Re: Request for Permit
To: Curtis Townsend,
The Alaska Native Tribal Health Consortium, Div. of Environmental Health and Engineering, is
assisting homeowner (Albright) with upgrading and replacing their existing onsite septic system.
The existing septic tank and trench style drainfield are in failure mode, with reports from the
local septic tank pumping company that the tank is not holding wastewater.
We are proposing to replace the entire septic system, with two new sewer service lines (2 house
connections), two sets of foundation double clean-outs, a new 1,500 gal, polyethylene septic tank
by Infiltrator, and a new deep-trench absorption field. The new field will be built directly over
the existing trench and will be longer and wider, and a little deeper. The existing trench is 2.5ft
wide, by 6ft deep of sewer rock (about 11ft to bottom), and 34ft long (Reference current COSA
in Muni electronic files.) The existing trench will be removed, to include 15” of soil on either
side of the trench, 12” over the old trench, 12” under the trench, and 12” on either end, along
with the contaminated sewer rock; and will be backfilled with new clean sewer rock. The new
trench dimensions will be 5ft wide, by 12ft deep (with 7ft of sewer rock and 5 ft of soil cover),
and be 52ft long.
Absorption field design criteria
• 4-bedroom house.
• Design flow rate: 150 gal/day/bedroom.
• Total design flow: 600 gal/day.
• Note: There were 3 test holes dug on this site. One in 1982, one in 2016, and one by
ANTHC in 2025.
• Test hole #1 (1982) (O&E Engineering & Development Co (Oyster and Ellis) - soil type
from 4ft to 12ft is: sand with gravel and cobbles up to 24” in size – Field classification:
SP. The Test Hole was dug to 16ft. It was located down at the bottom of the hill,
probably with the intent of using the upper portion of the lot for the well and house.
• Test hole #2 (TH 16-1) (2016) (ArcTerra Consulting, Inc) - soil type from 2ft to 15ft is:
moderate dense soil, with traces of silt and cobbles to 1’ – Field Classification: SP/gp-
gm-sm. Test Hole was dug to 15ft.
• Percolation test TH#2 (TH 16-1) (4-12-16) (ArcTerra Consulting, Inc) = Percolation rate
is: 1.73 min/in
• Test Hole #3 (ANTHC 10-24-25) – There were 3 samples taken in all, one at 4ft and one
at 8ft with soil type being: “Well-graded gravel with silt and sand”, (GW-GM). The third
sample was taken at 16ft with soil type: “Well-graded gravel with clay and sand” (GW-
GC). Analyzed by Shannon and Wilson Geotechnical and Environmental Consultants
11-5-25.
• Test Hole was dug to 17’ within 30’ of the proposed drainfield trench location.
o However, upon completion of the design, the bottom of the new trench will be at
an elevation of 489’. Which is about 13.5ft deep. In order to provide 6ft of
vertical separation distance, the test hole would need to be down to an elevation
of: 489’ - 6’ = 483’ This would require a test hole depth of 502.5’ – 483’ = 19.5’,
based on the highest point of the existing grade along the trench. (The yard slopes
a bit along the alignment of the trench and drops about 2ft along the 52’ run. In
order to verify that there is no impermeable layer within 6ft of the bottom of the
trench, evaluations were made to the well logs on both this lot (Lot 1) and the
neighboring lot (Lot 2). From these two reference documents, it can be seen that
there is no bedrock, nor an impermeable layer, down to 19.5ft.
Figure 1 - Well Log from Lot 1, Skyline View Subdivision –
showing no bedrock to 19.5ft.
Figure 2 - Well Log from Lot 2, Skyline View Subdivision –
showing no bedrock to 19.5ft.
