Ref: Waiver Request from Tank to Lot Line for Bernard S/D; Tract F-1
To whom it may concern:
We are requesting a 0 foot waiver from the edge of the tank to the North property line. During
our initial inspections of the tank, the contractor did not have the north lot line flagged by a
registered land surveyor and proceed at his own risk. We performed our final grade and swing -
tie inspection, after the system was installed and have recently been sent a update as -built
survey; and based upon these documents, we have discovered that the corner of the tank is
approximately 2 feet over the North lot line and encroaches on a 20 foot alley for recreational
purpose. We have notified the contractor and they are in the process of obtaining a letter of non-
objection from the MOA Right -Of -Way department. Once we have received this, we will forward
you a copy. The area to the north is a 20 foot undeveloped alley and the encroachment appears
to have no adver e effects on the dedicated recreational alley. If you have any questions,

The referenced property had its permit expire on January 20, 2010. The owner, John Meno,
would like to have the permit re -issued so that he may install the proposed septic system. Please
note that the well was drilled on 3/10/09 at its proposed location. In regards to the surrounding
area there has been no changes that would affect the septic system design or location. In short,
we request that you re -issue the well and septic permit. The design package is attached.

Ref: Proposed Septic Upgrade for Bernard Subdivision; Tract F-1,
To whom it may concern:
The existing 3 bedroom house is served by a community water system that is being abandoned. We are
proposing to install a private well. The existing septic system is undocumented and needs to be upgraded.
We are proposing to install a 1250 gallon S.T.E.P. tank, and dual 5 -wide shallow trench type drainfields
with sand filters. One test hole was excavated on the property. The drainfields will be designed around the
30 foot radius of the test hole. Comments regarding the design are summarized as follows:
1. SOILS: See the attached logs which show the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There was no visible surface water with in 100' of the proposed septic system,
however, a ditch line exists that may have surface water in the spring. We propose to be 100' away from
this ditch line.
4. TOPOGRAPHY: As can be seen on the attached design drawing the average topography in the area
below the drainfield is approximately 5% running south to north. In short, there are no slope concerns with
this installation.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems. If

Ref: Proposed Septic Upgrade for Bernard Subdivision; Tract F-1,
To whom it may concern:
The existing 3 bedroom house is served by a community water system that is being abandoned. We are
proposing to install a private well. The existing septic system is undocumented and needs to be upgraded.
We are proposing to install a 1250 gallon S.T.E.P. tank, and dual 5 -wide shallow trench type drainfields
with sand filters. One test hole was excavated on the property. The drainfields will be designed around
the 30 foot radius of the test hole. Comments regarding the design arc summarized as follows:
1. SOILS: See the attached logs which show the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAINFIELD DESIGN: Sec attached design drawing for drainfield specifications.
3. SURFACE WATERS: There was no visible surface water with in 100' of the proposed septic
system, however, a ditch line exists that may have surface water in the spring. We propose to be 100'
away from this ditch line.
4. TOPOGRAPHY: As can be seen on the attached design drawing the average topography in the area
below the drainfield is approximately 5% running south to north. In short, there are no slope concerns
with this installation.
We are unaware of an adverse impacts this installation would have on adjacent wells or septic systems.

guidelines and regulations established bythe Municipality of Anchorage and industry practices. The reported results describe the
