feet.
This waiver approval applies to the existing septic tank to private well separation only.
Any future upgrade to the on-site wastewater disposal system will require all separation
distances be met or another approval from this department.

Well to Tank Waiver
Dear Mr. Roth,
There are two septic tanks on this property which fall with in the 100' protective well
radius.
The first tank is a 1000 gallon tank that was installed in or before 1970. It set
approximately 68 feet from the exterior of the well casing. Since this tank was in place in
1970, it is my understanding that a waiver is not required for this tank.
The second 500 gallon tank was installed in 1983. This tank sets approximately 72 feet
from the well. Though this tank was installed under a Municipal permit, a tank to well
waiver is being required in order to bring this system into conformance with the State's
mandated requirements.
The following covers our evaluation of the criteria for granting a well waiver

The well is to reported to be 90' deep and the existing water table (on 4/15/2000)
was 43 feet below the ground. The house has been continuously occupied since its
construction in 1970. The well water was tested in April and showed no signs of
contamination. Coliform was zero, other bacteria was zero, and nitrates were 2.23
which is well below the drinking water standard of 10 mg/ml.
The distance from the bottom of the system to the bottom of the water table is
estimated at 40 feet. (Point Value = 5.1)

In reviewing well logs for adjacent lots (14 & 16), as well as other lot nearby, the
soils range from clays & silts to gravelly silts between the surface and the water
table. {Point Value = 5.0)

The silty/clay soils which are described in the well logs generally have percolation
rates greater than 50 minutes per inch. We expect a majority of the soils between
the bottom of the system and the water table to fall with in this range.
(Point Value -- 3.0)

The water table gradient is from east to west. The water table on the lots to the
west is reported to be about the same depth below ground as on the subject lot. We
have estimated the gradient at 10%. {Point Value = 6)

The horizontal separation between the tank and the well is approximately 72' feet.
(Point Value --2.9)
The total Point Value that we have calculated is 22.0. This value indicates a very Iow risk
of contamination from household sewage. This is supported by the fact that the tank has
been in place and in use for over 17 years with out any contamination problems. We
recommend approval of the requested waiver.

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.
4. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation,
based on procedures outlined in the Certificate of On-Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe. functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the information
obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water
supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes,
ordinances, arid regulations in effect at the time of installation.

Engineers Comments: In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in
accordance with MOA DSD Guidelines & Regulations. The reported results describe the performance of the system under the conditions
encountered at the time of the test, and separation distances measured to readily identifiable features.
The operational life of all wells and septic systems depend on the local soil condition, ground water
levels that may fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of this system. All systems eventually fail and
satisfactory test results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. PES can therefore not provide any warranty for future
performance nor give any estimate of how long the system will continue to meet the operational
requirements of the MOA DSD. The content of this report is for the sole benefit of the owner listed
above. Any reliance upon or use of this report by any other person or party is not authorized nor will it
confer any legal right whatsoever.

In conducting this evaluation, GEG, LtD. atternpto~ to provide a thorough,
conscientious engineering analysis of the system in ~ccordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the b'me of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate dudng tho year, and the water usage of the family being served by the system.
These conditions are outside the control of the eva/uator of the system. Satisfacto/y test
results do not guarantee future performance of the syst6m, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of bow long the system will continue to meet the
operational requiramonts of tho ADEC or MOA DSD. The content of this report is for

Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates cf Health Authority Approval are required for the b'~nsfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues HAAs upon request to' homeowners. Certificates of Health Authority Approval are
valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued with
new water sample results. (Certificates may be reissued for a period of up to one year with valid water samples.)
Certificates are valid for one year for properties served by Class A or B wells or a public water system. The
Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

Percolation Test on Existing Disposal System; Lot 15, Block C, Knik
Heights subdivision
Dear Mr. Moon.'
On August 24, 1976, at yourrequest, our office conducted a percolation
test on the above described lot.
The following table represents the level of the liquid in the tank. The
depth of the tank was 8.2 feet below the vent level. The depth of
liquid in the tank was initially 3.1 feet above the bottom and 5.1 feet
below the top of the stand pipe. The tank had not been pumped prior to
the test and was, therefore, assumed to be at the outlet level. For the
test, the liquid level was measured from the top of the stand pipe.

The meter used during the test was a Neptune 5/8" standard water meter
which was borrowed from the Anchorage Water Utility. The meter had
previously been calibrated and approved for installation by the utility
company.

Since the house on the lot concerned was occupied, it can be assumed
that the leach field in question was at its normal degree of saturation.
To further insure normal saturation and to fill any possible storage in
the system, 100 gallons of water were added to the tank before beginning
the percolation test.
If it is assumed that the existing three bedroom residence will house
five people, the average daily load on the disposal system can be expected
to be 375 gallons per day. The maximum hourly load on the system would
be about 0.8 gallons per minute (gpm).
Since the existing disposal system accepted 422 gallons in 2.2 hours or
3.2 gpm without a level rise, I would conclude that the leach field is
presently performing in a satisfactory manner for a private residence.
