Bragaw Street Anchorage, Alaska Re: Mr. David VanOrd, Owner T15N, R1E, Sectionl, NW4SE4SE4 37350 Eklutna Lake, Eklutna Lake, Alaska PERCOLATION TEST RESULTS AND GENERAL SITE INVESTIGATION REPORT On August 27, 2012, the above -referenced 10 -acre property was inspected for application and approval of the installation of two on-site wastewater disposal systems and two wells, one well and septic system for a 50' X 80' shop situated in one area of the lot and the other well and septic system in another area for a four-bedroom residence. The property is located at mile 7.9 of Eklutna Road. The immediate areas that have been selected for both wastewater disposal systems have an average slope of 1 to 2%. Both areas are moderately treed with birch, spruce, cottonwood and alders, but the trees surrounding the residence area are closer to the site. This site also has the greatest variation of slope. The rest of the property is heavily treed and also the slopes vary greater than 1 to 2%. Regarding the residence site, on-site observation and physical survey show that there are no water wells or private wastewater disposal systems within a 100' radius of the proposed system. No surface water was observed at the time of the inspection, and it appears that there is no potential for contamination of adjacent water wells or streams. On August 28, 2013, Test Hole #2 was dug to a depth of 17.2 feet and a percolation test was performed to assess the adequacy of sub -surface soils to accommodate a septic on- site wastewater disposal system and placement of a well for the proposed four-bedroom house. The results of this test are on page 6 of 6 of the well and septic design. The residence tank was installed on August 30, 2014, without a permit, but with the permission of Jay Crewdson and Jeff Poet of M.O.A. On-site. The shop tank and leach field were installed at the same time with a permit. Mr. David VanOrd Page 2 August 25, 2015 The proposed system will have no measurable impact on reserve space, surface or subsurface soils, or on drainage to adjacent lots. Please consider this report a request for review and approval of the attached design drawings for an on-site wastewater disposal system and well for a four-bedroom residence. Bragaw Street Anchorage, Alaska Re: Mr. David VanOrd, Owner T15N, R1E, Sections, NW4SE4SE4 37350 Eklutna Lake, Eklutna Lake, Alaska PERCOLATION TEST RESULTS AND GENERAL SITE INVESTIGATION REPORT On August 27, 2012, the above -referenced 435602 sq. ft. property was inspected for application and approval of the installation of two on-site wastewater disposal systems and two wells, one well and septic system for a 60' X 100' shop situated in one area of the lot and the other well and septic system in another area for a four-bedroom residence. The property is located at mile 7.9 of Eklutna Road. The immediate areas that have been selected for both wastewater disposal systems have an average slope of 1 to 2%. Both areas are moderately treed with birch, spruce, and alders.. The rest of the property is heavily treed and also the slopes vary greater than 1 to 2%. Regarding the shop site, on-site observation and physical survey show that there are no water wells or private wastewater disposal systems within a 100' radius of the proposed system. No surface water was observed at the time of the inspection, and it appears that there is no potential for contamination of adjacent water wells or streams. On August 27, 2012, Test Hole #1 was dug to a depth of 16 feet and a percolation test was performed to assess the adequacy of subsurface soils to accommodate a septic on-site wastewater disposal system for one bedroom and placement of a well for a proposed shop. The results of this test are on pages 1-2 of 8 of the well and septic design. The proposed system will have no measurable impact on reserve space, surface or subsurface soils, or on drainage to adjacent lots. A 12 -inch wide strip of of bituthane was installed around the perimeter of each tank. The bituthane was installed such that approximately 6 inches wrapped up over the top of the tank lid and 6 inches extends down the sidewall of the tank. Prior to backfilling and covering the tank 2 inches of rigid insulation, overlain with 2 layers of 6 mil of plastic sheeting, were installed over the top of each tank. A total of six photographs are attached to this letter for your records. Three photographs are labeled with the serial number for Tank 151201. An additional three photographs are labeled with the serial number for Tank 151202. Thank you for your assistance in completing the installation of this sewer system. CPM 2.10 Engineer's Cover Letter: Please provide an engineer's cover letter that addresses the following items. --a nef explanation of the design intent and methodology, fib. Discuss sealing the lid to the tank body, and the lid opening covers to the lid, LL,lc6,,—Call out the two subject tanks by serial number. �.6. Lid Openings — Reinforcement: The drawings provided are not clearly describing how the lid openings are reinforced and the structural calculations do not appear to take them into account either. Please substantiate and detail the lid opening structural design. 11,�,-'Tank Bottom & Sides — Reinforcement: The reinforcement design appears to have been revised from 4x4 - 4.0/W4.0 to 6x6-W4.01W4.0, but the design drawings do not appear to have been revised. Please clarify. 8. Sealing: It appears the tank lid is intended to be sealed to the tank body by placing a gasket (preformed or calking) between the tank lid and the upper edge of the tank body. Please substantiate how the seal will be established near the tank lifting eyes. Note: per the photos, there doesn't appear to be enough of a tank edge around the fling eyes to provide a seal. FYI (no response required): The engineer references UPC Appendix I for the "Septic Tank Checklist". Appendix K is the correct reference. OZ --FYI (no response required): The following test procedure will be included on the permit to construct and shall be lowed to ensure water tightness. Ref. CPM 2.13(a) 11. On-site shall be notified at least two hours prior to the beginning of the test for inspection and documentation rposes. 2r the septic tank has been set into the ground fill the tank to its brim with water and let it stand for 24 hours. r-�3.Measure the water loss; if there is no water loss during these first 24 hours the test is acceptable. If however the water level dropped, some water absorption may have occurred. If so, refill the tank and determine the exfiltration by measuring the water loss over the next two (2) hours.