• Percolation test (ANTHC 10-24-25) = Percolation rate is: 4.0 min/inch. (There was a
mis-calculation on the earlier form of the perc test, which has been since corrected.)
o The percolation test was performed at 3-4ft deep, to meet OSHA requirements for
Competent Person Excavation Requirements and Confined Space Entry rules,
which begin at 4ft. However, the Muni does require the percolation test to be
taken in layer of soil where the absorption trench will be located. In the case of
Albright, the sewer rock portion of the trench begins at 5ft and extends down to
12ft. The formal soil analysis from Shannon and Wilson, shows fairly similar and
consistent materials at 4ft, 8ft, and 16ft depths. The test holes from O&E
Engineering and ArcTerra also show similar soil types in this vicinity. The ground
appears to fairly consistent down to 17ft. The ArcTerra percolation test was
performed at a 4ft to 5ft depth and had a similar percolation rate to the ANTHC
percolation rate (1.7min/inch versus 4.0 min/inch). The red clay layer was
encountered about 4ft down, and varied a bit, but does show a distinct layer (see
photo). The Percolation Test was performed into this layer, which resulted in the
4.0 min/inch perc rate. This would also explain why the perc rate was a bit slower
than the percolation rate that ArcTerra determined (1.73 min/in). However, the
Page 4 of 9
soils under this layer are in more porous material and is where the treatment
trench will be located. The percolation rate would be assumed to be somewhat
faster, under this layer. The subsequent strata’s at 4ft, 8ft, and 16ft were mostly
well-graded gravel with silt and sand, with some clay showing up deeper in the
hole.
o However, for design purposes, the more conservative loading rate used in the
design calculation also mitigates and offsets the percolation test being run at
4ft. Based on the soil type, photos, and percolation rates, it is reasonable to
assume that the rate would not be that much different another 1’ or 2’ deeper, and
probably better (faster).
• No groundwater water encountered at any test holes
• The existing system has failed (backed up), as has the septic tank (not holding
wastewater) which was identified by local septic tank pumping company
• The new system will be a deep-trench system.
• There is a steep slope of 72% in the back yard of this site. The existing system and
proposed new system is within 50ft of a slope greater than 25% over 10ft vertical. The
slope is also within 100ft of a slope greater than 46%.
o However, there is an allowance from the municipality that allows for a reduced
minimum horizontal distance if the system can be 35ft or more from a point that
Figure 3 - Photo of Test Hole from ANTHC TH, 10-24-25 –
showing soil type and band of clayey material at 3ft.
is 2.5ft above the discharge piping, and runs an artificial line of slope 25%, that
intersects the existing slope. In this case, the existing slope is 72%. When the
topography and cross section is calculated, we are able to meet 35ft from the
closest point to the bluff. Which for the most part, also coincides with the
existing trench. See below Profile Diagram on the Slope (Figure 4).
o Below is the scaled drawing indicating that the trench will meet the Muni
modified requirements to be within 35ft of the intersection of the existing slope
with an artificial 25% slope.
• Figure 4
• The plan is to place the new trench over the existing trench. The existing trench is 2.5’
wide, by 6ft deep of sewer rock, and 34ft long. The new trench is will be 5’ wide, and
have 7ft of sewer rock, and be 52ft long. This allows for the removal of an extra 15” of
soil on either side of the trench, 12” above the field, 12” underneath, and 12” on either
end. This will remove all contaminated soil associated with the site, and will be hauled
off-site to a suitable disposal location.
• The new trench was also based on the following calculations.
• The GW-GM type soils, backed by the percolation test would require a loading rate of
150 sq ft/ bedroom. Therefore a 4-bedroom house would require: 4-bedrooms * 150 sq
ft/ bedroom = 600 sq ft. Initially, the new trench was based on 6ft of sewer rock. So,
600 sq ft / 6ft * 2 sides = 50 ft long. However, with the addition of the extra material, the
trench is 7ft deep. If we keep the 50ft long distance, the resulting treatment area is: 7ft *
Page 6 of 9
2 sides * 52ft = 728 sq ft. This is a loading rate per bedroom of: 728 sq ft/4 bedrooms =
182 sq ft per bedroom, which is reasonable for these soils, as they do contain some silts
in them, and a higher loading rate is an appropriate consideration.
