Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The absorption bed on the subject lot has failed and must be replaced. We are
therefore requesting a permit be issued for the upgrade of the septic system to
accommodate the three-bedroom home on the lot. The attached Site Plan and backup
documentation identify the location and configuration of the existing and proposed
septic systems. The existing septic tank will be decommissioned in accordance with
Municipal Code. The existing absorption bed will remain in place as the alternate site
for the new four-bedroom Advantex Treatment System proposed for the lot. Also
identified on the plans are the locations of the existing well this lot. No conflicts exist
between the proposed septic system and the wells on adjacent lots. Drainage arrows
and ground surface contours are shown indicating the current drainage patterns. The
drainage patterns will be maintained after construction.
The test holes placed on the lot during the original design indicated Poorly Graded
Sand in the absorption lense underlain by Silty Gravel. The percolation rate was
determined to be 2.7 minutes per inch. Groundwater was encountered during the
placement of the test hole and stabilized at 4.5' below the surface during the monitoring
period. We were unable to replicate the percolation rate in the soils encountered in our
test hole. The percolation rate obtained was 20 minutes per inch and no groundwater
was encountered but was monitored at 9' below the surface on 9/g/16. A subdrain was
installed on the lot at a depth of 10' at some point in the past which may now control the
groundwater level and account for the drier test hole. We are proposing to place a
four-bedroom Advantex Treatment System with 2 new 20' long x 5' wide x .5' effective
depth absorption trenches using an application rate of 3 gallons per day per square
foot. The trenches are oversized to allow for additional surge area and to provide free
flow of treated effluent through the soil strata. Coarse MOA approved sand will be
placed beneath the drainfield rock to a depth of 4' below the original ground surface.
The distribution line will be placed at the original ground surface with the total depth of
the trenches 4' below original ground which includes the depth of the sand. The end of

the trenches will be a minimum of 20' from the subdrain. The surface slope in the area
of the trenches is away from the subdrain and effluent should migrate in that direction.
Any effluent which may reach the subdrain would enter only after at least 20' to 30' of
travel through silty soils and then travel more than 70'-80' prior to surface discharge.
At that point the treated effluent should provide little or no danger of contamination to
surface water or the environment in general.
The ground surface on the lot in the area of the new trenches slopes to the southwest.
The trenches will be placed parallel to the contours of the existing ground. A minimum
of 100' will be maintained between all components of the septic system and the
proposed well and from any surface water in the area.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

ADVANCED WASTEWATER TREATMENT SYSTEM
MAINTENANCE AND REPAIR AGREEMENT
BETWEEN MUNICIPALITY OF ANCHORAGE
THIS MAINTENANCE AND REPAIR AGREEMENT made and entered into as of this

