Ladies and Gentlemen:
I am writing to request a septic system upgrade permit for the above referenced property. The
existing septic system is no longer functioning adequately. The proposed system will serve a 3 -
bedroom single-family residence. Soil logs, design calculations, a site plan, design drawings and
construction specifications are enclosed for your review.
Design Cates: No ground water observed to a depth of 16 feet.
(6/8/2012).

12 min/in = 0.8 gal per sq.ft/day

Required Area per Bedroom: 150/ 0.8=187.5 ft
Total area required: 187.5 x 3 = 562.5 6
Shallow Trench with 0.5 feet effective depth
Trench Length: 562.562 /5'=112.5'
System: A 1250 gallon STEP tank and a pressurized shallow trench absorption field consisting
of 2 trenches 5 feet wide 60 feet long with 1 foot coarse sand and 0.5 feet of sewer rock.
Soils: Two test holes were excavated on June 1St, 2012. Seethe attached soil logs. Ground
water was not observed during the excavation or after the 7 day ground water monitoring period.
Ground water monitoring will continue through the construction of the system. A percolation
test was conducted at a depth of 3 feet in the vicinity of test hole #1. A percolation rate of 12
min/in was observed. The existing absorption fields have been in use since 1976 (trench 9 feet
of gravel) & 1962 (crib, unknown effect depth) that the indicating that the soils to a depth of 9
are not impermeable.

Surface Water: There are no surface waters within one hundred feet of the proposed septic
system upgrade.
Topography: The ground level slopes down towards the south west at approximately 5%-10%
grade. There are no slopes or ditches walls greater than 25% within 50 feet of the proposed
absorption field.
Waivers: None.
The installation of this septic system will not prevent wells and septic systems from being
installed on the adjacent lots. The proposed septic system will not change the general slope of
the area. Ponding and/or concentration of surface runoff will not result from this installation.

conscientious engineering analysis of the system in accordance with ADEC and MOA o6�0 t
DSD Guidelines & Regulations. The reported results described the performance of the

septic systems depend on the local soils condition, groundwater levels that may

Nitrate Advisory
Certificate of On -Site Systems Approval # OSC131616
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on T12N R3 W Sec25
S2 NW4 SE4 NW4 NW4. This inspection revealed a nitrate concentration of
6.97 milligrams per liter (mg/L) was reported for the property's well water
sample. The Environmental Protection Agency (EPA) has established a
maximum contaminant level (MCL) of 10.0 mg/L for public drinking water
systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Please see the
attached "Nitrate Fact Sheet" for important information regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

To help eliminate high nitrate levels in this well, we performed the
Lined well with 4" pvc casing to 52'
Installed two shale packers at bottom of 4" liner
First packer contained dry grout during installation
Second packer was grouted via tremmie from the bottom up
Tremmie was rotated around casing to ensure positive seal
Once grouting was completed, great care was taken to avoid movement
and a 72 hour hold on pump operation was observed.
Once pumping began it is noted that the resting static water level changed��
from 20' to 57'
Samples were then taken by Garness Engineering.

As we verbally discussed on June 16, I am providing a copy of my field data (along with these
explanatory notes) and requesting your determination whether the existing wastewater disposal system .
serving the residence at 12641 Rya Road is functioning adequately for issuance of a HAA certificate.
The original wastewater disposal system was installed around the time the house was built in
1962. It begins with a concrete septic tank with a probable capacity of 750 gallons. The 1976 upgrade
permit states that the capacity is 1000 gallons: we are in the process of having the pumper verify the
actual capacity. Effluent from the septic tank then flows into a seepage pit of unknown dimensions,
which was also installed around 1962. In 1976 the system was upgraded by the installation of a soil
absorption trench downstream of the seepage pit. The Municipal inspector's report is not fully filled out,
however the trench is probably 36 feet long with an effective gravel depth of 9 feet.
Since I was unable to locate either the cleanout or the sump for this trench we arranged for A+ to
dig down to find the trench and install a perforated monitor tube. They reported that the fluid level was
approximately 3 feet below the level of the perforated distribution pipe.
To test the adequacy of the system on June 14 —15, 1999 I added measured quantities of water
into it through different standpipes while monitoring the fluid levels in the septic tank, seepage pit and
trench before, during and after the flow of water was stopped. The initial fluid depths were 45 inches in
the septic tank, 62 inches in the seepage pit, and 49.5 inches in the new monitor tube in the trench. The
addition of 298 gallons of water into the septic tank caused the fluid level in the seepage pit to rise a
maximum of 17.5 inches with no measurable effect on the fluid level in the absorption trench. Once the
flow of water was stopped, I measured a reabsorption rate in the seepage pit of approximately 1 gpm
when the fluid level was 2 feet below ground surface. Based on this absorption rate the system appears
to be functioning adequately for a 3 -bedroom residence.
Since water does not readily flow through from the seepage pit to the trench, I then added water
directly into the newly installed trench monitor tube. Due to a relatively low well yield (1.35 gpm) it took
me portions of two days to run another 837 gallons of water into the trench, which brought the fluid level
up 35 inches to approximately the level of the perforated distribution pipe reported by A+, Once this was
done I measured a reabsorption rate over a 4 -hour period corresponding to 233 gallons per day. This rate
is only half that required for approval of a 3 -bedroom residence.
According to the owners the system has functioned adequately for them for many years and I
could see no sign that effluent has ever surfaced in the vicinity of the seepage pit. In fact, the absorption
rate of the seepage pit appears to be adequate in and of itself, even though the upgrade trench (which is
constructed at a much greater depth) has a quite slow absorption rate.
Based on this test data, I feel the system could be approved as is, with the buyer being informed of
the test findings and made aware that the future longevity of the system is unknown. If so, there is then a
question whether or not a waiver is required because portions of the upgrade trench are probably less than
100 feet from the well. I have prepared the enclosed site plan to help you visualize the layout. While
both the existing septic tank and seepage pit are significantly closer to the well than the trench, they were
installed at a time when the required separation distance was only 50 -feet, so they should be
"grandfathered" in.

