Lot 6 well was inspected with video camera to a depth of 80.5'. No perforations
were noted'abeve 43' below top of casing. Well was flow tested at 15 gpm for 4 hours
with a drawdown of 46' total and a beginning static water level of S' 8". In subdivision
approval flow tests this well was designated as well #3.

3. The engineer must notify DSD at least 2 hours prior to each inspection. Provide notification by calling
(907) 343-7904 ( 24 hours ). ( Not required for a Water Supply Permit only ).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
must be either: A. Open and closed on the same day.
B. Covered, sealed, and heated to prevent freezing.

THE WELL SHALL BE CONSTRUCTED IN ACCORDANCE WITH AMC 15.55.060.

Ref: Exploratory Well Permits for a new subdivision to be known as Creek View Estates
(subdivision of Tract B, Rexview Terrace S/D)
Mr. Roth:
We have been hired by Mr. Jeff Regnart, the developer of this subdivision, to obtain exploratory well
permits and perform an aquifer study for the purpose subdividing the referenced property. Attached is a
site map that shows the majority of the subdivision with the proposed well and septic sites. There is an
existing well located on proposed lot 6. We are proposing that two additional wells be drilled and a well
drawdown test be completed using these three wells. We are proposing the well located on Lot 6 be
inspected and documented. If the well is found to not meet MOA well ordinance requirements, we would
propose a new well be drilled next to thi~ one and the existing well be abandoned per MOA code. We are
proposing a well be drilled on Lot 5 approximately 200 feet northeast of the well on Lot 6. We a~e also
proposing that a third well be drilled on Lot 4 which is approximately 140 feet north/northeast of the well
on Lot 5. We will require all wells to be surveyed and staked prior to drilling to insure proper separation
distances to the proposed platted reserve sites. These well locations were chosen for the accessibility for a
well rig. All other lots would require extensive grading and driveways to be installed in order to drill
wells on any other lots. We request three exploratory well permits be issued for the purpose of performing
an aqu4fer test (see attached permit applications).
We are unaware of any adverse hnpacts this installation would have on adjacent wells or septic systems.
