Septic tank replacement permit application
Dear On-Site Services Engineer:
The owner of the lot referenced above has a permitted well and septic system on his property, but
no building. The lot has been vacant since the septic system was built, and it was never put into
service. The owner would like to replace the septic tank and move it to a more convenient location
based on his possible home. We are submitting this permit application for the placement of a new
septic tank. The attached site plan identifies the location of the existing well and septic system as
well as the location of the new septic tank. The drainage pattern in general will not be changed by
the construction.
The existing 1250-gallon tank will be decommissioned per code. The new tank of the same size
will then be placed as shown on the site plan. After the new tank,we will install a double-cleanout
and connect into the end of the existing absorption trench. If needed due to shallow bury,insulation
will be placed above the new tank.
If the system is constructed as designed, it will have no adverse impact:
• On the wells in the area or those to be constructed in the future.
• On existing septic systems in the area or those to be constructed in the future.
• On reserve space, either surface or subsurface, on any lots located in the area.
• On drainage patterns in the area. The current drainage pattern will be maintained.

Well Construction Permit
Dear Onsite Services Engineer:
The owner of the subject lot intends to construct a new home on the site in the future.
A permit was issued for the well construction in 1998, but only the septic system was
constructed. We are therefore requesting a permit be issued to construct a private well
on the lot. The location of the well is shown on the attached Site Plan. The wells and
septic systems on adjacent lots are also shown. No conflicts existing between the
proposed location of the well and septic systems on adjacent properties.
The location of the well and the proposed location of the house and sewer service on
the lot will not impact adjacent properties. If the well and sewer service are constructed
in accordance with our design the following statements apply:
The well, if constructed as designed, will have no adverse impact on the wells in
the area or those to be constructed in the future.
2. The well, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The well, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.

Septic System Design
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The owner of Lot 1A, Block 213, Trails End Subdivision intends to construct
a four bedroom home on the lot. We are therefore requesting a permit to
construct a well and septic system to serve the home. The location of the
new system and the well are shown on the attached Site Plan. Design
information and documentation is also provided for your review.
The testholes placed on the property revealed silty sands and gravels
which percolated at a rate between 22 and 25 minutes per inch.
Groundwater was not encountered in the upper testholes during the
excavation nor was any noted during the monitoring period. The testhole
placed on the lower portion of the property revealed water at 4.5'. The
alternate site will be centered around this testhole, but will be a shallow
bed system. We have designed the primary system to be a deep trench
with 6' of gravel beneath the distribution pipe. The trench will be a total
of 85' in length with a total depth between 9' and 10'.
The system will be placed near the center of the lot in a flat area. The
ground surface slopes from east to west at a grade of approximately 5%.
The slope increases toward the west property line to 10%. There is very
little slope from south to north. The approximate ground contours are
shown on the Site Plan.

If the system is constructed as designed the following statements apply:
1. The system, if constructed as designed, will have no adverse impact
on the wells in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact
on existing septic systems in the area or those to be constructed in
the future.
3. The system, if constructed as designed, will have no adverse
impact on reserved space, either surface or subsurface, on any lots
located in the area.
4. The system, if constructed as designed, will have no adverse impact
on drainage patterns in the area. The current drainage pattern will
be maintained.

In conducting this evaluation, GEG provided an engineering evaluation of the well and/or septic system in
accordance with the guidelines and regulations established by the Municipality of Anchorage and industry
practices. The reported results describe the condition of the system/s on the datels of the evaluation.
Separation distances were measured to readily identifiable features. Hidden defects or encroachments may
exist that were not identified during the evaluation. The operational life of all wells and septic systems depend
upon a variety of variables, including (but not limited to) soil conditions, groundwater levels (that may fluctuate
during the year), quality of construction (materials and workmanship), and the water usage of the family utilizing
the system/s. These conditions can vary, and are outside the control of GEG. Satisfactory test results do not
guarantee future performance of the system/s; therefore, GEG makes no warranty (express or implied) regarding
the future performance of the well or septic system. GEG makes no representation whether an alternative well
or septic system can be installed on the property in the event either of the current systems fail to perform
adequately in the future. The content of this report is for the sole benefit of the person/party that retained GEG
to perform the evaluation. Reliance upon the information provided in this report by any other person or party