By means of this letter I am requesting issuance of a waiver allowing portions of an existing soil
absorption trench to remain 85 feet from the well on the same lot, instead of the normally required 100
feet. The legal description of the property is the S '/2, NW '/4, SE 114, NW '/4, NW '/4, Section 25, T 12N,
R3 W, S.M., and the 3 -bedroom residence on it is located at 12641 Rya Road. This waiver request will
supplement the HAA application submitted June 18, 1999. A site plan, water sample test results, well
logs, pumping receipt, sketches, and copies of other relevant information from your files are enclosed to
assist you in your review and evaluation. As we have previously discussed, and as described in my
earlier letter to you dated June 18, 1999, other components of the wastewater disposal system are closer to
the well. However, these other components have "grandfather rights" allowing them to remain as close
as 50' from the well because they were installed in the early 1960's.
Please refer to my June 18 letter for a detailed description of the septic system components and my
adequacy testing procedure. As shown on the enclosed pumping receipt, Isaac's Pumping Service
reported the capacity of the septic tank to be 1200 gallons when they pumped it on June 18, 1999. At my
request, the property owners dug a hole by hand down to expose the concrete cover of the seepage pit.
This allowed me to verify that imported sewer rock extends over the top of the seepage pit to a depth of 2
feet below ground level. Thus, the seepage pit would not be operating in a surcharged condition until the
fluid level is less than 2 feet below ground surface. A sketch of the soil configuration over the pit, which
was drawn by the owner, is enclosed.
As we agreed in our telephone conversations on June 29, the adequacy of the wastewater disposal
system is demonstrated by the measured absorption rate within the 1962 concrete ring seepage pit only.
Because of the seepage pit's age no waiver is necessary to allow its continued operation. You informed
me that because the soil absorption trench was installed after the required separation distance increased
from 50' to 100' it had to either be formally abandoned and disconnected from the rest of the system or a
separation distance waiver obtained. In order to maximize the potential for future longevity of the system,
the property owners elected to repair the connection between the seepage pit and the trench and to apply
for this waiver allowing the trenches future use. To accomplish this, on July 8, 1999 A+ Home Services
installed anew 4" diameter D-3034 pipe connection between the two components. An insulated
overflow was installed at 2' below ground level in the seepage pit standpipe, and that was connected into
the horizontal distribution pipe in the trench in the location shown on the site plan.
Although no driller's log is extant for the original drilling of the well, a copy of the log for the
1979 deepening of the well is enclosed. The deepening from 78' to 230' was all in bedrock. According
to this log the yield at that time was 1 1/2+ gpm and the static water level was at 80 feet. During my
adequacy test on June 14, 1999 I measured the static water level to be at 69 feet and the yield to be 1.35
gpm. Two other well logs in the immediate area (enclosed) indicate that bedrock depth is in the vicinity
of 16' — 24' below ground surface.
The following is a breakdown of how waiver analysis points could be assigned using D.E.C.'s
1985 "Separation Distance Waiver Guidelines for SCRO"

Based on this point assignment it appears that the waiver can be safely granted without concern as
to contamination of the well due to the proximity of the trench. The fact that no coliform or other
bacteria were encountered in the water sample after 35+ years of system operation indicates that the
contamination potential is low. The fact that the trench is located further from the well than other
"grandfathered in" wastewater disposal system components also supports granting of the waiver.
Any efforts you can make to expedite the completion of this waiver process and issuance of the
previously submitted HAA certificate will be most appreciated. Please feel free to give me a call if you
