Dear On -She,
Per .your request this letter is to address an undocumented drainfield on the property referenced above.
The drainfield in question was originally discovered by the T-im Ecklund, the owner of Lot 19 while he
was installing EPDM membrane to stop water from coming onto his property. As part of the design for
the new drainfield for La'. 19 we were instructed to verify the existence of the undocumented drainfield.
Our original intent was to verify this, call Ort -Site to come out for verification but the amount of water
and soil caving in required our immediate installation of EPDM and closure of the site. We did take
photos and videos which were forwarded to Mr. Ecklund.
Upon excavation we found thatthere, yes, is a drainfield with aboutSOO gallons of fluid. The total depth
was approximately &' deep. There was about a 3' wide by 4' deep cross section of sewer rock covered
with fabric with discoloring around the trenches edges_ The length of the field could not be determined.
Mr. Ecklund claims when hefirst discovered this that is was approximately 3' on his property and only
inches away from his original drainfield. Mr- Ecklund claims it contributed to the failure of his original
drainfield. This is a reasonable assumption.

I have done the field final inspection and have all the as built information. It was all done according to our
revised design. The surveyor has not finalized the as built and that is what is holding up the final submittal. My
last conversation with Tim Ecklund was that the system was working well with no accumulation of effluent in the
bed.

Dear On Site Services Engineer:
The total area of this lot is 19,814.5 square feet. The placement of the well restricts the
area available for a septic system to a small section along the eastern lot line. The
usable area is further restricted by the steep slopes and surface water at the northern
boundary of the lot. An Advantex Treatment System has been designed for the lot, but
the absorption bed must be placed at 0' from the east property line to provide the
necessary separation from the well. The use of the nitrate reducing system insures
proper treatment of the septic effluent and reduces the possibility of system failure or
damage to adjacent properties. We therefore recommend a waiver be issued allowing
the placement of the bed at 0' from the east property line.

Special Provisions: PRIOR TO CONSTRUCTION: A groundwater monitoring tube shall be placed
prior to construction to verify the drainfield's required vertical separation to groundwater. After
seven days from the day of installation, the On-site Water & Wastewater Program shall check
the groundwater elevation to determine if the drainfield is in accordance with the code regarding
drainfield to groundwater separation requirements. The results shall be included with the final
inspection report.
Special Provisions: Well Easement: The well easement shall be recorded with the State of Alaska
prior to final approval of the well.

Impacts to Adjacent Properties
Dear Onsite Services Engineer
The existing absorption bed system on Lot 19, Block 1, Williamson Subdivision Unit No.
1 must be expanded due to slow percolating soils and the presence of organic fill which
will not allow the absorption and free movement of subsurface water. We are therefore
modifying the existing to expand the system. The attached Site Plan and backup
documentation identify the location and configuration of the proposed absorption bed.
No existing wells are closer than 100' from the proposed absorption bed. The surface
drainage on the lot is to the northeast as shown by the drainage arrows on the plan.
The drainage pattern will not be changed as a result of this project.
The new absorption bed will be constructed with Engineered Receiving Soils (E.R.S.) in
accordance with proposed AMC 15.65.201.D3c and an Advantex Treatment System
with an AX -20 Filter Pod. The bed will be placed at 3.5' below existing ground. A
minimum of 2' of coarse sand will be placed below the bed and 5.5' of clean well
graded gravel (GW) to a depth of 11.5' below the surface.
We are proposing to construct a new 1,200 square foot by .5' effective depth absorption
bed to treat and dispose of the effluent generated in the house. The additional porous
material beneath the bed will provide surge volume, treatment and increase the surface
area for water absorption to the surrounding strata. It is essential the organic fill
material be removed and replaced with granular free draining fill to facilitate the
movement of water through the area.
The ground surface on the lot in the area of the absorption bed is flat with a slight bias