• This also provides a loading rate of 4-bedroom * 150 gal/ day / bedroom = 600 gal/day.
728 sq ft / 600 gal/day = 1.2 gal/ sq ft / day. Which is a little high for sandy soils.
However, this does not account for the bottom 5ft of treatment area, which is not
typically used in the sizing of a deep trench, but does add additional treatment. An
effective overall treatment area is 728 sq ft, plus, 5ft x 52ft = 260 sq ft. = 728sq ft + 260
sq ft = 988 sq ft. Therefore 600 gal/day / 988 sq ft = 0.61 gal/sq ft/day; which is
consistent with the values that Conway and Tyler from the University of Wisconsin,
recommend in their onsite wastewater treatment systems manual for infiltrative rates for a
coarse sandy loam material.
• The deep-trench will be 7ft deep, by 5ft wide, and be 52ft long.
• The discharge piping from the septic tank will be 50 ft of perforated 4” PVC pipe.
• The pipe will be covered with 10 ea, Quick5 Infiltrator chambers with end caps, to
protect the orifices and piping.
• The chambers will be covered with non-woven geotextile fabric.
• The chambers will also be covered with 4” of rigid boardstock insulation.
• There will also be 5ft of soil cover over the trench.
• The septic tank will be a 1,500 gallon polyethylene septic tank made by the infiltrator
company, as we need 2 inlets into the tank, and this tank has the ability to have side
inlets. The septic tank will be covered with 4” of rigid boardstock insulation. Note:
There are 3 risers that can be installed for this tank, with the middle riser allowing access
to the baffle that separate the primary chamber from the second chamber.
• There will be two sets of double clean-outs at the foundation, as there are 2 separate
wastewater lines coming from the house. (there was a mother-in-law apartment added to
the house, which had brought the bedrooms up to 4).
Below is a copy of the Civil Design Sheet showing the layout of the system. This is from the
Plans-for-Agency -Review (PAR) set for AKA-24-ANC – Albright Site. The set will be
converted to an Issued-For-Construction (IFC) set, once the Muni permit is received and any
changes required or recommended are incorporated.
The following Figures are excerpts from the Engineering Planset, Plans-for-Agency-Review
(PAR), AKA-24-ANC for the Albright Site. (To be converted to Issued-For-Construction (IFC)
once the permit is received.

The design of this system conforms to standard engineering design practice and meets the intent
of Municipal Wastewater code AMC 15.65.
• A test hole was dug within 30ft of the proposed drainfield and the soils analyzed by
Shannon and Wilson Inc, in November of 2025, and a perforated monitoring tube left in
place to record groundwater over a 7 day period. (the pipe has since been removed).
• The percolation test was performed by an ANTHC field technician under the direction of
an engineer, in October of 2025.
• There are no surface waters impacted by this design.
• The topography of the site is mostly flat, around the house, and there is about a 4ft rise to
the north side of the lot and a 8ft drop toward the neighbor to the south, and a steep 72%
incline along the entire back of the lot, over 30ft down.
• There is no violation of separation distances other than the Steep Slope Reduction
Allowance.
• The sewer service line, in order to bring in the 2 lines from the house, and use
conventional fitting angles, will not have the full 10ft of uninterrupted pipe in front of the
septic tank, which has since been reduced to 5ft per DEC. In order to get the two lines to
enter the tank, with the least amount of plugging issues, there will be an angle of 22.5
degrees about 6ft from the inlet of the tank, at 2% slope on the incoming 2nd line (which
enters from the front end of the septic tank. The 1st (and main) line has a straight shot
into the tank of 16ft at a 2% slope. Both service lines meet both Muni and DEC
requirements.
• The residential well on this lot is protected by the 100ft horizontal separation distance
from the septic system, but has shown high Nitrate levels in the past (11.1 mg/l - 2016).
