Aquifer Study
Dear Jay:
Woodhaven Subdivision is composed of 13 Lots and 2 tracts and is located off of
Elmore Road near Huffman Road in Anchorage. It is surrounded by developed lots
served by both the AWWU water system and on site private wells. The lots proposed for
the subdivision are in excess of 40,000 square feet and are proposed to have on site
wells and septic systems.
Three wells are proposed for placement in the subdivision on proposed Lots 6, 7 and
8. The wells will be placed at sites which will represent standard locations relative to
septic system sites and home sites on typical lots in the subdivision. Each well will be
pumped in accordance with AMC 15.55.060F1 to establish the sustained yield as
determined by the well driller. Water quality testing from each well will be accomplished
in accordance with AMC 15.55.060F2. A formal report including sustained yield results
for each well will be submitted once all data is gathered and analyzed.
Permit applications are attached for placement of the three wells in the subdivision. We
hereby request your approval to complete the well and water study to determine the
capacity of the aquifer to support the proposed wells for the subdivision.
The location of the wells and the proposed location of the houses and septic systems on
the lots will not impact adjacent properties. If the wells and proposed septic systems
are constructed in accordance with our design the following statements apply:
The well, if constructed as designed, will have no adverse impact on the wells in
the area er those to be constructed in the future.

septic systems in the area or those to be constructed in the future.

5. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures
outlined in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or
wastewater disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated
herein. I further verify that based on the information obtained from the Municipality of Anchorage files and from my investigation and
inspection, the on-site water supply and/or wastewater disposal system is (are) in compliance with all applicable Municipal and State
codes, ordinances, and regulations in effect at the time of installation.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MCA
COSA guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time
of the test, and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on
the local soil condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not
guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot
provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole
benefit of the owner listed above. Reliance on this report by another person is at their own risk. Pannone Engineering Services LLC highly
recommends buyers hire their own engineer to evaluate this report.

ON-SITE WATER & WASTEWATER SYSTEMS INSPECTION REPORT
Per your request, we have inspected the on-site water and wastewater systems of the above referenced 4 -
bedroom property. This investigation was based on observations of the day in which the septic adequacy
and well flow inspections were performed and according to available Municipality of Anchorage (MOA)
record documents. This is to certify that as of the date of this letter, the subject on-site well appears to
meet or exceed minimum state requirements pertaining to water production and quality coli, arsenic and
nitrates (CAN).
The 5 -bedroom Advantex septic system constructed in 2017 consists of a 2500 -gallon steel septic lift
station tank with 2 AX -20 pods and 2 40' long trenches with 2' effective sewer rock depth. Based on the
MOA record docs (soils, groundwater, design,) -- it appears the system was installed correctly. Upon our
arrival to conduct the well flow and septic adequacy tests, it was observed that there was 22" and 28" of
standing fluid in both of the fields monitoring tubes. The western trench had fluid 4" above the lateral.
Based on these observations on the day we conducted our inspection, the fields are both currently
saturated and will not meet MOA COSA standards. Audible dripping sounds were noted in both
compartments when no one was inside the house. We were going to come back and remove the lids to
investigate these sounds, but were given a stop work order. It was also noted that the post tank / pod
cleanouts had heavy sludge and debris (looked like gravel) in them. There are several possible reasons
why the fields may currently be in a saturated state.
Although storm events, seasonal breakup / snowmelt, changes to drainage patterns and other events or
actions may contribute to the functionality and state of a septic system, there should be no direct
interactions of these types of events with the septic fields. Per our last conversation with your realtor, we
were going to monitor the field over the next several weeks, verify if the recent snowmelt was the
contributing factor of the fields being saturated and go from there to see if the system would meet MOA
COSA standards. However, we have since been requested by Pannone Engineering Services, LLC to
send them the water sample results which we took and can only assume that they are now completing the
COSA process.
If snowmelt / breakup is not the contributing factor of why we found the fields saturated at our inspection
— our first thought was that the tank lids may be leaking and allowing groundwater intrusion and

