4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
must be either: A. Open and closed on the same day.
B. Covered, sealed, and heated to prevent freezing.
5. The following special provisions.
BOTH THE 500 GALLON AND THE 1250 GALLON TANK NEED TO BE EXPOSED AND VERIFIED OF THEIR
INTEGRITY. IF ANY OF THE TANKS ARE FOUND TO BE IN FAILURE, THE TANKS AND CONFIGURATION OF
TANKS NEED TO BE INSTALLED ACCORDING TO TODAYS CODE.

Ref: Septic Design for Loam Estates S/D; Lot 6A, Block 3,
To whom it may concern:
The existing 4 bedroom house is served by a private well and septic system. The north and south
trenches where tested by G.E.G. on 10/13/04. The south trench absorbed 425 gallons during test
and the north trench was found to be completely surcharged. Per the owners request, we are
proposing to rebuild the existing north trench. A test hole was performed by S&S engineering on
9/1/1996. We are proposing to excavate and remove all contaminated material from the existing
· north trench and rebuild it to the existing or higher elevation. We are also proposing to expose
both the existing 500 gallon and 1250 gallon septic tanks to verify their integrity. If the tanks are
found to be structurally unsound, we will install a new 1500 gallon tank. Comments regarding
the design are summarized as follows:
1. SOILS: Attached is a log, which shows the soil classifications, groundwater monitoring, and
the percolation test results.
2. TRENCH DESIGN:
a. Pemolation Rate: 2 minutes/inch
b. Allowable Application Rate: 1.2 gallons/day/ft2
c. Design Flow: 600 gallons per day
d. Minimum Absorption Area: 262.5 fi2
e. Total Depth: Removal of all contaminated material and then rebuild to existing trench
elevation or higher.
f. M.O.A. Approved Sand Filter: 1 '+
g. Effective Depth: 0.5 feet
h. Width: 5 feet
i. Minimum Length: 52.5 feet long 2
j. Effective absorption area = 262.5 ft
3. SURFACE WATERS: There is no surface water within 100 feet of the proposed septic
system upgrade.

Dear Mr. Harper:
Your request for waiver of the required 40 feet minimum well depth has been
approved. The minimum allowable depth at which the perforations may begin is
30 feet from the original natural ground elevation.
Justification for this waiver is granted in part by the well logs supplied by you
and the property owner. It appears that the only appreciable water to supply the
water wells in the immediate area is at a lessor depth than the required minimum
well depth.
This waiver of the required minimum well depth applies to this well only. Any
future upgrade to this well or construction of another well on the property will
require the minimum allowable depth of 40 feet or another waiver from this
office.

Request you issue a permit to install a septic system to
serve the proposed four bedroom house on the referenced
property.
Test holes were excavated and percolation tests performed.
The approximate location of the test holes are located on
the attached site plan.
At the time of excavation, water was encountered at 7 feet,
in test hole %1, at 4.5 feet in test hole ~2, and at 8 feet
in test hole ~3. After seven day ground water monitoring,
the monitoring tubes in all the test holes had remained at
their same water level.
This property has enough area for a future septic upgrade
which can be seen on the attached site plan.
We do not anticipate any adverse effects on neighboring
wells, septic systems or drainage patterns by the
installation of the proposed septic system.

conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of th¢ test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.

Per the request of your agent, we inspected the well and septic system, which serve your property
for the purpose of obtain/rig an MOA Health Authority Approval (HAA). A site visit was
performed on 10/13/2004. Our findings are smmnarized as follows:
WELL: Prior to the start of the flow test the static water level was 24 feet below the top of the
well casing (BTC). The well is approximately 206 feet deep per the MOA records. Water was
metered fi:om a hose bib at a rate of 2.86 gallons per minute (547.5 gallons) for 191 minutes,
during which time the water level in the casing dropped to 33 feet BTC. Based upon this data, it
was determ/ned that the production rate of the well is approxi~nately 2.86+ gpm, which exceeds
the MOA requirements for a four (4)-bedroom house, which is 0.416.gpm.
WATER QUALITY: Water quality samples were taken for nitrates and bacteria and sent to a
state certified laboratory to be analyzed. We will forward the results to you once we have
received them.
SEPTIC SYSTEM:
The septic system consists of two septic tanks and two drainfields that were installed in
1998. Upon inspection it was determined that the lower drainfield was completely
submerged urlder water and deemed to be failed. The upper drainfield has 4.75 inches of
standing liquid in it (79% full). Over a period of 191 minutes, 338 gallons of water was
added to the drainfield which caused the liquid level to rise 4" inches (84.5 gallons per
inch). Fifteen & one half (15.5) hours later the level had dropped 3.25 inches, which
corresponds to an absorption rate of 425 gallons per day, which does not meet the MOA
absorption requirement ofa~r gallons per day. Although the system may be functional,
it is by MOA definition technically failed. Per our discussions, the most economically
viable options are to eitherL~c~/hemicallY rejuvenate the existing' dralnfield or re-build it.

The following is a summary of the engineering costs associated with upgrading the septic
system (should you choose that option):
Services to date for the well flow test, water quality samples, septic evaluation,
septic tank pumping and preparation of this report: $990.00
M.O.A. sewer upgrade permit: $460.00* Payable to the M.O.A.
Preparation of the design package: $600.00.
Engineering services to inspect the installation of the septic system and prepare
as-built drawings~ Assuming a conventional gravity flow septic system with a
single trench/bed and no sand tilter: $700.00. Upon request, we can provide you
with our standard rate sheet, which lists the inspection costs for other types or
configurations of septic systems. This information is also available on our

Upon completion of the installation we will need to prepare the MOA health certiticate
paperwork and deliver it to the MOA (GEG preparation/delivery fee of $150.00). The
MOA will charge $430.00 to process the HAA.