• No neighboring wells are in the vicinity of the proposed absorption field.
o A field survey was conducted of the lots surrounding Acre Lot 1, Skyline View
Subdivision and a review of the Muni COSA’s and the State’s WELTS database
was performed. There are no residential wells within 100 ft of the proposed
onsite septic system for Acre Lot 1.
• The design of the absorption field will not cause surface water ponding or surface water
run-off, to run onto neighboring lots.
• The design of this system adequately protects public health.

GENERAL DESIGN NOTES
1.THE GRAVITY WASTEWATER SEWER SERVICE LINE FROM THE HOUSE
TO THE SEPTIC TANK SHALL BE SET AT A MINIMUM OF 2% SLOPE.
HOWEVER, THIS CAN BE STEEPER AS SLOPE VARIES BASED ON LENGTH
AND DISTANCE AND THE SLOPE EQUALS RISE OVER RUN, BUT THE
SLOPE CANNOT BE MORE THAN 20% FOR 4” PIPE.
2.THERE SHALL BE A MINIMUM OF 4 FT OF COVER OVER PIPES, SEPTIC
TANK, AND DRAINFIELD, PLUS 2” OR 4” OF BOARDSTOCK INSULATION
OVER PIPE, TANK AND DRAINFIELD, TO PREVENT FREEZING,
DEPENDING ON LOCATION.
3.PER DEC AND THE MUNI THE BOTTOM OF DRAINFIELD (GRAVEL BASE)
MUST HAVE 4 FT OF VERTICAL CLEARANCE TO GROUNDWATER AND 6
FT TO AN IMPERMEABLE LAYER. (HOWEVER, WITHIN THE MUNICIPALITY
THERE CAN BE A REDUCTION TO VERTICAL AND HORIZONTAL
SEPARATION DISTANCES WHEN AN ADVANCED WASTEWATER
TREATMENT SYSTEM IS USED.)
4.A 100 FT HORIZONTAL SEPARATION DISTANCE MUST BE MAINTAINED
BETWEEN THE WELL AND SEPTIC TANK AND DRAINFIELD. INCLUDING
100 FT FROM NEIGHBORING WELLS, LAKES, PONDS, RIVERS, SLOUGHS,
CREEKS, AND STREAMS.
5.THERE MUST BE 10 FT OF HORIZONTAL SEPARATION FROM THE SEPTIC
SYSTEM TO THE PROPERTY LINES. ENSURE NEW SYSTEMS DO NOT
ENCROACH OVER PROPERTY LINES.
6.THE DRAINFIELD MUST BE LOCATED GREATER THAN 50 FT AWAY FROM
A 25% OR STEEPER SLOPE.
GENERAL CONSTRUCTION NOTES:
1.CONTRACTOR MUST COMPLY WITH ALL OSHA TRENCHING
REQUIREMENTS, REFER TO SECTION 31 23 16.13, TRENCHING.
EXCAVATION LIMITS AND SLOPES SHOWN ARE 1:1 AND ASSUMED TO BE
TYPE B SOILS, BUT MUST BE VERIFIED BY CONTRACTOR. MOUND
SYSTEM FINISHED GRADE SLOPES SHOULD BE 3:1 FOR EASE OF
MAINTENANCE.
2.GENERAL LANDSCAPING AND SITE CLEAN-UP - IN GENERAL, ALL
DISTURBED AREAS SHALL BE GRADED, COMPACTED, LEVELED, AND
HAND OR MACHINE RAKED. TOP SOIL SHALL BE USED TO PROVIDE
ORGANIC MATERIAL FOR ROOT STRUCTURE OF GRASS.
HYDRO-SEEDING IS PREFERRED. ROCKS LARGER THAN 2” SHOULD BE
REMOVED FROM THE SURFACE, AND IDEALLY THERE ARE NO ROCKS
WITHIN 4” OF THE SURFACE. ALL CONSTRUCTION DEBRIS, BRUSH,
ROOT BALLS, AND LIMBED TREE BRANCHES SHALL BE REMOVED FROM
THE SITE. BUCKED LOGS CAN BE NEATLY STACKED AND LEFT, IF
HOMEOWNER AGREES.
