proposed three (3) bedroom house. Currently the lot is undeveloped. The proposed system will utilize a 1500g S.T.E.P. tank with an AX20 Pod and Auxiliary Pump Vault. The surrounding lots are served by private wells. There are no wells on surrounding lots within 100 feet of this system. Soils. One test hole was performed by Pannone Engineering Services in November of 2013, and groundwater was monitored for seven days. Ground water was observed at one foot (1.0') below the ground surface after the seven day monitoring period. Per AMC 15.65.060 a drain field may not be installed where the water table during any season of the year will be within four feet of the bottom of the absorption area. As there are sufficient slopes for drainage from this location to sea level it is not possible for ground water to rise above 0' at any time of the year, providing four feet separation at seasonal high ground water. Based on numerous test holes and ground water monitoring performed in the area by PES it is highly unlikely that ground water will rise above its current level at any time of the year. It is my opinion, based on the results of the percolation tests and overall soils appearance; an application rate of 2 gallons/day/square feet should be used for a CAT III wastewater system utilizing ERS. See waiver Request. 1. Soil Absorption System Design. a. See Sheet 2 of the design package. 2. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain field. The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage ditches. 3. Topography: The existing topography slopes from the south to the north at approximately 5% - Mailing: P.O. There are no steep slopes in the vicinity of the drain field. 4. Waiver Request for use of Engineered Receiving Soil (ERS). I am requesting a waiver to the 1992 Code to use the proposed Code Update Section of Engineered Receiving Soils (ERS). Below I will detail my request justifying the use of ERS: a. One test hole was excavated by PES in 2013. The soils logs are attached. Ground water was monitored. b. This design proposes to use two types of ERS. The base soils that will act as an interface between the existing soils and the soil absorption system (SAS) will be MoA Type II or Type IIA, which has a specified gradation per MASS. This interface will be approximately three feet in depth and will be compacted to approximately 80 percent to allow free flow of the effluent through it into the native soil using MoA Waste Water Code application rates. The infiltration area of the interface between the ERS and the in-situ soil was based on the 30.3 minute per inch percolation rate of the in-situ receiving soil. A two foot thick filter sand layer will be placed over the MoA Type II material to act as an interface between the SAS and ERS. c. The SAS is sized assuming a percolation rate of the ERS soil, assumed 6-15 MPI, which gives an application rate of 4 GPDSF. d. The system will protrude above the ground surface. The cover of the system will produce a mound, which will have at a minimum, three to one side slopes and six inches of organically rich soil to promote the growth of vegetation. 5. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification that the review is complete and that there are no further comments is received from MoA On -Site Department, the note will be removed and "Issued for Construction" drawings will be issued. The proposed installation will not affect the future development of the surrounding or existing lots. There are no surrounding wells within 100 feet of the proposed septic location. Comment: Drainfield — Groundwater Separation: Please provide 6 feet. Response: Water monitoring on February 18, 2014 shows the water to be 1.0 feet below ground level. The bottom of the drain field is four feet above the ground, which provides for a five foot separation to ground water. The intent of requiring a six foot vertical separation to ground water in off peak months is to acknowledge that seasonal high ground water fluctuates. Using engineering judgment is always a must. If the ground water were to rise two feet, it would be surface water one foot deep. This lot has a constant 20 percent slope for many hundreds of feet below this site. It is highly unlikely that surface water one foot deep would occur. Therefore I request judgment be used in applying this "rule of thumb" to ground water separation and your Department allow the drainfield to be constructed as designed with five foot separation to a February ground water monitoring. Definitions. Alteration. Any change to the design or function of an AW WTS that includes the installation or removal of any parts, components or pieces not included in the original construction permit and design. Certificate of On -Site Systems Approval (COSA). An approval by the Municipality of existing water and wastewater disposal systems given at the time of property sale and title transfer in accordance with Anchorage Municipal Code (hereinafter, "AMC") 15.65. These approvals certify that Cod the systems are adequate for the homes that they support and meet the codes that were in place at the time of system construction. Damage. Any man-made or natural change in a system that would inhibit the system from performing as designed. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AWWTS that were included in the original design which would allow the AWWTS to continue to perform as designed. Permit. An On -Site Wastewater Disposal Permit as required by AMC 15.65 to construct and operate an AWWTS. 3. Term. The term of this Maintenance and Repair Agreement shall begin on the date of approval by the Municipality to operate the installed system or issuance of a COSA, and shall continue while the AWWTS is in use or is operational or until the property is sold or title is transferred by the owner and a new COSA is issued to the new owner or transferee of the property. 4. Alterations. Installation and Removal of Additional Equipment Prior to performing any alterations to an AWWTS, the owner agrees to obtain an On-site Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65. 5. Maintenance and Repairs. A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall maintain their AWWTS in a satisfactory condition capable of producing treated septic effluent in accordance with the equipment's approval for operation in the Municipality. The owner shall enter into a service agreement with an AWWTS Co �- service and maintenance provider approved by the municipality and the manufacturer of the AWWTS for the entire term of the AWWTS.. In addition, it shall be the responsibility of the Owner during the term of this Maintenance and Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs. B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules and orders for the AWWTS. C. Upon request by the Municipality, the owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system. When a record of maintenance is documented and maintained by the system vendor, the owner agrees to allow the Municipality access to this information. D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be assessed in accordance with AMC 14.60 for improper discharge. E. Owner agrees that only maintenance and repair personnel approved by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. F. Owner agrees to grant the Municipality reasonable access to test and inspect the AWWTS upon 24 hours written notice. G. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On -Site Systems Approval. �I H. Owner agrees that the AWWTS installation and maintenance requirements as provided by the AWWTS vendor/installer and approved by the Municipality are the governing guidelines for the construction, maintenance and repair of the Owner's AWWTS. 6. Nonwaiver. The failure of either party at any time to enforce a provision of this Maintenance and Repair Agreement shall in no way constitute a waiver of the provisions, nor in any way affect the validity of the Maintenance and Repair Agreement or any part hereof, or the right of such party thereafter to enforce each and every provision hereof. 7. Amendment. A. This Maintenance and Repair Agreement shall only be amended, modified or changed by a writing, executed by authorized representatives of the parties, with the same formality that this Maintenance and Repair Agreement was executed with, and such writing shall be attached to this Maintenance and Repair Agreement as an amendment. As this has been extensively documented in emails, I will further present the issues and work performed here. This system was installed in the summer of 2014.The contractor placed poor draining soils (silts and clays from the building foundation excavation) around the ERS system as there are no requirements for the material type or quality to be placed around an ERS mound—this material is generally considered unclassified and incidental to the work. This impervious soils concentrated the effluent within its bounds. On August 27, 2018, Mr. Ben Schiller, P.E. performed an adequacy test of this field.The field was dry at the start of the test and as water was added, the liquid levels rose to completely fill the field.The water levels would lower over time, but not at an acceptable rate. Mr. Schiller, P.E. declared the system in failure. He contacted me to discuss the failure and handed the project over to me. I suspected the cause of the failure was hydraulic mounding due to slow percolating soils and the confining impervious layers surrounding the ERS system. On September 8, 2019, the Owner hired a contractor to remove the silty unclassified fill from the downhill side of the ERS Mound down to the silty gravel receiving soil.The ERS Structure was not touched or disturbed during the process. Clean granular fill (SW/SP—Similar to MoA Type II but without the analysis)was placed on the downhill side of the ERS structure at a 45%slope downward from the top of the distribution pipe.The removed material was placed over the clean granular fill and placed at a 3:1 slope.Six inches of topsoil was placed over the unclassified fill. The Topsoil was Hydro-seeded on July 19, 2018.All work was performed under the supervision of PES's Designer and Field Technician. I was regularly consulted and updated with the status of the project. PES's Field Technician presoaked the field with 750 gallons of water and then added an additional 450 gallons (for a total of 1200 gallons) to test the field.The water levels in the field remained dry during the entire process.