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The owner of Lot 19, Block 1, Williamson Subdivision No. 1 intends to construct a
three-bedroom home on the lot. We are therefore requesting a permit be issued for a
well and septic system to serve the new home. The available area for the septic system
is limited and groundwater elevations have been found close to the surface which
makes it difficult to place a conventional septic system. We have therefore decided to
place an Advantex Treatment System on the lot to provide the most effective treatment
of the septic effluent prior to discharge into the surrounding soil strata. The location of
the absorption system is shown on the attached Site Plan. Drainage patterns are also
shown on the plan and will not be modified as a result of the proposed construction.
Seasonal surface water was noted in the northeast corner of the lot at 60' from the
proposed alternate absorption system site and 88' from the primary site. A waiver
allowing the absorption systems to be placed as designed has been requested.
The test hole placed on this lot in 2004 revealed poorly sandy silt with small amounts of
gravel (ML). The percolation rate in the materials encountered was 45 to 60 minutes
per inch. Groundwater seeps were found at 4' below the surface during the excavation.
A substantial amount of fill was placed on the lot in the summer of 2005 and the surface
regraded to channel drainage away from the septic system area. The groundwater
elevation was measured on May 15, 2012 at 8' below the surface. A second test hole
was placed at that time in the same silty material. The groundwater was also found at
8' below the surface. Our design provides for the bottom of the absorption trench to be
as shallow as possible. We are therefore proposing to place a three-bedroom
Advantex Treatment System with 2-22.5' long x 5' wide x no effective depth absorption
trenches. The distribution line in the trenches will be placed at the existing ground
level. Coarse sand will be placed after the organic layer is removed to bring the
excavated area to original ground level. The alternate site will be an absorption bed

will be 20' long x 15' wide. The distribution line in the bed will be placed at the existing
ground level. Coarse sand will be placed after the organic layer is removed. Topsoil
and seed will be placed in the top 6" of the 3' of fill to be placed above the bed. A
minimum of 4' of fill will be placed over the septic tank to meet Municipal Code and
prevent freezing.
The ground surface of the lot slopes at 10% to the southeast near the location of the
primary absorption bed. The bed will be constructed parallel with the slope as much as
possible in conformance with Municipal requirements. All components of the new
septic system will be constructed a minimum of 60' from any surface water in
accordance with the approved waiver and 100' from the well proposed for this lot. No
conflicts exist between the proposed well and septic system and those in the area.

The total area of this lot is 19,814.5 square feet. The placement of the well restricts the
area available for a septic system to a small section along the eastern lot line. The
usable area is further restricted by the steep slopes and surface water at the northern
boundary of the lot. An Advantex Treatment System has been designed for the lot, but
the absorption bed must be placed at 6' from the east property line to provide the
necessary separation from the well. The use of the nitrate reducing system insures

ADVANCED WASTEWATER TREATMENT SYSTEM
MAINTENANCE AND REPAIR AGREEMENT
BETWEEN MUNICIPALITY OFf ANCHORAGE
THIS MAINTENANCE AND REPAIR AGREEMENT made and entered into/as of this

herein the "OWNER," and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Maintenance and Repair Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. Municipality grants permission to
Owner to utilize and operate an Advanced Wastewater Treatment System
(A W WTS), described as