surcharging the field. If this is the case, we proposed having a contractor inspect the tank, conduct any
necessary repairs, monitor fluid levels over a couple weeks and with dropping levels demonstrate to the
MOA that the field will meet the COSA septic adequacy guidelines. If so, we believe the MOA will issue
the COSA. However if this were not the case, we'd recommend conducting a testhole to verify
groundwater and soils in preparation for a new field. Please let us know if you have any questions or how
we can be of service in regards to these septic findings.
Prior to commencing the well flow test, the static water level was measured at 43' from the top of casing.
The wellhead consists of 6" steel casing extending 1.5+ above grade with a sanitary seal and conduit.
The sanitary seal was jacked above the top of casing and needs to be addressed. The well produced 4.6+
gallons per minute (GPM) with a maximum drawdown of 22', which is well above minimum MOA water
production requirements for a 5 -bedroom single-family residence. The well log shows a total depth of
1.18', a static water level of 40' and a water production of 10 GPM. A water sample was drawn and
submitted to the lab for coli, arsenic and nitrate analysis (CAN). The results of this water analysis show
that the water meets state and municipal safe drinking water standards (CAN).
The assessment of the condition of the well and septic applies only to the conditions as of the day tested.
The absorption and flow rates may change due to subsurface conditions that may not be observed from
the surface, changes in land use, local soil characteristies, groundwater levels that may fluctuate during
the year and the water- usage of the family being served by the system. The operational life of all well and
septic systems is subject to these various and dynamic characteristics and are outside the control of the
evaluator of the septic system. Therefore, we cannot give any estimate of how long a system will
function satisfactory for current or future occupants or can we guarantee that no unseen encroachments,
deficiencies or discrepancies exist.
Based on this investigation, which was completed in compliance with state / municipal regulations and
industry standards, I have certified the adequacy of the water well. If after review of these records you or

I am writing in response to your comments concerning our Adequacy Test and COSA for this property. The field was
tested by First Water Consulting on April 28, 201. Their report indicated that the field was saturated and operating in
a surcharge manner. The report further states that there were "several possible reasons why the fields may currently
be in a saturated state".
PES conducted our adequacy test on May 12, 2021. The liquid levels were as noted on the COSA Checklist submitted
to your department (13 inches at the front and 17 inches at the rear of the field). No audible drips were heard during
our test. We did not observe any sludge in the monitor tubes.
The entire system was inspected by Anchorage Tank as part of the Advantex maintenance program at the end of
January, 2021. Their report indicated that the water levels were 17 inches at the front and 21 inches at the rear of
the field, similar to the readings we found. The Anchorage Tank report did not indicate the presence of sludge in the
field, nor dripping inside the tank.
Some reason for the water levels observed by First water Consulting could be the excessive snow melt working its
way through the soils. Anchorage had a near record snow fall this past winter. The Snow -tell sites in Anchorage bowl
(Snow -Tell sites record the snow packs moisture content in an attempt to predict the anticipated run-off) recorded
the highest ever level of moisture in the snow pack in late April in Snow -tell history in this year's snowpack. This
combined with the rapid melt off has led to areas of Anchorage becoming saturated for a short duration giving the
appearance of failed septic fields. The level of moisture running through.the soils are not unlike the effects of a
dripping faucet on the drain field — the field becomes saturated with excessive moisture. Soils are only able to absorb
so much water per day — thus causing the soils to become super saturated as water accumulates to be absorbed. The
soils are silty sands, having a percolation rate ranging from 60 minutes per inch to 2.5 minutes per inch, thus
indicating a highly variable soils. Further, Anchorage Tank did not note any deficiencies when reporting the current
water levels of the monitor tubes or within the Advantex system.
I believe the high levels of liquid in the drain field observed by First Water was due to the excessive moisture running
through the soil. PES does not believe that this year's super saturation of surrounding soils will create a problem for
the future owners. Please contact me if you have any questions.

THIS MAINTENANCE AND REPAIR AGREEMENT, herein the "AGREEMENT" made and
entered into as of this 6th Day of May of 2021 , by and between

Anchorage, herein the "MUNICIPALITY", in accordance with Anchorage Municipal Code
(AMC) 15.65.365. In consideration of the mutual covenants contained herein, the parties to this
Agreement agree as follows:
I . Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS),
described as 3850 Marcel -le Circle, Anchorage AK 99516

2. Maintenance, Repairs and Alterations.
(Owner is required to read, understand and initial each section)
W Throughout the term of this Agreement, the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.
W It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This
includes an annual maintenance fee (typically $400 to 5600).
W Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance,
repairs or permitted alterations to the system.
M3 Owner acknowledges that regular- maintenance of an AWWTS reduces the potential
failure of the system, which could include sewage backup and costly repairs or drainfield
replacement.

by the AWWTS vendor/installer and approved by the Municipality are the governing
guidelines for the construction, maintenance and repair of the Owner's AWWTS.
�"I� Owner agrees to maintain remote monitoring of the AWWTS as required by the
AWWTS approval.
3. Term. The tenn of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions, nor in any way affect
the validity of the Agreement or any part hereof, or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.

As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures outlined
in the Certificate of On-Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or wastewater
disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated herein. I further verify that
based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply
and/or wastewater disposal system is(are)in compliance with all applicable Municipal and State codes, ordinances,and regulations in effect at
the time of installation.
In conducting an adequacy test,I attempt to provide a thorough,conscientious engineering analysis of the system in accordance with MoA COSA
guidelines and regulations.The reported results describe the performance of the system under the conditions encountered at the time of the test,