herein the "OVY'NER," and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Maintenance and Repair Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. Municipality grants permission to
Owner to utilize and operate an Advanced Wastewater Treatment System
(AWWTS), described as
Advantex Treatment System
located at (legal description).
Lot 2, Winter Estates Subdivision
2. Definitions.
Alteration. Any change to the design or function of an AWWTS that
includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design.
Certificate of On -Site Systems Approval (COSA). An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with Anchorage
Municipal Code (hereinafter, "AMC") 15.65. These approvals certify that
the systems are adequate for the homes that they support and meet the
codes that were in place at the time of system construction.
Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AWWTS that were included
in the original design which would allow the AWWTS to continue to
perform as designed.
Permit. An On -Site Wastewater Disposal Permit as required by AMC
15.65 to construct and operate an AWWTS.
3. Term. The term of this Maintenance and Repair Agreement shall begin on the
date of approval by the Municipality to operate the installed system or issuance of a
COSA, and shall continue while the AWWTS is in use or is operational or until the
property is sold or title is transferred by the owner and a new COSA is issued to the
new owner or transferee of the property.
4. Alterations, Installation and Removal of Additional Eguipmen . Prior to
performing any alterations to an AWWTS, the owner agrees to obtain an On-site
Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65.
5. Maintenance and Repairs.
A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall
maintain their AWWTS in a satisfactory condition capable of producing treated
septic effluent in accordance with the equipment's approval for operation in the
Municipality. The owner shall enter into a service agreement with an AWWTS
service and maintenance provider Wroved by the municipality and the
manufacturer of the AWWTS for the entire term of the AWWTS. In addition, it
shall be the responsibility of the Owner during the term of this Maintenance and
Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay
for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement
costs, and (5) inspection costs.
B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules
and orders for the AWWTS.
C. Upon request by the Municipality, the owner agrees to provide the Municipality a
written schedule of routine maintenance and repairs which have been performed
on the system. When a record of maintenance is documented and maintained by
the system vendor, the owner agrees to allow the Municipality access to this
information.
D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS
may be assessed in accordance with AMC 14.60 for improper discharge.
E. Owner agrees that only maintenance and repair personnel approved by the
Municipality will inspect and make any necessary maintenance, repairs or
permitted alterations to the system.
F. Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
G. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On- Site Systems Approval.
H. Owner agrees that the AWWTS installation and maintenance requirements as
provided by the AWWTS vendor/installer and approved by the Municipality are
the governing guidelines for the construction, maintenance and repair of the
Owner's AWWTS.
6. Nonwaiver. The failure of either party at any time to enforce a provision of this
Maintenance and Repair Agreement shall in no way constitute a waiver of the
provisions, nor in any way affect the validity of the Maintenance and Repair
Agreement or any part hereof, or the right of such party thereafter to enforce each and
every provision hereof
7. Amendment.
A. This Maintenance and Repair Agreement shall only be amended, modified or
changed by a writing, executed by authorized representatives of the parties, with
the same formality that this Maintenance and Repair Agreement was executed
with, and such writing shall be attached to this Maintenance and Repair
Agreement as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this agreement, the only authorized representatives of the parties
are:

C. Any attempt to amend, modify, or change this contract by either an unauthorized
representative or unauthorized means shall be void.
8. Jurisdiction: Choice of Law. Any civil action arising from this Maintenance
and Repair Agreement shall be brought in the Superior Court for the Third Judicial
District of the State of Alaska at Anchorage. The laws of the State of Alaska shall
govern the rights and obligations of the parties under this Maintenance and Repair
Agreement.
9. Severabilit . Any provisions of this Maintenance and Repair Agreement decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining
provisions of the Maintenance and Repair Agreement.

REFERENCE: Lot 21 Winter Estates Subdivision
Request you issue a P-ermit to drill a well and install a
septic system to serve the proposed two bedroom house on
the referenced property.
A test hole was excavated and percolation test performed.
The approximate location of the test hole is located on the
attached site plan.
At the time of excavation water was encountered in the
test hole at 5.5 feet and after seven day ground water
monitoring, water was found at 4.5 feet. A curtain drain
was installed after seven day monitoring (on 11-4-95) and
the monitoring tube at the test hole was dry 11-9-95 to a
total depth of 7.5 feet.
This property has enough area for a future septic upgrade
which can be seen on the attached site plan.
We do not anticipate any adverse effects on neighboring
wells, septic systems or drainage patterns by the
installation of the proposed septic system.
There are no points of contamination within the proposed
well radius which can be seen on the attached site plan.

