The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid
water samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water
system. The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's
work.
Note: Alaska Water and Wastewater Consultants, Inc. shall be paid ~at, er prior
to closing for the engineering services provided.
4[ STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my
investigation, based on procedures outlined in the Health Authority Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is(are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the
information obtained from the Municipality of Anchorage files and from my investigation and inspection, the
on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal
and State codes, ordinances, and regulations in effect at the time of installation.

Engineer's Comments:
In conducting this evaluation, AWWC, Inc. attempted to previde a thoreugh,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operetional life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfacto~ test
results do not guarentee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. A WWC, Inc. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.

ENCROACHMENT AGREEMENT
THIS AGREEMENT, made and entered into this 17th day of
June, 1986, by and between CHUGACH ELECTRIC ASSOCIATION, INC., an
Alaska non-profit electric cooperative membership corporation of
Anchorage, Alaska, hereinafter called 'First Party', and Ed R. &
S.Catherine Conyers of Anchorage, Alaska, hereinafter called
'Second Party', WITNESSETH THAT:
WHEREAS, First Party occupies an easement on the
Northeasterly (NE'ly} side of Lot Twenty-seven (27), Block Three
13}, Southpark Subdivision, Addition No. 2, Anchorage Recording
District, Alaska; and
WHEREA~, Second Party has installed a
drain field which encroaches approximately eight
foregoing easement.

NOW, THEREFORE, for and in consideration of their
mutual agreements and other good and valuable consideration, the
receipt of which is hereby acknowledged, First Party, for itself,
its successors and assigns, waives its objections to Second
Party's encroachment on the said easement for the purpose men-
tioned, subject to the following oonditions:
1. Second Party will hold First Party harmless from
liability for any injury or damage to any person or property
which may result from such encroaohment.
2. The said encroachment will be without prejudice to
First Party's full enjoyment of any and all rights it may have in
and to such easement.
3. Any facilities or property of First Party which
ar~ damaged or destroyed as a result of such encroachment will be
repaired or replaced at no cost to First Party.
4. Seco~d..Party will comply with all applicable
safety codes and regulations.
·5. Any expenses which First Party may incur in the
relocation or modification of its facilities to accommodate the
said encroachment will be paid by Second Party.
By its execution of this Agreement, Second Party
agrees, for itself, its successors, and assigns, that its encroach-
ment upon the named easement as hereinabove mentioned will be
subject to the foregoing conditions.
IN WITNESS WHEREOF, the parties have caused this Agree-
ment tO be executed by their duly authorized representatives and
agents as of the day and year first above mentioned.

Waiver Request for a Two Foot Encroachment of Lot Line.
Dear Keith:
Due to the length of septic system (143'), exlstin9 house and driveway,
gas, water and power lines, this was the only feaslble location for the
septic system on subject lot. The area north of the house was llmlted
by the 20 foot separation dlstance to foundation and was heavily
populated with mature trees. Excessive slopes influenced the area
northwest of the house. East of the residence would co~romise the
separation from the key box to the absorption area. Only the northwest
end of the trench encroaches to wlthin 8 feet of the lot line. The
trench does not parallel the lot line, but angles southeast from the
area of encroachment. The limitations of developing the residence in
advance of installation of the septic reduced the available area for
the absorption area.