2. Definitions.
Alteration. Any change to the design or function of an AW WTS that
includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design.
Certificate of On -Site Systems Approval (COSA). An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with Anchorage
Municipal Code (hereinafter, "AMC') 15.65. These approvals certify that
the systems are adequate for the homes that they support and meet the
codes that were in place at the time of system construction.
Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AWWTS that were included
in the original design which would allow the AWWTS to continue to
perform as designed.
Permit. An On -Site Wastewater Disposal Permit as required by AMC
15.65 to construct and operate an AWWTS.
3. Term. The term of this Maintenance and Repair Agreement shall begin on the
date of approval by the Municipality to operate the installed system or issuance of a
COSA, and shall continue while the AWWTS is in use or is operational or until the
property is sold or title is transferred by the owner and a new COSA is issued to the
new owner or transferee of the property.
4. Alterations. Installation and Removal of Additional EOnipment. Prior to
performing any alterations to an AWWTS, the owner agrees to obtain an On-site
Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65.
5. Maintenance and Repairs.
A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall
maintain their AWWTS in a satisfactory condition capable of producing treated
septic effluent in accordance with the equipment's approval for operation in the
Municipality. The owner shall enter into a service agreement with an AWWTS
service and maintenance provider approved by the municipality and the
manufacturer of the A WWTS for the entire term of the AW WTS In addition, it
shall be the responsibility of the Owner during the term of this Maintenance and
Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay
for any and all: (I) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement
costs, and (5) inspection costs.
B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules
and orders for the A W WTS.
C. Upon request by the Municipality, the owner agrees to provide the Municipality a
written schedule of routine maintenance and repairs which have been performed
on the system. When a record of maintenance is documented and maintained by
the system vendor, the owner agrees to allow the Municipality access to this
information.
D. Owner acknowledges that the fine for failing to maintain and repair an AW WTS
may be assessed in accordance with AMC 14.60 for improper discharge.
E. Owner agrees that only maintenance and repair personnel approved by the
Municipality will inspect and make any necessary maintenance, repairs or
permitted alterations to the system.
F. Owner agrees to grant the Municipality reasonable access to test and inspect the
A W WTS upon 24 hours written notice.
G. Owner agrees that any sate or transfer of title of the property will not occur
without a new Certificate of On -Site Systems Approval.
H. Owner agrees that the A W WTS installation and maintenance requirements as
provided by the A W WTS vendor/installer and approved by the Municipality are
the governing guidelines for the construction, maintenance and repair of the
Owner's A W WTS.
6. Nonwaiver. The failure of either party at any time to enforce a provision of this
Maintenance and Repair Agreement shall in no way constitute a waiver of the
provisions, nor in any way affect the validity of the Maintenance and Repair
Agreement or any part hereof, or the right of such party thereafter to enforce each and
every provision hereof.
7. Amendment.
A. This Maintenance and Repair Agreement shall only be amended, modified or
changed by a writing, executed by authorized representatives of the parties, with
the same formality that this Maintenance and Repair Agreement was executed
with, and such writing shall be attached to this Maintenance and Repair
Agreement as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this agreement, the only authorized representatives of the parties
are:

C. Any attempt to amend, modify, or change this contract by either an unauthorized
representative or unauthorized means shall be void.
8. Jurisdiction: Choice of Law. Any civil action arising from this Maintenance
and Repair Agreement shall be brought in the Superior Court for the Third Judicial
District of the State of Alaska at Anchorage. The laws of the State of Alaska shall
govern the rights and obligations of the parties under this Maintenance and Repair
Agreement.
9. Severabilitv. Any provisions of this Maintenance and Repair Agreement decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining
provisions of the Maintenance and Repair Agreement.

Due to the likelihood that there will be some minor design revisions; except for the current permit application page, On-site
will need a complete new submittal (cover letter, waiver request, design, soils reports, specifications).
Per my previous review and our meeting this morning, the following items will need to be incorporated into the permit.
1. 7 -Day Groundwater Monitoring: There will be a special provision added to the permit requiring 7 -Day Groundwater
Monitoring to be completed and approved by the Department prior to construction of the septic system. Please
add the requirement to your design too.
2. Existing Curtain Drain: There will be a special provision added to the permit requiring the curtain drain to be
removed prior to construction of the septic system. The drain removal will be required to be inspected and
documented by the engineer, and a least one two-hour inspection notice given to On-site for an opportunity to
inspect the bottom of the excavation in the portion of the curtain drain that is closest to the proposed drainfield.
Please add the requirement to your design too.
3. Drainfield — Surface Water Waiver: Please request and pay the fee.
4. Grading: Please add a requirement to the design for the subject property to be graded such that no surface water
will collect or pond within the minimum distance established by the surface water waiver (see 3 above).
5. AWWTS Maintenance Agreement: Please provide.

In addition to the items we discussed at this fill site we need to be concerned about the slope of
the fill and the possibility of dirt moving down hill to an adjacent home. I think there should be
some kind of silt fence and perhaps we need to look at the file to see what was required.

An On -Site Water/Wastewater Permit, number SW070305 issued by this office for a single-family
system, will expire on 08 November 08. Permits are valid for 365 calendar days from date of issue.
If this was a well permit and the well has been drilled, a well log and a pump install log must be sent
to this office for documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as -built inspection report must be
sent to this office for review, approval and documentation. This as -built inspection report must be
signed by the licensed Professional Engineer who inspected the installation of the system. As -built
inspection reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A one-time renewal fee for a
second year may be issued for a $115.00 if the renewal application is received on or before the
expiration date of the original permit. When applying for a new permit, the fees are $460.00 for a
wastewater permit and $175.00 for a well permit.