3.ALL SEPTIC SYSTEM OBSERVATION PORTS SHALL BE VERTICAL AND
PLUMB, AND CAPPED, AND SHALL BE CLEANED-OFF OF ALL MUD, DIRT,
LABELS, AND MARKINGS. ACCESS RISERS ON SEPTIC TANKS SHALL BE
SECURED AND BE CLEARED OF DIRT AND MUD. IF GRAVEL OR PAVED
DRIVEWAYS ARE IMPACTED THEY MUST BE REPLACED TO AN “OR
BETTER” STATUS.
4.ANY ABOVE-GRADE OR EXPOSED INSULATED RISERS ON THE SEPTIC
TANKS AND LIFT STATIONS MUST BE WRAPPED IN DENSO BLUE
VISCOTAQ, VISCOWRAP ST TAPE.
5.THE SEWER ROCK FOR THE DRAINFIELD MUST BE RAKED LOOSE AND
NOT COMPACTED BY EQUIPMENT.
6.THE BOARDSTOCK LAYER MUST BE LAID ONTO SMOOTH SOIL SURFACE
WITHOUT LARGE ROCKS OR VOIDS, AND CAPPED WITH CLEAN LOAMY
BACKFILL SOIL WITHOUT LARGE ROCKS OR VOIDS.
7.CONSTRUCTION PHOTOS AND WORK LOGS MUST BE SUBMITTED DAILY,
AS WORK PROCEEDS.
8.THE SYSTEM MUST BE INSTALLED AS DESIGNED, AND ONLY CHANGED
OR RE-POSITIONED WITH APPROVAL OF THE ENGINEER.
9.UNDERGROUND UTILITIES MUST BE IDENTIFIED AND LOCATED PRIOR
TO EXCAVATION. PLEASE CALL ALASKA DIGLINE, INC AT 811, A MINIMUM
OF THREE WORKING DAYS PRIOR TO INSTALLATION OF THE SYSTEM.
10.ONCE LOCATED, UNDERGROUND UTILITIES MAY NEED TO BE
PROTECTED FROM DAMAGE DURING INSTALLATION.
11.IT IS THE RESPONSIBILITY OF THE CONTRACTOR TO NOT ENCROACH
OVER PROPERTY LINES. IF SURVEY SUPPORT FROM ANTHC IS NEEDED,
IT SHOULD BE REQUESTED WITH ADVANCED NOTICE. NOTE: ANTHC
SURVEY SCHEDULE MAY NOT MEET CONTRACTOR'S NEED FOR
CONSTRUCTION START DATE.
12.SEWER ROCK AND COARSE FILTER SAND MUST MEET BOTH AK DEC
AND/OR THE MUNICIPALITY OF ANCHORAGE SPECIFICATIONS.
13.THE CONTRACTOR IS RESPONSIBLE FOR LOCATING ALL BURIED
UTILITIES, EASEMENTS, PROPERTY LINES, RIGHT-OF-WAYS, SURFACE
WATER SET-BACKS, WELL RADIUS, AND WATER SERVICE LINES, THAT
MAY INTRUDE INTO THE DESIGN LAYOUT OF THE PROPOSED ON-SITE
SEPTIC SYSTEM.
14.ANY ENCROACHMENTS FOUND, THAT WERE NOT IDENTIFIED ON THE
ENGINEERING PLANS SHOULD BE BROUGHT TO THE ATTENTION OF THE
ENGINEER-OF-RECORD.
15.CONTRACTOR TO ESTABLISH SURVEY BENCHMARKS, FROM WHICH TO
ESTABLISH ELEVATIONS. THE ON-SITE SEPTIC SYSTEM HAS BEEN
DESIGNED USING ELEVATIONS ABOVE SEA LEVEL (ASL).
