Ref: Proposed Well and Septic System for Spendlove View Heights SID #3; Lot 4, Block 5,
To whom it may concern:
The proposed 3 bedroom house will be served by a private well and septic system. We are proposing that
a new 1000 gallon septic tank and a dual deep trench type drainfield be installed. Two test holes where
excavated on the property. The drainfield will be designed around the 30 foot radii of both test holes.
Comments regarding the design are summarized as follows:
1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There is a creek that runs along the west property line. The proposed septic
system will be installed 100 feet from the creek(see attached design drawing.
4. TOPOGRAPHY: As can be seen on the attached design drawing the average topography in the area
of the drainfield is a 15-20 percent slope running approximately northeast to southwest. In short there are
no slope concerns.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems.

Separation distances were measured to readily identifiable features. Hidden defects or encroachments may
exist that were not identified during the evaluation. The operational life of all wells and septic systems depend
upon a variety of variables, including (but not limited to) soil conditions, groundwater levels (that may fluctuate
during the year), quality of construction (materials and workmanship), and the water usage of the family utilizing
the system/s. These conditions can vary, and are outside the control of GEG. Satisfactory test results do not

To whom it may concern:
The existing three (3) bedroom house is served by a private well and septic system. The
owner would like to sell the home as a four (4) bedroom residence. Per the percolation
results for testhole #1 and testhole #2, a 1.2 application rate could have been justified
1 the 2005 design. Utilizing the 1.2 application rate, the drainfieids are sized
adequately for a 4 bedroom system (500'.eet2 required). The septic tank is adequately
sized (1250 gallons) and both trenches have passed a 4 bedroom adequacy test within
the last 2 years (6/18/2013 & 10/17/2014). At this time we request your department
recognize the septic system installed in 2005 as a four (4) bedroom septic system.
Note: During our initial inspections in June 2013, it was found that a diverter valve was
originally installed rather than a splitter valve as called for in the design. The diverter
valve was sending 100% of the flow to the south trench and had probably heen doing so
since the system was installed in 2005. Upon inspection on 6/18/2013 the liquid level in
i:he north trench was dry and the south trench was 39 inches. In 2013, the splitter valve
was converted to a diverter valve.
Prior to testing on 10/17/2014, the north trench was dry and the south tench had 13
-Iches of liquid in it. In short, the south trench appears to have recovered significantly.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Onsite Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for'the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of Onsite Systems
Approval are valid for 90 days from the date of Issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions In the professional engineer's work.
4. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my
investigation, based ori procedures outlined in the Certificate of on -Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the
information obtained from the Municipality of Anchorage riles and from my investigation and inspection, the
on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal
and State codes, ordinances, and regulations in effect at the time of installation.

Engineer's Comments:
In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of tho ADEC or MOA DSD. The content of this report Is for .
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party Is not authorized, nor will it confer any legal right whatsoever.

Certificate of On -Site Systems Approval #
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block 5, Lot 4' of
Spendlove View Ills #3 Subdivision. This inspection revealed an arsenic
concentration of 5.16 micrograms per liter (ug/L) for the property's well
water sample. The Environmental Protection Agency (EPA) has established
a maximum contaminant level (MCL) of 10.0 ug/L for public drinking water
systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Information on
arsenic is available from the On -Site Water and Wastewater Program
website (%nvw.muni.org/onsit ) or at 343-7904. 1
This advisory must be attached to all copies of the subject Certificate of
Site Systems Approval.
