Septic System Design
Dear On-Site Services Engineer:
The absorption bed on the subject lot has failed and must be reconstructed before a COSA can be
issued for the property. We are submitting this design and permit application for the
construction of a new Intermittent Dosing Sand Filter (IDSF) to serve the four bedroom home.
The attached site plan identifies the location of the home and the existing well and septic system.
The IDSF filter will be constructed at the same location as the existing filter. No conflicts exist
between this proposed system and any other well or septic system on this lot or adjacent lots.
All contaminated material in the absorption bed location will be removed and disposed in
accordance with Municipal Code. MOA approved sand will be utilized to raise the bed area to
required grade. The new bed will be constructed in accordance with the attached design
documents and to the specified elevations.
The ground surface on the lot is contoured in the area of the proposed bed to shed drainage. The
final surface of the new bed will be similarly contoured and graded. There are no slopes greater
than 25% within 50 feet down slope of the proposed absorption bed. Ground surface contours
are shown on the site plan indicating the grade and direction of flow. Storm water drainage will
not impact this septic system. The new bed will be constructed parallel to the slope as much as
possible.
Wells on this and adjacent lots are shown. The new system will be a minimum of 100’ from all
wells and surface water, and more than 5’ away from the septic tank.
Please refer to the attached test hole logs and plan and profile sheets for the septic design. If this
design is followed, there will be no adverse impacts to adjacent properties.

Impacts to Adjacent Properties
Dear Onsite Services Engineer:.
The owner of the subject property intends to construct a new four-bedroom home on
the lot. We are therefore requesting a permit be issued for the construction of a new
well and septic system to serve the new house. The attached Site Plan and backup
documentation identify the location and configuration of the new well and septic system
and the parameters used in the design. Also identified on the plans is the location of
the alternate site and the test hole location. Existing drainage patterns are shown and
will not be altered by the development of the lot.
Two test holes have been placed near the location of the new absorption bed and
alt,mate site. These'holes indicate silty sand with approximately 1.5' of organic
overburden. The sand is Underlain by a silty gravel. Percolation tests in the sand
resulted in rates between less than 15 and 17 minutes per inch. No groundwater was
found during placement of the test holes but was noted at 7.2' below the surface during
the monitoring period. The groundwater has been monitored for the past several years
during wet periods and has never ds~n higher than 7.2' from the surface. We are
therefore proposing to place an intermittent bottomless sand filter system on the lot due
to the proximity of the. groundwater and the availability of usable area for the absorption
bed. The top of the filter will be set at 2' below the surface to provide sufficient
separation from groundwater.
The ground surface on* the lot slopes as shown on the attached Site Plan with only
shallow slopes near the proposed septic system site. The new absorption bed will be

Soil Test
A third percolation test was performed for Lot 10, Blk. 3 tO confirm the' resuits of test
hole #2 analyzed on 9/13/96. Field measurements showed that/he distance between test
Hole #1 and Test Hole #2 actually was 85 feet, not 50 feet as indicated on the percolation
test performed 9/13/96. This was verified by measuring the distance between the existing
monitoring tubes. A third test hole was dug between the first two holes approximately 35
- feet from test hole #2 and 50 feet from test hole # 1 ( see diagram). Test hole # 3 was
excavated to a depth of 3.3 feet. Soil conditions were dry. A percolation test was
performed; the hole Was presoaked 3 times to a depth of 6 inches.

ON-SITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 10, BLOCK 3, GLACIER VIEW HEIGHTS SUBDIVISION
ADDITION NO. 4
GENERAL:
The scope of this project includes the procurement and placement of a
new 1,500 gallon combination septic tank and lift station and pressure
distribution system. Work also includes the construction of .a new 30'
long x 12' wide Bottomless Intermittent Sand Filter at the location
shown on the attached Site Plan: A minimum of 2' of coarse sand
meeting the specification from Anchorage Tank must be imported and
placed in the filter area. The 6" of pea gravel and distribution piping
must then be constructed atop the imported filter sand material in
conformance with specifications furnished by the supplier. The
distribution piping for the filter must be placed 2' below the odginal
ground surface at the area shown on the Site Plan to provide a
minimum of 4' separation to groundwater. All work must be completed
in stdct conformance with the manufacturer's requirements for
installation of the intermittent sand filter.
Construction shall be in accordance with the approved site plan,
design'drawings, Municipal Permit with any special provisions or
conditions, and all applicable State and Municipal Wastewater
Disposal Regulations.
The Contractor shall be responsible for obtaining all underground utility
locates and for the layout of the septic system and verification of the
location of all lot lines.
Unless specifically agreed .otl~erwise, the contractor shall be
responsible for final grading areas subsequently depressed from soil
settling. Property owner shall be responsible for revegetation of'
affected areas unless specifically agreed otherwise.

Contractors installing wastewater disposal systems must be certified
by the Municipal Department of Health and Human Services for
system installations. Owners installing their own systems must receive
prior approval from D.H.H.S. before beginning system installation.
SEPTIC TANK/LIFT STATION INSTALLATION
The 1,500 gallon septic tank/lift station must be constructed by a
certified tank manufacturer. Construction shall include an 18" manhole
to provide access to the lift station.
2..The septic tank shall be sufficiently bedded to prevent settling or
shifting of the tank.
3. All standpipes on the septic tank shall extend a minimum of 12 inches
above final grade.
4. Tanks installed without 4' of cover shall have a minimum of 2" of direct
burial insulation.

