Gentlemen:
My firm conducted a Health Authority Investigation on the referenced lot in March of this year. At that time, the system
was determined to be adequate for a five bedroom house, but the records indicate the system was constructed for a three
bedroom house. A total of 750 gallons of water was added to this system during the test. The new owners would like to
upgrade the system to four bedrooms. Attached is the proposed design to upgrade the system. The upgrade will be
attached to the existing system, and is located inside the testhole used to design the existing system. No groundwater or
bedrock was encountered in the test hole excavated in 1986.
The lot is approximately 1.61 acres in size. Lot 12A slopes to the northwest at a rate of approximately 1 to 2 percent.
The proposed installation will be located between the existing system and the house on the lot. A new 500 gallon septic
tank will be installed between the existing tank and the existing leach field. The existing 1,000 g septic tank will be
verified during the installation. It will be reused if found competent, and replaced with a new 1250 gallon tank outside
the well radius if found to be deteriorated. Double dean-outs wilt be installed down stream from the tank and between
the two tanks. The proposed location is greater than 100 feet away from the existing well serving this property and 25
feet from the water service lines. The surrounding wells are located greater than 100 feet from the proposed installation.
There is an embankment to the west of the proposed installation. The new system will be located approximately 50 feet
from this bluff. There should be no problem with effluent day-hghting along this bank due to the depth of installation.
The area located to tho south of the existing house was designated in 1986 are the reserve are. A test hole excavated in
1986 to investigate an undocumented crib verified the soils as being acceptable for absorption rates from 85 to 125 sfper
bedroom. This area is of sufficient size to install a four bedroom system with no interference from existing wells or other
obstacles. The proposed installation will not affect the future development of the surrounding or existing lots. See the
attached design.

Gentlemen:
My firm conducted a Health Authority Investigation on the referenced lot in March of this year. At that time, the system
was determined to be adequate for a three bedroom house. The new owners would like to upgrade the system to four
bedrooms. Attached is the proposed design to upgrade the system. The upgrade will be attached to the existing system,
and is located inside the testhole used to design the existing system. No groundwater or bedrock was encountered in the
test hole excavated in 1986.
The lot is approximately 1.61 acres in size. Lot 12A slopes to the northwest at a rate of approximately 1 to 2 percent.
The proposed installation will be located between the existing system and the house on the lot. A new 500 gallon septic
tank will be installed between the existing tank and the existing leach field. The existing 1,000 g septic tank will be
verified during the installation. It will be reused if found competent, and replaced with a new 1250 gallon tank outside
the well radius if found to be deteriorated. Double clean-outs will be installed down stream from the tank and between
the two tanks. The proposed location is greater than 100 feet away from the existing well serving this property and 25
feet from the water servicelines. The surrounding wells arelocatedgreater than I00 feet from the proposed installation.
There is an embankment to the west of the proposed installation. The new system will be located approximately 50 feet
from this bluff. There should be no problem with effluent day-hghting along this bank due to the depth of installation.
The proposed installation will not affect the future development of the surrounding or existing lots. See the attached
design.

Dear Steve:
Since my letter of June 5, 1986, I have performed an adequacy test on
the two sewer systems on the above referenced lot. The system serving
two bedrooms, installed 9/18/71, failed the test. The unknown system
serving one bedroom, passed the test and results show it to be more than
adequate for three bedrooms. Attached are my test results. 559 gallons
of water was added to the system with a total rise of 1.87 feet. This
gives us 299 gallons per foot. From the drawdown readings, 200 gallons
was absorbed in 80 minutes. Projecting this data on the graph, this
system could be capable of absorbing thousands of gallons of water in a
24 hour period. The soil that this system is in, is GP rated at 12§ SQ
FT/bedroom. To avoid replacement of the failed system, the homeowner
wishes to totally abandon the failed crib and its septic tank, and run
the pipe just prior to this septic tank around the house and Into the
new septic tank of the unknown system. This would mean the entire
8-bedroom home would be served by the unknown crib.
The new septic tank for this unknown crib has not been installed yet.
With your approval of this request, the new tank will be installed, the
pipe from the failed system extended to the new tank, and total
abandonment of the two old septic tanks and failed crib can all be done
at the same time.

Dear Steve:
The above referenced lot consists of a three bedroom single family
residence. Two of the bedrooms are served with an on-site sewer system
installed September 13, 1971. The third bedroom is served by an~unknown
system on the west side of the house. This system was installed
sometime from 1977-1979. It was never filed with the Municipality of
Anchorage. It has a crib of unknown size and an old concrete septic
tank. The septic tank was exposed and inspected on May 29, 1986 and
found to be leaking. A new 1000 gallon septic tank will be installed
and inspected once you issue a permit for the tank. The old septic tank
will be crushed and filled for total abandonment. An adequacy test will
then be done on the crib and on 'the two bedroom system.
A soils test was done on May 29, 1986 near the unknown crib. This test
hole was dry and went a minimum of 4 feet below the crib. A water
monitor tube was not put into this hole but will be installed when the
new septic tank is installed.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems
Approval are valid for 90 days from the date of issue for properties served by a.private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEO and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that

Certificate of On-Site Systems Approval (COSA) # 111472
During a recent COSA on-site inspection and test of the potable water
supply well on Block 2, Lot 13qof Prospect Heights subdivision, the well's
productivity was determined to be 0.5 gallons per minute. The minimum
well productivity required by this Department (AMC 15.55) for a 3-bedroom
residence is 0.3 gallons per minute. Although the subject well currently
exceeds this minimum requirement; all parties concerned are advised that the
production capacity of the well may fluctuate. Restriction of non-critical
water uses such as washing cars and watering lawns and gardens may be
required.
This advisory must be attached to all copies of the subject Certificate of On-
Site Systems Approval.