16.SITE INSPECTIONS - THE CONTRACTOR MUST NOTIFY THE PROJECT
MANAGER AND ENGINEER AT LEAST 24 HRS PRIOR TO BEGINNING
CONSTRUCTION. DEC MUST ALSO BE NOTIFIED AND SHOULD BE
COORDINATED WITH ANTHC. INSPECTIONS SHOULD TAKE PLACE
DURING NORMAL BUSINESS HOURS.

ABSORPTION FIELD DESIGN CRITERIA
·4 BEDROOM HOUSE.
·DESIGN FLOW RATE: 150 GAL/DAY/BDRM.
·TOTAL DESIGN FLOW: 600 GAL/DAY.
·TEST HOLE (ANTHC 10/24/25) SOIL TYPE: WELL-GRADED GRAVEL WITH
SILT AND SAND (GW-GM).
·PERCOLATION TEST (ANTHC 10/24/25) PERCOLATION RATE: 4.0 MIN/IN.
·TEST HOLE DUG TO 17'; NO GROUNDWATER OR IMPERMEABLE LAYER.
·THE EXISTING TEST HOLE WAS NOT DEEP ENOUGH TO RULE OUT
BEDROCK UNDER THE HIGHEST EG POINT ON THE DRAINFIELD TRENCH
ALIGNMENT; HOWEVER, WELL LOGS FROM THIS SITE AND THE
NEIGHBORING SITE (LOT 2) SHOW NO IMPERMEABLE LAYER TO 19.5'
·THE EXISTING SYSTEM HAS FAILED (BACKED UP), AS HAS THE SEPTIC
TANK (NOT HOLDING WASTEWATER) IDENTIFIED BY LOCAL SEPTIC
TANK PUMPING COMPANY.
·THE NEW SYSTEM WILL BE A DEEP-TRENCH SYSTEM.
·THERE IS A STEEP SLOPE OF 72% IN THE BACK YARD OF THIS SITE. THE
EXISTING SYSTEM AND NEW SYSTEM IS WITHIN 50' OF A SLOPE
GREATER THAN 25% OVER 10' VERTICAL. THE SLOPE IS ALSO WITHIN
100' OF A SLOPE GREATER THAN 46%.
·HOWEVER, THERE IS AN ALLOWANCE FROM THE MUNICIPALITY THAT
ALLOWS FOR A REDUCED MINIMUM HORIZONTAL DISTANCE IF THE
SYSTEM CAN BE 35' OR MORE FROM A POINT THAT IS 2.5' ABOVE THE
DISCHARGE PIPING, AND RUNS AN ARTIFICIAL LINE OF SLOPE 25%,
THAT INTERSECTS THE EXISTING SLOPE. IN THIS CASE, THE EXISTING
SLOPE IS 72%. WHEN THE TOPOGRAPHY AND CROSS SECTION IS
CALCULATED, WE ARE ABLE TO MEET 35' FROM THE CLOSEST POINT TO
THE BLUFF. WHICH ALSO COINCIDES WITH THE EXISTING TRENCH.
·THE PLAN IS TO PLACE THE NEW TRENCH OVER THE EXISTING TRENCH.
THE EXISTING TRENCH IS 2.5' WIDE, BY 6' DEEP OF SEWER ROCK, AND
34' LONG. THE NEW TRENCH IS WILL BE 5' WIDE, AND HAVE 7' OF
SEWER ROCK, AND BE 52' LONG. THIS ALLOWS FOR THE REMOVAL OF
AN EXTRA 15” OF SOIL ON EITHER SIDE OF THE TRENCH AND 12”
UNDERNEATH AND 12' ABOVE. THIS WILL REMOVE ALL CONTAMINATED
SOIL ASSOCIATED WITH THE SITE, AND WILL BE HAULED OFF-SITE TO A
SUITABLE DISPOSAL LOCATION.
·THE NEW TRENCH IS BASED ON THE FOLLOWING CALCULATIONS.