Well and Septic System Permit
The groundwater level was measured in Test Hole No. 3 on October 15 and found to be
3' below the surface. The area surrounding the test hole is suitable for a raised
absorption bed since the surface slope is less than 10%. The bed will be 15' wide by
25' long and require a minimum of 1' of sand to raise it above ground level to provide 4'
of separation from ground water. The alternate site may never be required for this
project as an Advantex Treatment System has been designed. If the alternate site is
required it will be at least 10 years before it is necessary. The disturbance caused by
the test hole will be completed settled by that time and placement of the bed over the
test hole will not be a problem.
The design for the primary absorption system will include 2-5' wide absorption trenches
at the location shown. No groundwater was found in Test Hole No. 2 on October
15 nor was any found on May 15 as noted on the test hole log. The ground
elevation of Test Hole No. 2 is more than 10' higher than the ground elevation of Test
Hole No. 3 which may explain the difference in groundwater levels.

Attached is the revised design for the septic system for Lot 19, Block 1, Williamson
Subdivision No. 1. The surface slope in the area of the new absorption trench was
measured at slightly more than 10%. We have therefore revised the design to include
2-5' wide absorption trenches rather than an absorption bed. We have included a new
test hole at the location of the alternate site.
The well has been moved to the west to allow 100' separation from all components of
the septic system.

Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The owner of Lot 19, Block 1, Williamson Subdivision No. 1 intends to construct a
three-bedroom home on the lot. We are therefore requesting a permit be issued for a
well and septic system to serve the new home. The available area for the septic system
is limited and groundwater elevations have been found close to the surface which make
it difficult to place a conventional septic system. We have therefore decided to place
an Advantex Treatment System on the lot to provide the most effective treatment of the
septic effluent prior to discharge into the surrounding soil strata. The location of the
absorption system is shown on the attached Site Plan. Drainage patterns are also
shown on the plan and will not be modified as a result of the proposed construction.
The test hole placed on this lot in 2004 revealed poorly sandy silt with small amounts of
gravel (ML). The percolation rate in the materials encountered was 45 to 60 minutes
per inch. Groundwater seeps were found at 4' below the surface during the excavation.
A substantial amount of fill was placed on the lot in the summer of 2005 and the surface
regarded to channel drainage away from the septic system area. The groundwater
elevation was measured on May 15 of this year at 8' below the surface. A second test
hole was placed at that time in the same silty material. The groundwater was also
found at 8' below the surface. Our design provides for the bottom of the absorption
trench to be as shallow as possible. We are therefore proposing to place a three-
bedroom Advantex Treatment System. The absorption bed will be 20' long x 15' wide.
The distribution line in the bed will be placed at the existing ground level. Coarse sand
will be placed after the organic layer is removed. Topsoil and seed will be placed in
the top 6" of the 3' of fill to be placed above the bed. A minimum of 4' of fill will be
placed over the septic tank to meet Municipal Code and prevent freezing.
The ground surface of the lot slopes at 10% to the southeast near the location of the

primary absorption bed. The bed will be constructed parallel with the slope as much as
possible in conformance with Municipal requirements. All components of the new
septic system will be constructed a minimum of 100' from any surface water and 100'
from the well proposed for this lot. No conflicts exist between the proposed well and
septic system and those in the area.
If the system is constructed in accordance with our design the following statements
apply:
1. The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

To whom it may concern:
The existing drainfield is approximately 17 feet from slopes greater than 25%. As can be seen on
the attached profile drawing, a 25% line drawn 2 feet above the top of rock, does not intersect until
39.5 feet. Additionally, during our site visits on 5/26/2016 and 6/30/2016 no daylighting was
observed. We are requesting that your department wavier the required separation distance of 50
feet to slopes greater than 25% down to 17 feet.
As can be seen there is minimal risk in granting this waiver. We are unaware of any adverse
impacts associated with this waiver. Please call or email if you have any questions.
