Septic System Upgrade Permit Request
Design Narrative
This is a design narrative for a permit to install an upgrade/repair to the existing septic system to be issued
for this property. The systems serves an existing three-bedroom (3) house. Currently the lot is developed.
This lot and the surrounding lots are served by private wells. The well on this lot and the surrounding wells
are over 100' from the septic system.
The system had previously been incorrectly documented as to the length, depth, effective depth and
overall construction of the system. During our investigation we snaked/camera'd both existing lines of the
field, conducted two percolation tests, and exposed various locations of the field to determine the
construction. Using the various percolation results and orientation of the field, it was determined that the
field as emplaced is sufficient to dispose of the household effluent, however, two repairs/modifications
need to be done. First is replace the Corrugated Low Density Polyethylene pipe currently used in the field
with 3034 PVC and second is replace the existing drain rock and replace with new. The field as installed
does not have filter fabric placed on it and the drain rock needs to be replaced. The modification of the
system is the emplacement of various cleanouts; monitoring tubes and double cleanouts as noted on page
1 of the design set.
1. Soils. Two test holes were performed on this lot by PES on 29MAR16 and 04APR16, and groundwater
was monitored for at least seven days on the initial test hole. Ground water was not observed to a
depth of 16.0' below the surface in the test hole monitor tube after the 7 -day monitoring period.
Bedrock was not encountered in the test hole. Based on the results of the percolation tests and overall
soils appearance; an application rate of 0.45 gallons/day/square feet was used for a conventional
wastewater system in the area of Test Hole 2 and 1.2 gallons/day/square feet in the area of Test Hole
3.
2. Soil Absorption System Design.
a. See Sheet 1 of the design package.
3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain field.
The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage
ditches.
4. Topography: The existing topography generally slopes down from the north to the south at a 2%
grade. The property slopes from north to the south in the area surrounding the existing septic system at
approximately 2%. There are no steep slopes within 50' of the proposed drain field. The proposed drain

field will maintain 50' separation all steep slopes.
The proposed installation will not affect the future development of the surrounding or existing lots. There
are no wells within 100 feet of the proposed septic location. If you have any questions or concerns, please
contact me at 272-8218.
We are also requesting a conditional COSAto install the system by 01AUG16 after the ground thaws to
save cost. This will have no negative impact on public health.

* Check of discharge point and wet weather overflow for blockage (if appli-
cable.)
• Inspection and adjustment of control panel setting and overload protection,
if there is access at time of inspection.

build-up warrants removal.

This is in response to your letter dated 9/4/90 regarding our request for a HAA certificate for Lot 1,
Block B, Rolling Hills Estates.
(1) Based on measurements through the monitor tubes, the north soil absorption trench is slightly
deeper than the more southerly one, with the bottom of the monitor tube being 117 inches below ground
level, or approximately 10 feet. The bottoms of the cleanouts are 5 feet below ground level, giving an
effective gravel depth of 5 feet. Since the as -built inspection report prepared by the Borough inspector
states that the depth form the top of the drainfield tile to finish grade was 3 feet at the time of installation, it
appears that subsequent owners have put a couple of feet of fill over the system in the course of
landscaping the yard.
(2) As confirmed by our site inspection on 6/6/91, the exposed portion of the service line from the
house has been thoroughly insulated as it enters the vault above the septic tank, as you requested.
During our inspection we did not observe any indication that a plumbing leak was causing excessive
water to be entering the tank from the dwelling, so the potential problem you noted last fall has apparently
been corrected.
Finally, since 9 months have elapsed since our original HAA submittal, we have obtained the enclosed
new water sample results for both coliform and nitrates, per your request. I trust that this written report
adequately addresses your concerns and that you can now complete the processing of the enclosed HAA
certificate, so that the new owner can close out his loan transaction.

Per our conversation of August 31, I am returning the Health
Authority Approval for the subject lot so that the following
items can be addressed:
1) Verify total depth of leach lines and depth of gravel below
each line.
2) Insulate the service line from the.house as it enters the
vault above the septic tank.
At the time of our inspection it was noted that water was
entering the septic tank from the dwelling.This water source
should be identified and fixed to prevent premature failure of
the septic system.
Please provide written confirmation that these items have been
adressed so that the Health Authority Approval can be processed.
