Ref: Proposed Septic Upgrade for The Tablelands Subdivision; Block 6, Lot 2
To whom it may concern:
The existing 4 bedroom house is served by a public water system and a private septic system.
The septic system consists of a 1250 gallon septic tank and a bed type drainfield that were
installed in 2004. The bed is in a state of failure and must be upgraded prior to the sale of the
house. Two test holes were excavated on the property. The new drainfield will be designed
around the 30 foot radii of these test holes. Comments regarding the design are summarized as
follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring,
and the percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic
system.
4. TOPOGRAPHY: As can be seen on the attached design drawing the topography around the
drainfield is a 1 to 5 percent slope running from approximately southeast to northwest. In short,
there are no slope concerns.
5. PIPE SLOPE CONCERNS: As can be seen on the attached profile design drawing, we are
proposing to use a 1/16" per foot slope of the 4" sewer line from the tank to the new drainfield.
We are proposing that the system be installed according to the elevations shown on the profile
design drawing. If groundwater levels change at the time of the construction or the actual
elevations dictate that gravity flow cannot be achieved with this slope, a 500 gallon Anchorage
Tank lift station will be installed after the existing 1250 gallon septic tank.

Dear Ms. Slimes:
Your request for a waiver of the required 10 feet horizontal separation from the
absorption field to property line has been approved. The approved separation distance is
4.0 feet. Your request to modify'the existing wastewater disposal system in order to meet
the minimum size 'requirements ,for the';4-bedroomsingle-family dwelling is also
approved. This modification involves extending the existing absorption field by 20 feet.
An additional test hole, percolation test. and 7 -day groundwater monitoring must be
conducted in the vicinity of northwester portion of the existing drainfield during the
addition. Results of the additional test hole; percolation test, and 7 -day groundwater
monitoring shall be included in the final Inspection Report.:' .
This waiver approval applies to the existing absorption field to property line separation
only. Any future upgrade to the on-site wastewater disposal system will require all
separation distances be met or another approval from this department.

Groundwater detected in AKTH#1 at 12 feet below grade on 7/8/04. GWMT checked on
7/15/04, and GW again detected at 12 feet (see attached Soils Log — Perc Test). Based on this,
design changed from trench to bed. The following is a summary of the proposed design of the
bed SAS:

A 15'x60' bed was installed in July, 2004, for a 3 -bedroom home, and was inadvertently installed
in the 10 -feet lot line setback. The additional 20 feet of bed will result in the bed being
approximately 4 feet from the lot line (see attached Site Plan). Prior to the installation of the
additional 20 feet of bed, the lot line will be staked so that the lot line is not crossed.
Due to the encroachment into the 10 -feet lot line setback, we are requesting a waiver. Enclosed
with this letter is the $175 fee for waiver requests.

The owner of the above referenced lot is proposing to construct a 3 -bedroom single
family dwelling (see attached Plot Plan). The proposed dwelling is to be served by a
community water system and a private septic system. With this letter we are requesting
plan review and a permit to install an onsite septic system to serve the proposed
development.
DESIGN SUMMARY
Soils
A test hole was excavated on March 15, 2002, to a depth of 13 feet below grade, and
was logged by Mr. Tobin Spurkland (labeled TS TH L1 B8 on the attached Site Plan). A

around the 30 -feet radius of the test hole. The soils were classified as SM, silty sand.
The percolation test indicates a soil percolation rate of 7 minutes per inch which
translates to a wastewater application rate of 0.8 gpd/sgft.
A monitor tube was installed and groundwater levels were rechecked on April 1, 2002.
According to the information on Mr. Spurkland's Soils Log — Percolation Test form, no
groundwater was encountered at that time. A separation distance of 5 feet will be
applied between the bottom of the trench and groundwater since the groundwater level
was monitored in April.
Because of the fluctuating groundwater levels in this area, a test hole will be excavated
at the time of installation of the septic system. The design will be adjusted at that time, if
necessary. We will contact your office to confirm the necessary changes.

the west. There is adequate usable area for an onsite septic system and replacement
system.
Surface Water
There are no surface waters within 100 feet of the proposed septic system.
Surrounding Properties
This property and the parent subdivision will be served by community water system. We
are unaware of any adverse impacts this installation would have on adjacent wells or
septic systems. Development on this lot will not preclude development of wells and
septic systems on neighboring lots in the future.

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the ,performance of the
system unde.- the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system,
These conditions are outside the control of the evaluator of the system. Satisfactory lest
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
