1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing
Special Provisions:
1. In order to close out this permit, the well permit (OSP151193) shall be closed out by providing the following:
a) Well log
b) Pump install log
c)Water sample results for total coliform, nitrates and arsenic.
2. The drainfield shall be entered at the midpoint, as shown on the site plan, to comply with AMC
15.65.355H.2, allowing two foot separation to seasonal high groundwater.

Subject: US Survey 3202 Lot 22—NHN Seward Highway
Septic system design
Dear On-Site Services Engineer:
The owner of the above lot has septic system on the property that protrudes onto the adjacent
property to the southwest. The intention is to upgrade to a permitted septic elsewhere on the lot,
so we are submitting this permit application for the construction of a new septic system. The
attached site plan identifies the location of the home as well the septic location and alternate sight.
No conflicts exist between this proposed system and any other well or septic system,whether on
this lot or adjacent lots.
The ground surface on the lot slopes toward the north steeply near the front of the lot, but the
slopes are much milder towards the center of the lot. It is in that flatter area that the new proposed
septic system is located. There are no slopes greater than 25%within 50 feet downslope of either
the primary or alternate site. Contours are shown on the site plan showing the grade and direction
of flow. Stormwater drainage will not impact this septic system. The new trench will be
constructed parallel to the slope as much as possible.
A percolation rate of 1-5 minutes per inch was obtained from the test-hole used for this design.
However,this rate did not seem indicative of the soils encountered in the test hole so a percolation
rate of 30 minutes per inch was considered for sizing the trench to ensure adequate absorption.
Wells on this and adjacent lots are shown. The new system will be a minimum of 100' from all
wells and surface water, and more than 5' away from the septic tank. The alternate site is located
greater than 6' away from the primary site.
Please refer to the attached test hole logs, soils lab analysis, plan and profile pages for the septic
design. If this design is followed, there will be no adverse impacts to adjacent properties.

Ladies and Gentlemen:
May 19, 2014
The owner of the above referenced property would like to apply for a land use permit to build a
three bedroom single family residence. The property has an MOA approved septic system that
was installed in 1976 to service a restaurant. The onsite file contains a permit application, a soils
log and percolation test results, and an inspection report. The septic system consisted of a 750
gallon septic tank, a 1000 gallon septic tank and 36 feet of deep trench absorption field with 8
feet of effective depth.
The septic system was inspected on May 19, 2014. Tank clean outs and a trench sump were
located. The tanks appear to be full to the outlet levels and the trench sump was 70 inches deep
and dry.
Based upon this information, the existing septic system appears to meet current minimum
standards for a 3 bed room single family residence.

The following history regarding well and septic on the subject property is what I'm finding in On -site's records.
History:
• 1976: On-site issued a permit to construct a septic system serving a "bar and cafe" at the subject property
on 8/10/76. The system received final operational approval on 8/30/76.
Note: although On -site's scope of regulation probably never officially included commercial properties,
occasionally we will find where On-site was in fact involved with one. This is more common prior to the
1980's.
• 2014: Lars Spurkland/Spurkland Engineering (deceased) appears to have been retained by the owner to
inspect the septic system. In his report dated 5/19/14 he states the "existing septic system appears to
meet current minimum standards for a 3 bed room single family residence". While his report is
encouraging that the system may be approvable; it doesn't indicate that an adequacy test was performed
on the septic system drainfield, which is one of the requirements for a Certificate of On-site Systems
Approval (COSA). Per AMC 15.55 and 15.65, a COSA is required prior to the transfer of ownership for
residential properties served by a well and/or septic system.
Note: a COSA doesn't appear to have been submitted to On-site.
• 2015: Terra Nelson applied for an on-site private residential well permit (OSP151193) on 6/23/15. Review
comments were issued and emailed to her at tnelson0880(domail.com on 7/8/15. Per On -site's records,
she doesn't appear to have responded to the review comments.
Commentary:
• On-site has discussed the issues with the owner and/or her fiance at the front counter on multiple
occasions. I don't believe any of the discussions were in a documented meeting.
• Deb Wockenfuss handled some of these discussions. I don't know the details of those discussions.

RE: Test Hole and Soil Log Report for Sanitary System
Bennies Paradise Seward Highway
Dear Mr. Garcia:
We are submitting herewith the test boring results and our comments
regarding soil conditions encountered at the subject site. This investigation
was performed in accordance with your request of July 27, 1976 and those
procedures outlined in a letter dated July 15, 1975, by Mr. Rolf Strickland
of the Greater Anchorage Area Borough Department of Environmental Quality.
A single test hole was put down within the subject site area for the
purpose of defining general subsurface soil conditions for the proposed
sanitary system. Excavation was accomplished with an auger type drilling
rig and the test hole was extended to a total depth of 18.5 feet below
ground surface. The final log prepared for the test hole has been
included in Drawing A-01.
Ground water was not encountered in the test hole while drilling.