5. A foundation cleanout shall be installed one to four feet from the
building foundatio.n. Two cleanouts are required between the tank and
the drainfield.
6. Final grading over the, tank/lift station shall be such that a positive
slope exists away from the septic tank.
ABSORPTION BED CONSTRUCTION:
1. Th~ absorption bed.shall be constructed to the dimensions shown on
the design. The bottom of the bed shall be within 2" of level.
2. Distribution piping must be placed level with perforations down atop a
level bed of drainfield rock. Rock should then be placed over the pipe
to .provide a minimum of 2" of cover.
3. A silt barrier or geotextile fal~dc must be placed between the drainfield
rock and the natural soil backfill.
4. Monitor t~bes must be 4" in' diameter and installed at the locations
shown on the design. The podion below ground must be perforated.
5.' contractor shall vedfy the septic tank.and drainfield are a minimum
100' away from any private water wells in the area, 150' from a Class
"C" Well or 200' from any community ~,ell.
6. ';Direct I~ury insulation must be' placed over the distribution system if
' less than'3' bf backfill depth is available. Finish grade over the trench

7. Grade area surrounding the absorption trenches to drain away.

3. Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Fernco, or equal).
4. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed
between the final drain rock layer and the native soil layer.
5. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing
the #200 sieve.
EQUIPMENT SPECIFICATIONS:
1. Air Supply Line shall be a Wasteflow Emitterline, %" inside diameter
from Anchorage Tank. Line shall be insulated with %" foam pipe wrap
inside a 2" Schedule 40 PVC Jacket buried a minimum 3' below the
surface.
2. Air Compressor shall be a Thomas Industries Model 5070 as supplied
by Anchorage Tank. Compressor shall be located in the garage or
crawlspace.
3. A'pressure gauge calibrated from 0 to 10 PSI shall be furnished and
installed in. line with the air line. The gauge shall be mounted in an
area which provides easy viewing and is seen on a regular basis Such
as the entrance to the home.
4. Material for Filter Sand must meet the following gradation:
50% or More Passing the No. 4 Sieve
2% or Less Passing the No. 100 Sieve
1% or Less Passing the No. 200 Sieve
INSPECTIONS:"
A minimum of two inspections are required by Municipal Ordinance.
These inspections must be conducted under the supervision of a
professional engineer registered in the State of Alaska. The first
inspection must be conducted after the excavation of trenches, beds or
pits and before the installation of any gravel. A septic tank may be set in
place, but may not be backfilled..
The second inspection must be conducted after the placement of the
· geotextile fabric, gravel, distribution piping, standpipes, cleanouts and
insulation. No backfill should be in place at the time of inspection.
Contractor'shall provide a copy of all field survey layout and construction
notes for use in preparing the certified as-built of the completed system.

This is a request for an on-site septic permit for Lot 10, Blk. 3, Glacier View Heights
Subdivision, Unit No. 4. The percolation tests were performed for the lot on September 22,
1996. At the time of the percolation tests were performed monitoring tubes where installed at
both test hole sites. The two test holes have been monitored for ground water fluctuation for the
past three years starting each April and ending in October. The results of the monitoring indicate
the water level is static and has not raised above 4.2 ft. in test hole #1 and 7.0 feet in test hole #2.
The design is based on the 7 foot water level at test hole #2. In preparation for the house
construction the lot was topographically surveyed. In the area proposed for the primary and
alternate septic drainfields the grade is approximately 9%.

MAINTENANCE AND REPAIR AGREEMENT
THIS MAINTENANCE AND REPAIR AGREEMENT,herein the"AGREEMENT"made and

(AMC) 15.65.365. In consideration of the mutual covenants contained herein,the parties to this
Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System(AWWTS),

2. Maintenance,Repairs and Alterations.
/I (Owner is required to read, understand and initial each section)
._.>/t0 Throughout the term of this Agreement.the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.
...463 It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance,adjustment(s),replacement costs,and inspection costs. This
includes an annual maintenance fee(typically$400 to$600).
!? Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance,
repairs or permitted alterations to the system.
Owner acknowledges that regular maintenance of an AWWTS reduces the potential
failure of the system,which could include sewage backup and costly repairs or drainfield
replacement.

pp repairs from the manufacturer's representative or maintenance provider.

Owner agrees to grant the Municipality reasonable access to test and inspect the
WAWWTS. The Municipality will give at least 24-hour notice.
Owner agrees that any sale or transfer of title of the property will not occur without a new
Certificate of On-Site Systems Approval.
Owner agrees that the AWWTS installation and maintenance requirements as provided
by the AWWTS vendor/installer and approved by the Municipality are the governing
guidelines for the construction,maintenance and repair of the Owner's AWWTS.
Owner agrees to maintain remote monitoring of the AWWTS as required by the
AWWTS approval.
3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system,or upon transfer of title,and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions,nor in any way affect
the validity of the Agreement or any part hereof,or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.