·THE GW-GM SOILS, BACKED BY THE PERCOLATION TEST WOULD
REQUIRE A LOADING RATE OF 125 SF/BR (UPSIZE TO 150 SF/BR).
THEREFORE A 4-BEDROOM HOUSE WOULD REQUIRE - 4 BR * 150 SF/ BR
= 600 SF. INITIALLY, THE NEW TRENCH WAS BASED ON 6' OF SEWER
ROCK. SO 600 SF / 6FT * 2 SIDES = 50' LONG. HOWEVER, WITH THE
ADDITION OF THE EXTRA MATERIAL, THE TRENCH IS 7' DEEP. IF WE
KEEP THE 50' LONG DISTANCE, THE RESULTING TREATMENT AREA IS: 7'
* (2 SIDES) * 52' = 728 SF. THIS IS A LOADING RATE PER BEDROOM OF:
728 SF/4 BR = 182 SF/BR, WHICH IS REASONABLE FOR THESE SOILS,
THAT DO CONTAIN SOME SILTS IN THEM.
·THIS ALSO PROVIDES A LOADING RATE OF 4 BR * 150 GAL/DAY/ BR = 600
GAL/DAY. 728 SF / 600 GAL/DAY = 1.2 GAL/DAY/SF. WHICH IS A LITTLE
HIGH FOR SANDY SOILS. HOWEVER, THIS DOES NOT ACCOUNT FOR
THE BOTTOM 5' OF TREATMENT AREA, WHICH IS NOT TYPICALLY USED
IN THE SIZING OF A DEEP TRENCH, BUT DOES ADD ADDITIONAL
TREATMENT AREA. AN EFFECTIVE OVERALL TREATMENT AREA IS 728
SF PLUS, 5' x 52' = 260 SF. 728 SF + 260 SF = 988 SF. 600 GAL/DAY / 988
SF = 0.61 GAL/DAY/SF; WHICH IS CONSISTENT WITH THE VALUES THAT
CONWAY AND TYLER FROM THE UNIVERSITY OF WISCONSIN,
RECOMMEND IN THEIR ONSITE WASTEWATER TREATMENT SYSTEMS
MANUAL FOR INFILTRATIVE RATES FOR A COARSE SANDY LOAM
MATERIAL.
·THE DEEP-TRENCH WILL BE 7' DEEP x 5' WIDE x 52' LONG.
·THE DISCHARGE PIPING FROM THE SEPTIC TANK WILL BE 50' OF
PERFORATED 4” PVC PIPE.
·THE PIPE WILL BE COVERED WITH 10 EA, QUICK5 INFILTRATOR
CHAMBERS WITH END CAPS, TO PROTECT THE ORIFICES AND PIPING.
·THE CHAMBERS WILL BE COVERED WITH NON-WOVEN GEOTEXTILE
FABRIC & 4" OF BOARDSTOCK
·THE SEPTIC TANK SHALL BE 1500 GALLON POLYETHYLENE BY
INFILTRATOR COMPANY WITH TWO INLETS, AND SIDE INLET UTILIZING
THE OPTION. COVER TANK WITH 4" RIGID BOARD INSULATION.
·THERE WILL BE TWO SETS OF DOUBLE CLEAN-OUTS, AS THERE ARE 2
SEPARATE WASTEWATER LINES COMING FROM THE HOUSE. (THERE
WAS A MOTHER-IN-LAW APARTMENT ADDED TO THE HOUSE, WHICH
HAD BROUGHT THE BEDROOMS UP TO 4).