2. The septic tank shall be sufficiently bedded to prevent
settling or shifting of the tank.
3. All standpipes on the septic tank shall extend a minimum of
12 inches above final grade.
4. Septic tanks installed with less than 4 ft. of cover shall
be insulated.
5. A foundation cleanout shall be installed one to four feet
from the building foundation. In the line between the tank
and the leachfield there shall be two adjacent cleanouts
(unless an effluent pumping system exists within the septic
tank). These cleanouts shall be located on undisturbed
soil not more than 10 f t. from the tank. The f irst
cleanout, in line, shall be to clean toward the leachfield.
The second cleanout shall be to clean toward the septic
tank.
6. Final grading over the septic tank shall be such that a
positive slope exists away from the septic tank.
LEACHFIELD BED INSTALLATION:
1. Excavate the proposed Bed Area to the depth shown on the
design. The bottom of the excavation shall be within 2
inches of level. If the bottom of the excavation becomes
smeared, it must be raked or scratched (ruffed -up) before
gravel or sand placement.
2. if a sand layer is required, place sand over entire
excavation to the required depth shown on the design. The
top of the sand layer must be within 2 inches of level.
3. Sewer rock shall be placed uniformly throughout the entire
bed. The distribution piping is to be of PVC (ASTM D3034
or equal). All joints are to be solvent cemented. Gravel
depth below the distribution pipes shall be a minimum of
six (6) inches. Gravel depth above the perforated pipe
shall be a minimum of two (2) inches.
4. The distribution piping must be as shown on the design.
The distance between the outermost perforated distribution
pipes and the sidewall of the absorption bed must be no
more than three feet.

5. Silt barrier material must be installed between the final
gravel layer and the native soil backfill, unless
insulation is usedr then the silt barrier should be
installed over the insulation. Ensure the silt barrier
covers the entire gravel surface before placing backfill.
6. Monitor tubes shall be of four (4) inch diameter,
installed at the locations shown on the designt and extend
a minimum of 12.inches above final grade. The portion of
the monitoring tube extending through the gravel depth
shall be perforated six (6) inches below the bottom of the
horizontal distribution lines. The monitoring tube should
not extend below the bottom of the gravel surface.
7. Backfill over the final gravel layer must not be less than
twenty -f our (24) inches. Insulation must be installed when
the backfill depth is less than thirty-six (36) inches.
The finished grade over the bed must be mounded to prevent
the formation of a depression after settling.
MINIMUM MATERIAL SPECIFICATIONS:
Any septic tank proposed for installation must be
constructed by a Municipally approved septic tank
manufacturer.

system in . stallations in the Municipality of Anchorage:

burial polystyrene (Dow Chemical Company Styrofoam HI or
equal).

watertight couplings (Caulder, Fernco, or equal.).

5. A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N,
or equal) must be installed between the final leachfield
gravel layer and the native soil backfill.
6. All leachfield gravel- (sewer rock) shall be 0.5"-2.5"
screened gravel with less than 3% passing the #200 sieve.
7. When sand is being used as a filter material, its gradation
specifications must conform to current M.O.A. or D.E.C.
requirements.
INSPECTIONS:
Typically there will be a minimum of three (3) inspections
required during the installation of the wastewater disposal
system. These inspections will occur as follows:
1. The first inspection must be conducted after the
excavation of ditches, pits, trenches, or beds and
before the installation of any gravel. A septic tank
may be set in place, but may not be backfilled before
this inspection.
2. The second inspection must be conducted after the
placement of the silt barrier, gravel, distribution
lines, standpipes, cleanouts, and insulation, but
before the placement of any other backfill.
3. The final inspection is to occur upon final grading of
the property.
Often there will be more than these 3 inspections required.
Especially with the installation of multiple trenchest sand
filters, pressurized distribution systems, etc. Thus, the
inspecting engineer is to be contacted at least 24 hours prior
to the start Of construction. If necessary, a pre -construction
meeting will take place on-site. The inspecting engineer will
not coordinate, direct or control in any way the contractors
activities.

The owner shall contract with the contractor to perf orm the work
outlined in these specifications and plans and in accordance
with the attached M.O.A. permit. There will be no contractual
arrangement existing between the contractor and S & s
Engineering. S & S Engineering shall be the owner's
representative and will inspect the work as stated above to
document the contractors activities. Final acceptance of the
contractors work rests with the owner and the M.O.A.
S & S Engineering shall have no liability to the owner or to
others for acts or omissions of the contractor or any other
persons performing work on this project or the failure of the
contractor to carry out the work in accordance with these
construction documents. S & S Engineering's inspecting engineer
will not be responsible for the construction meansr methods,
techniquest sequence, procedures or the safety precautions
incident to this project.
