All construction shall be in accordance with:
1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing
Special Provisions:
An additional percolation test is required prior to the construction of the septic field, so that the entire primary and
reserve fields are within the 30 ft radius of a percolation test. Please submit stamped and signed results with
the Inspection Report. If the results require a design change, construction of the system will stop pending On -
Site review and approval.

To Whom it may concern:
This is a request for a renewal of the septic and well permit. A test hole was excavated to 16 feet revealing
silty sandy gravel for the entire test hole depth. The perc rate was 28 minutes per inch with no water
observed during excavation but after 7 day monitoring the water was measured at the bottom of the hole, 16
feet. A simple deep trench has been designed with 7 feet of effective depth. The slopes at the septic area are
greater than 25% therefore fill will be added to the area from the house excavation to keep it less than 25%
for 50 feet. The site has been filled for the house therefore fill for the septic area will not be a problem, see
the site plan.
We are also requesting a slope waiver from the existing slope which is greater than 25 percent. The lot has
a lot of fill already and additional fill will be added to create slopes less than 25 percent down slope from the
proposed leach field, see site plan.
This well and septic system plus the waiver will not impact any of the neighboring properties due to the
large lot sizes.

filled therefore the system will be more than 50 feet from the steep slope, see attached drawing.
