Ladies and Gentlemen:
I am writing to request a renewal of the septic system installation permit for the above referenced
property. The original permit application was submitted by Flattop Technical Services and
issued on August 13, 2009, permit # SW090149.
A site inspection was conducted on December 12th, 2013. Both test holes groiund water
monitors tubes were located. Ground water was observed at a depth of 7.5 feet in TH#1 and 9.5
feet in TH#2. The design as approved does not require any modifications.

The purpose of this letter is to provide a design narrative in support of our application to upgrade the
wastewater disposal system serving the 4 -bedroom residence on Lot 4, Block "D", Timberlux S/D, Unit
# 1, located at 15320 Curvcll Drive. Soils logs, perc test results, a site plan, design drawings and
specifications are enclosed for your review.
As can be seen from the soils log for test hole 42, the native soil between 1.25 and 9.5 feet below ground
level is sandy gravel with a measured percolation rate of less than one minute per inch. Although no
groundwater was observed at the time the test hole was dug or during the ensuing week, relatively
shallow seasonal groundwater tables have been documented in the general area. According we propose
to construct a shallow bed -type absorption system with a two -foot thick sand filter to position the bottom
of the sewer gravel at a depth of 0.75 feet below original ground level.
Using a soil application rate of 0.7 gpd/sq. ft. as specified in the wastewater ordinance for filter sand, the
required absorption area for a 4 -bedroom residence is (4 x 150Y0.7 = 858 square feet. The proposed 15'
wide by 58' long soil absorption bed has a basal area of 870 square feet. The topography of the lot is
almost level, with about a 2% slope downwards towards the WNW.
In order to construct the system parallel to existing contours of the land and avoid conflict with an
existing shed, the south end of the absorption bed will extend into a platted 20 -foot wide "screening and
fencing easement". I discussed this configuration with Laura in the Municipal R.O.W. section and was
informed that such an easement is not a concern of theirs, so no R.O.W. easement would be required. I
also discussed this with Jeff Poet in your office and he concurred that there should not be any objection
to having a portion of the absorption bed extend into this easement.
According to Municipal records, the existing 1250 -gallon, single compartment, concrete septic tank was
installed in 1970 at a distance of approximately 90 feet from the well on the same lot. At that time the
required separation distance from a private well was 50 feet, so no waiver was or should be required for
continued use of the tank. A Municipal waiver was issued in 1992 allowing the more recent well on Lot
3 to be approved at a distance of 85 feet from this septic tank.
Since concrete tanks do not necessarily deteriorate rapidly with age, the property owner would prefer to
utilize this septic tank in conjunction with a new 500 -gallon lits station located outside the 100 -foot
protective radii of all wells. We propose to do so, providing an inspection of the tank conducted at the
time of bed construction reveals that the tank is still in sound condition, does not leak and has a
functional outlet baffle. If the existing septic tank is found to be no longer in acceptable condition, it
will be properly abandoned and replaced with a new 1500 -gallon STEP tank located more than 100 feet
from all wells.
The proposed system will have no significant impact on present or future water supply and wastewater
disposal systems serving adjacent properties, nor will it have any significant impact on reserved space -
surface and subsurface, or on drainage.

Wastewater disposal system
Specifications
1.0 General:
1.1 The scope of the project consists of inspecting an existing concrete septic tank and installing
either a 500 -gallon lift station or a new 1500 -gallon STEP tank that pumps effluent up into a new 15 -
foot wide by 58 -foot long soil absorption bed with 0.5 feet of washed sewer gravel and 2.0 feet of ISF
filter sand under the distribution pipes.
1.2 Construction shall be as depicted on the approved site plan and design drawings. Minor
deviations from these drawings may be allowed or required by the engineer conducting the inspections.
All construction procedures and material specifications shall conform to Municipal and State
requirements. All separation distances shall be in conformance with Municipal requirements, unless
specifically waived. Prior to the start of construction the contractor shall arrangefor a land surveyor
to place survey laths to delineate the 100 -foot well radii for the Lot 4 and Lot 3 wells in the vicinity of
the existing septic tank and proposed STEP tank. The surveyor should also place survey laths at the
southeast corner of the lot and at 50' and 100' divtances from that corner along the south and east lot
lines.
1.3 The contractor shall be responsible to obtain any necessary utility locates, and to work
around any buried utilities.
1.4 Unless agreed otherwise, the contractor shall be responsible for any tree clearing and
grubbing necessary to complete the project.
1.5 The contractor shall provide adequate cover material and rough grading over all system
components to ensure that proper drainage is achieved after settlement and that there are no residual
depressions. Insofar as possible the contractor shall minimize damage to trees and existing lawn areas.
1.6 Unless specifically agreed otherwise, the homeowner shall be responsible for finish grading
after the soil is compacted, and seeding all areas disturbed by the construction.
2.0 Tanks:
2.1 The existing concrete septic tank shall be exposed to allow the engineer to inspect its
condition and to check for possible leakage around seams and to verify the condition of its outlet baffle.
If the tank is found to be in unsatisfactory condition it shall be properly abandoned per code by pumping
and backfilling with soil. The existing log crib seepage pit shall be properly abandoned per code.
2.2 If the existing tank is found to be in good condition with a functional outlet baffle, the
contractor shall install a new 500 -gallon, Municipally approved lift station outside the 100 -foot radius
around the well. if the existing septic tank is not in good condition it shall be properly abandoned and
replaced with anew Municipally approved 1500 -gallon STEP tank located outside the 100 -foot radius
around all wells. All tanks that are buried less than 4 feet shall be insulated with 2 inches of approved
burial type, rigid insulation.
2.3 All pipe connections to the tanks shall be equipped with waterproof mechanical couplings.
The waste line from the residence to the septic tank shall have a minimum slope of 1/4"' per foot. A
foundation cleanout shall be installed within 4 feet of the building foundation, or a double cleanout
installed upstream of the septic tank.
2.4 The lift station shall be installed either in the separate 500 -gallon tank or in the second
compartment of the STEP tank. The lift station package shall be an "Orenco' system as supplied by
Anchorage Tank.
2.5 The contractor shall be responsible to obtain the necessary Municipal electric permit and
inspection, and shall provide a copy of the electrical inspector's report to the engineer overseeing the
construction of the wastewater disposal system.
2.6 The lift station shall be equipped with a watertight, insulated manhole riser and cover
extending a minimum of 6" above final grade with positive surface drainage away from the manhole.
The contractor shall be responsible for determining the necessary length of the manhole riser.
2.7 The on, off, and alarm floats shall be set at the elevations specified by the lift station
supplier, and their proper operations shall be verified by the contractor.
2.8 The discharge line from the lift station to the soil absorption system shall be 1.25" dia.

