To whom it may concern:
The subject lot had a permit issued on 5/14/2014 that has expired. Per the request of the owner
(Martin Bellerive), he would like the permit re -issued. As can be seen on the attached design
drawings, the proposed structure has changed as well as the drainfield design. The revised
design consists of a 3 -bedroom shallow trench type drainfield and a 1000 -gallon HDPE Greer
tank. In addition, we are revising the slope waiver drawing with the "steep slope" provision of the
onsite wastewater code. Per the "steep slopes" provision, filter sand is being used to a depth of
3 feet (36 inches) below the organic layer. Attached is a profile drawing which shows a line
drawn two (2) feet above the top of drainrock at 45% to the "steep slopes". As can be seen on
the drawing, if the bank was as tall as 10 feet, the line does not "daylight".
It should also be noted that during a GEG site visit on 12/31/2019 TH#4 could not be located.
Prior to the installation of the system, TH#4 must be located to ensure the proposed drainfield is
within the 30 -foot radius. If it cannot be located, a surveyor must stake the location prior to
installation of the drainfield.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic

Septic System Owner-instalier Agreement
The On-site Water and Wastewater Section (On-site) may issue an approval for ahomeowner
to perform work on an on-site wastewater disposal system to serve that individual's owner-
occupied, single-family or duplex home if the homeowner meets and agrees to the following
requirements;
1.The property owner and excavation equipment operator may perform work on no more
than one owner-installation project in a12-month period.

Ref: Proposed Septic System for Kasilof Hills; Block 1, Lot 1;
To whom it may concern:
The proposed 4 bedroom house will be served by a private well and septic system. We are
proposing to install a 1500 gallon septic tank and four (4) 5-wide trench type drainfields. The two
(2) 45 foot long trenches are connected to serve 37.5% of the flow per trench (75% total) and
the two (2) 25' long trenches are connected to serve 12.5% of the flow per trench (25% total).
Attached are a site plan and a design drawing showing the proposed septic system. Four test
holes were excavated on the property. The drainfield will be designed around the 30 foot radius
of testhole #4. Comments regarding the design are summarized as follows:
1. SOILS: See the attached soil logs which show the soil classifications, groundwater
monitoring, and the percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface water concerns.
4. TOPOGRAPHY: As can be seen on the attached topography drawing, the average
topography around the proposed drainfield is 15% running east to west.

Ref: Request from Proposed Drainfields to Cutbank (Road Ditch) for Kasilof Hills, Block 1, Lot 1;
To whom it may concern:
We are requesting a 23 foot waiver be granted from the proposed drainfields to the cutbank. As can
be seen on the attached drawing, at a 25% slope (starting 2.5 feet above the drainfield) the potential
point of daylighting is greater than 50 feet horizontally from the drainfields. The following are mitigating
factors for the justification of these waivers:
· The road ditch is in a highly visible area. If effluent were ever to overflow, it would be recognized.
· The slope of the cutbank is highly vegetated. This would serve to impede effluent from daylighting.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic
