We are requesting the department consider re -approval of the previously approved On-site wastewater
disposal system / water supply permit, issued 8/23/2006, for the above referenced property.
C&M Engineering Service conducted a thorough site investigation, and review of onsite documents to
verify the design conditions indicated on the previously approved permit.
A record search using the department's On-site document search system revealed no permitted well or
septic modifications on the lots to the South and East of the proposed system (Lots, 5A,9A, and 11 A)
since previous approval of the permit. Recreation Center Tracts 87 and B2 are to the North and West of
Lot 7A, respectively. Tract B1 remains un -developed, and Tract B2 is served by Public Water and Sewer.
A site visit confirmed the neighboring well and septic systems are located approximately as shown on the
permitted site plan. Additionally, the site visit yielded the following observations:
- No foreseen impacts to or from nearby well and septic systems.
- No drainage concerns pertaining to the proposed well and septic.
- No steep slopes near the proposed septic.
- No surface waters observed near the ke
A new test hole was dug on 6/2712013 adjacent to test hole #1 to verify sub -surface conditions. The soils
were visually verified to match the conditions documented at test hole#1. Groundwater was encountered
at approximately 7' below ground surface. The hole was dug to a depth of approximately 9', and a
groundwater monitor tube installed. On 06129/2013, the groundwater table was measured to be
approximately 6.6' below ground surface.
At its current level, the required 4' separation between ground water and the bottom of the proposed;
disposal field will be maintained. ! B
Based on our field investigation, and review of available documentation. We believe the previously
permitted design is still valid, and request a permit be re -issued. In the event of a change of conditions
requiring a design change, a complete re -design will be required as C&M engineer n&i TN1A q?n

Absorption Trench to Lot Line
Dear On Site Services Engineer:
The south end of the western absorption trench on Lot 7A, Block 2, Fire Lake
Subdivision was inadvertently placed at 7' from the west property line. The trench
does not impact the property to the west as it is a large tract with more than 100' from
all wells in the area and more than 100' from all septic systems in the area. The
subsurface soils in the absorption area are absorb water well and little migration of
septic effluent is expected to the west. We recommend a waiver be issued allowing
the trench to remain 7' from the west property line.

7 -Day groundwater monitoring has not been completed. The groundwater elevation shall be
monitored prior to construction to verify 4 feet vertical separation to the bottom of the proposed
drainfield. If the separation is found to be less than 4 feet, an approved change order shall be
acquired prior to construction. Results shall be submitted with the final inspection report.

Dear Onsite Services Engineer:
A well and septic system construction permit was originally issued for this property on
August 23, 2006 with Gayness Engineering Group as the design engineer. It
subsequently expired but was reissued on July 1, 2013 with C & M Engineering
Services listed as the design engineer. The design did not change from its original
format in 2006. The current developer of the property, Troy Davis Homes has enlisted
Anderson Engineering to assist with the construction of the well and septic system on
the lot. They also intend to relocate the well to the eastern property line. A change
order has been submitted request the relocation of the well. The septic system will be
constructed in conformance with the current design and permit. Any modifications will
result in the submittal of a change order. Anderson Engineering will complete the
inspection and certification of the septic system construction.
The ground surface on the lot in the area of the septic system slopes to the northwest
at a very shallow grade. The new trenches will be constructed parallel with the slope as
much as possible in conformance with Municipal requirements. The new septic system
will be constructed a minimum of 100' from all wells and any surface water and 10' the
proposed and existing water services in the area.
The new location of the well allows for sufficient room on the property to the east for an
upgrade septic system site. It will not impact either the well or septic system on the
property to the east.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells

An On -Site Water/Wastewater Permit, number SW060277 issued by this office for a single-family
system, will expire on 23 August 07. Permits are valid for 365 calendar days from date of issue.
If this was a well permit and the well has been drilled, a well log and a pump install log must be sent
to this office for documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be
sent to this office for review, approval and documentation. This as -built inspection report must be
signed by the licensed Professional Engineer who inspected the installation of the system. As -built
inspection reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A one-time renewal fee for a
second year may be issued for a $115.00 if the renewal application is received on or before the
expiration date of the original permit. When applying for a new permit, the fees are $460.00 for a
wastewater permit and $175.00 for a well permit.

To whom it may concern:
The proposed 3 bedroom house will be served by a private well and septic system. We are proposing to
install a new 1250 gallon S.T.E.P. tank and a dual 5 -wide trench type drainfield. Two test holes were
excavated on the property. The drainfield will be designed around the 30 foot radii of these test holes.
Comments regarding the design are summarized as follows: I
1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring,' and the
percolation test results. I
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic system.

4. TOPOGRAPIIY: As can be seen on the attached design drawing the area around the proposed
drainfields are fairly flat. In short, there are no concerns.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems.