3.0 Soil absorption system:
3.1 The soil absorption bed shall be constructed by excavating a 15 -foot wide by 58 -foot long
excavation to a depth of 2.8 feet below ground level on the uphill side of the bed.
3.2 The bottom of the excavation shall be level. Any compacted or smeared surfaces shall be
raked to allow proper infiltration.
3.3 A total of 2.0 feet of ISF filter sand (with less than 2% passing the #100 sieve and 1%
passing the #200 sieve) shall be placed in the excavation and compacted by foot to create a level surface
at the level of the original ground at the location of the test hole monitor tube.
3.4 Next, a total of 0.8 feet of washed sewer gravel shall be placed on top of the sand with the
perforated distribution pipes laid level such that the pipe inverts are no less than 0.5 feet above the
bottom of the sewer gravel.
3.4 The distribution pipes shall be 1.25" diameter schedule 40 PVC with 3/16" holes drilled in
the bottoms at 2.0' spacing. Monitor tubes shall be of 4" diameter and installed in the locations shown
on the design drawings. The portion of the monitor tube extending through the sewer gravel shall be
perforated.
3.5 Approved filter fabric shall be placed over the entire top surface of the sewer gravel. Two
inches of rigid, burial type insulation is to be placed over the entire top surface of the gravel, in addition
to the filter fabric.
3.6 Two feet of soil cover material shall be placed above the top of the insulation, of which the
top six inches is to be topsoil. Fill slopes shall be no steeper than 3:1.
3.7 Unless specifically agreed otherwise the homeowner shall be responsible for arranging to
have the site finish graded after the backfill material has stabilized, and for placement of adequate seed
to promote rapid revegetation.
4.0 Inspections:
4.1 A total of 5 engineering inspections will be required during the course of the project: (1)
initial stakeout with the contractor to establish the location of the system and to discuss the plans,
specifications and construction procedures, (2) after the native material has been excavated to expose the
infiltrative surface to ensure that it is level and at the right elevation, and conforms with the soil test
information, (3) after the compacted sand filter material has been placed, (4) after the sewer gravel is in
place and the distribution pipes have been laid and connected up to the STEP tank, but prior to
placement of insulation or filter fabric, and (5) after rough backfill and grading is complete. The septic
tank and STEP tank installation inspection shall be in conjunction with one of the other listed
inspections.
4.2 The installer shall coordinate the timing of the inspections with the engineer sufficiently far
in advance to ensure the availability of the engineer.

In conducting this evaluation, GEG provided an engineering evaluation of the well and/or septic
system in accordance with the guidelines and regulations established by the Municipality of
Anchorage and industry practices. The reported results describe the condition of the systems on the
date/s of the evaluation. Separation distances were measured to readily identifiable features.
Hidden defects or encroachments may exist that were not identified during the evaluation. The
operational life of all wells and septic systems depend on a variety of variables including, but not
limited to, soil conditions, groundwater levels (that may fluctuate during the year), quality of
construction (materials and workmanship), and the water usage of the family utilizing the system/s.
These conditions can vary, and are outside the control of GEG. Satisfactory test results do not
guarantee future performance of the system/s; therefore, GEG makes no warranty (express or
implied) regarding the future performance of the well or septic system. GEG makes no
representation whether an alternative well or septic system can be installed on the property in the
event either of the current systems fail. The content of this report is for the sole benefit of the
person1party who retained GEG. Reliance upon the information provided in this report by any other
person or party, including but not limited to subsequent property purchasers, is not authorized. In

Ref: Timberlux #1; Block D, Lot 4 - Request COSA Approval for 4 Bedrooms
To whom it may concern:
I am writing this letter in regards to 15320 Curvell Drive Anchorage, AK (Timberlux #1;
Block D, Lot 4). Per the request of the agent, the system was originally inspected and
tested for 3 bedrooms. The home was confirmed by Anchorage Live to be a 3 bedroom
house. However, per MOA records, an appraisal of the home, and your staff the home is
actually 4 bedrooms. We are requesting your department issue the COSA as a 4 bedroom
approval to reflect the actual sizing of the septic system and house. Per MOA records,
the septic system is sized for 4 bedrooms. During our inspection/test, five -hundred and
one (501) gallons of water was introduced into the system over a period of 210 minutes,
causing no rise in the monitoring tube. Based upon this data, it is apparent that if an
additional ninety-nine (99) gallons of water were introduced the system would have been
capable of absorbing it. This would have resulted in a 4 bedroom passed test. It should
be noted that elevation shots were taken and the monitoring tube was shown to be at an
elevation of 101.7. Per the 2014 inspection report the bottom of the system elevation was
101.8. In short, the MT goes slightly below the bottom of the effective and still remained
dry throughout the test.
We understand that your department only considers these requests on a case-by-case
basis and is not something that your department will typically allow.