Engineer's Comments: This investigation was completed in compliance with ADEC and MOA regulations. The assessment of the condition
of the well and septic applies only to the conditions as of the day tested. The flow and absorption rates may change due to subsurface
conditions that may not be observed from the surface, changes inland use, local soil characteristics, groundwater levels that may fluctuate
during the year and the water usage of the family being served by the system. The operational life of all well and septic systems are
subject to these various and dynamic characteristics and are outside the control of the
evaluator of the well and septic system. Therefore,
ArcTerra can not give any estimate of how long a
system will function satisfactory for current or future
occupants or can ArcTerra guarantee that no unseen r OI, AL®

Engineer's Comments: This investigation was completed in compliance with ADEC and MOA regulations. The assessment of the condition
of the well and septic applies only to the conditions as of the day tested. The flow and absorption rates may change due to subsurface
conditions that may not be observed from the surface, changes inland use, local soil characteristics, groundwater levels that may fluctuate
during the year and the water usage of the family being served by the system. The operational life of all well and septic systems are
subject to these various and dynamic characteristics and are outside the control of the
evaluator of the well and septic system. Therefore,
ArcTerra can not give any estimate of how long a
system will function satisfactory for current or future
occupants or can ArcTerra guarantee that no unseen
encroachments, deficiencies or discrepancies exist. OF L \ X

Engineer's Comments:
In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system In accordance with ADEC and MOA
DSD Guidelines 8 Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily Identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report Is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any

Nitrate Advisory
Certificate of On -Site Systems Approval # 060219
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block , Lot
1 of Skyline View subdivision. This inspection revealed a nitrate
concentration of 11.9 milligrams per liter (mg/L) was reported for the
property's well water sample. The Environmental Protection Agency (EPA)
has established a maximum contaminant level (MCL) of 10.0 mg/L for
public drinking water systems. While private wells are not subject to this
regulation, EPA standards are based on existing health information and can
therefore be used to gauge the relative quality of water from private wells.
Please see the attached "Nitrate Fact Sheet" for important information
regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

Well Work
Regarding high nitrate levels at 19423 Wildwood Drive. Well camera was sant down to 50 feet with no
broken welds or perforadons. Piths depth Is 10 feet below top of casing. We were only able to drive our
grouting pipe to a depth of 15 feet due to larga rocks. We tried four diftent spots and that was the deepest
we were able to go due to the amount of rooks. We pumped In excess of 140 gallons of grout slurry with
none coming to the surface. It is my belief that R then was a surface water problem with this well it Is not
any longer.

RE: SKYLINE VIEW SUBDIVISION, LOT 1-19423 WILDWOOD DR.
Septic Symern Adequaa & Well Flow Test
Test Procedure & Results:
The well was pumped at a constant rate while an acoustic sounder monitored the
draw down. At the beginning of the test, the static water level was measured at
60' below top of casing. At a pumpin; rate of 4.33 gallons per minute, the water
level dropped only 4' to 64' and remained in the range of 63-64' for the duration
of the test, recovering within minutes after the flow was stopped. Water samples
were taken and found satisfactory with the exception of high nitrates (see
attached). The Municipality of Anchorage requirement for well flow is 150
gallons of water per bedroom per day. This well exceeds this requirement.
JR's pumped the 1,250 gallon septic tank prior to testing. At the beginning of the
septic adequary test, 3.7' of standing fluid was present in the monitoring tube. A
total of 600 gallons were added to the on-site wastewater absorption trench and
the fluid levels rose 025'. Within minutes after adding the water, fluid levels
returned to the pre-test level. The Municipality of Anchorage regwires an
absorption rate of 150 gallons of water per bedroom per day. This septic system
meets this requirement.
The assessment of the condition of the well and septic applies only to the
conditions as of the day tested. The flow and absorption rates may change due to
subsurface conditions that may not be observed from the surface, changes in the
land use, water usage and other factors.

Note: The operational life of all septic systems depends on the local soil
conditions, groundwater levels that may fluctuate during the year and the water
usage of the family being served by the system. These conditions are outside the
control of the evaluator of this septic system. Therefore we can not give any
estimate of how long this system will function satisfactory for current or future
occupants. All septic systems ultimately fail. Some systems last 15-20 years or
longer, while others fail after less than 5 years.
