Addendum # 1
Reference On-Site sewer/Well Submittal Comment Sheet, Dan Roth, Dated: 10/10/01 and
received, 12/5/01, the following additional information is provided:
A revised plan view is provided with proposed distances between the following annotated:
1. Septic Tank to House Foundation.
2. Septic Tank to Trench.
3. Midpoint of Trench to Lot Line.
Per conversation, revised soils logs are attached and have been modified to show the
following:
1. An estimated elevation of the original grade.
2. Documentation that original grade was lowered, total depth of test holes, in relation to
original grade.
3. Relationship of elevation of percolation test to original grade.
Per conversation, the construction of this wastewater system will not preclude the
development of adjoining lots. The wastewater system will be more than 100 feet from all lot
lines except to the north. The wastewater system will be approximately 90 feet from the lot
line on Lot 2, Welsh Subdivision. This separation distance should provide sufficient distance
to develop all adjacent lots.
Per conversation, the existing wastewater system is undocumented and will be completely
abandoned after this system is installed. The septic tank for this system will be a new Greer
1,000-gallon septic tank.

Re: Walsh Subdivision, Lot 1, On-Site Wastewater Disposal System Permit Application
In accordance with AMC 15.65.030, the following information is provided in support of
an On-site Wastewater Disposal System Permit Application for the subject property:
The subject property currently has a 1 bedroom single family dwelling with an
existing well and an undocumented on-site wastewater disposal system. The
existing wastewater system was installed by unknown persons and thero are
no visible signs of septic tank cleanouts or any other cleanouts indicating the
type and size of system. This application is for a 3 bedroom single family
dwelling. However, thero is a possibility of the construction of an additional
single family dwelling and conversion of the existing dwelling to a bed and
breakfast. Therefore, a 150-foot protective radius around the existing well is
being maintained.
The property lies adjacent to Eklutna Road and is located in Section 35,
Township 16 North, Range 1 East, Seward Meridian. The lot is in the shape
of a triangle with the hypotenuse being the northeast lot line, which is
common, the Eklutna Road Right of Way. A Matanuska Electric Association
Electrical Easement exists along the northeast lot line.
The property is on the northeast slope of the Eklutna River valley and is
approximately 3 miles from Eklutna Lake. Eklutna Road is cut into the slope
and the property has a series of natural and manmade slopes and terraces
which step-down towards the Eklutna River. In general, the property slopes
southwest with a vertical cutbank of approximately 10 feet along the electrical
easement. This is followed by a 10% slope changing to a 25% slope then
back to a 10% slope and continuing with a 25% or more slope down the
plateau of the river valley. The area also slopes to the northwest
approximately 5 to 10 %.
4. The area, where the soils absorption system will exist is located on a terrace
adjacent to the cutbank along the electrical easement. There is sufficient

area for both the system being applied for, and a replacement system on the
terrace. Both systems will meet the slope criteria for trenches and will be at
least 50 feet away from a natural or manmade slope of 25% or more.
The soils found during the soils test were sandy silty gravel. No water was
observed in the test holes, or in subsequent monitoring. Some of the soils
were observed to have a layer of sand around the individual gravel, which
made the percolation rates vary. For purposes of this design, a percolation
rate of 10 minutes per inch was assumed. Based upon this percolation rate,
a loading rate of 0.8 gal/day/S.F, was assumed. The effective depth of the
trench will be 5.0 feet below the distribution pipe and will be at least 56.2 feet
long. This will allow the bottom of the trench to be at least 6 feet above an
impermeable layer or bedrock and at least 4 feet above seasonally high
ground water.
There are no existing wells or on-site wastewater disposal systems on any of
the adjacent lots. There are no wells, wastewater systems surface waters
within 200 feet of the proposed systems.
Based upon the data and drawings attached, a permit for this wastewater system may

Water Well Advisory
Certificate of On-Site Systems Approval (COSA)# 151340
During a recent COSA on-site inspection and test of the potable water
supply well on Block , Lot 1 of Walsh subdivision, the well's
productivity was determined to be 0.3 gallons per minute. The minimum
well productivity required by this Department (AMC 15.55) for a 2-be&'oom
residence is 0.2 gallons per minute. Although the subject well currently
exceeds this minimum requirement, all parties concerned are advised that the
production capacity of the well may fluctuate. Restriction of non-critical
water uses such as washing cars and watering lawns and gardens may be
required.
This advisory must be attached to all copies of the subject Certificate of On-
Site Systems Approval.
