converting CAT III AWWTS to CAT II AWWTS septic system
To whom it may concern:
The permitted septic system was installed on the referenced property on 5/22-29/2014 and
was started up shortly after that. ARM Septic Services, LLC checked the monitoring tubes in the
drainfield in May of 2019 and found them to be dry. In order to simplify the overall operation
and maintenance of the system, we are proposing that this unit be permanently configured
as a CAT II system. Our justifications are as follows:
The receiving soil has percolation rates of 80 and 120 minutes per inch. Given these percolation
rates, an application rate of 0.5-1.0 gpd/sq.ft. application rate is required for a Cat III system and
a 0.3-0.5 gpd/sq.ft. application rate would be required for a Cat II system. There is 4.25 to 6.05
feet of MOA approved filter sand below the drainrock in this drainfield. After the effluent passes
through 4+ feet of MOA filter sand, the effluent should meet Cat III effluent quality levels at the
interface with the insitu soils.
The residence on this property has a total of 4 bedrooms, so 1200 sq./ft. of absorption area is
required for CAT III effluent (600 gpd / 0.5). The existing 2014 drainfield has 1200 sq./ft. of
absorption area. In short, the drainfield meets the Cat III effluent requirements.
At this time, we request that your department approve a CAT II configuration be used at the
referenced property. Upon your concurrence, we will have ARM Septic Service, LLC, either
leave the Jandy valve actuator in the discharge position; or have them completely remove the
actuator and leave the valve in the open position. We will also have them place a CAT II sticker

Ref: Moose Run Lot 2, Permit ft OSPI31435 ; Waiver Request for Separation to Ground Water —
Using CAT III Advanced Treatment System.
To whom it may concern,
We are requesting that the required vertical separation distance from the bottom of the proposed
drainfield to groundwater be waived to 3'. Justification for the approval of this waiver includes:
• A curtain drain was installed on the property in 2012, and the property has continued to show
signs that it is drying out.
• GEG used a conservative groundwater elevation observed during October of 2013, which is
reflected on our design drawing. Groundwater was monitored again this spring, and was found to
be approximately 1' lower in elevation.
• This system utilizes a Category III advanced wastewater treatment system unit. Before reaching
the drainfield, the effluent will pass through approximately 2'+ of foam media, then the effluent
will pass through a minimum of 3' of MOA filter sand. In short, the wastewater must pass
through five (5) feet or more of media (foam cubes & soil) before reaching groundwater.
• The effluent exiting the aerocell trickling filter will be treated to ADEC secondary standards or
better. In addition, the fecal coliform levels are typically 1-5% of that in standard septic tank
effluent. In short, the effluent is treated to such a level that a reduced separation distance to
groundwater is fully justified.
With the above items taken into consideration, we request that the required vertical separation distance
from the bottom of the proposed drainfield to groundwater be waived to 3'.
With the rotation of the drainfield to provide the maximum amount of room for a reserve site, a slight
encroachment was encountered between the Northwest corner of the bed, and the closest cutbank near the
road (see attached drawing). During a recent conversation with Jeff Poet from your department, he
conveyed that it was acceptable for the required separation distance of 50' from the drainfield to the
cutbank be waived. According to our measurements, the distance is currently 46'. We request a waiver be
granted for a separation of 40' to cutbanks.
Due to the mounding of the drainfield, a significant amount of material will be needed to provide the 3:1
slopes coming off of the drainfield. We are requesting that the requirement for 3:1 slopes be waived to 2:1
on the West side of the drainfteld only.

To whom it may concern
The referenced permit has expired, and we are requesting that your department re -issue the permit so that
the proposed wastewater system can be installed within the next couple of months. It is important to note
that this summer when the house foundation was constructed, it was observed that both of the test hole
monitoring tubes had been destroyed. Per a meeting with your department, it was decided that new
monitoring tubes would need to be installed, and seasonal high groundwater be monitored through
October.
The original test hole locations (based off on an as -built) were re -located by a surveyor, and new
monitoring tubes were installed within 1' of their original locations with Garness Engineering's oversight.
October groundwater monitoring was also completed in these test holes, and the groundwater elevations
were established and tied into a permanent benchmark (the garage slab of the new house). It is also worth
mentioning that the elevations called out on the original design are null and void as the control point was
wiped out by a contractor earlier this year. We intend to use our new elevation shots, groundwater
monitoring, and the new bench mark to layout the system as to provide the correct separation to
groundwater.
At this time, there are no major design changes, and we are unaware of any adverse impacts that this
installation would have on adjacent wells or septic systems. If you have any questions, please contact us

The referenced property is currently vacant. The owner is proposing to build a 4 bedroom house that will be
served by a private well and septic system. We are proposing to install a Quanics Aerocell treatment system on
this property which includes the installation of a 1500 gallon S.T.E.P. tank and bed type drainfield. Attached
are a site plan and a design drawing showing the proposed septic system. Two test holes were excavated on the
property. The drainfield will be designed around the 30 foot radii of these test holes. An additional
confirmation test hole will be required for to reserve site. Comments regarding the design are summarized as
follows:
1. SOILS: See the attached logs which show the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface water concerns.
4. TOPOGRAPHY: As can be seen on the attached topography, the average topography below the drainfield
is relatively flat. Please note, the topography drawing shows a cutbank near the road. This is not a cutbank but
a pile of dirt that was placed at the edge of the property when the road was cut in. Attached are pictures of the
berm.

To whom it may concern:
A permit application for the subject property was first submitted to your department on March 21, 2011.
On March 23, 2011 a comment sheet was returned to us by Jay Crewdson, P.E. with 6 items mentioned 4
of which required a response, all of which have been met. Comment 5 requested a site visit between the
On-site department and GEG. The site visit was preformed during the Spring of 2011. It was determined
at that time that a curtain drain would be required to be installed prior to issuance of a septic permit. The
curtain drain was installed during the fall of 2011 and a final site inspection was made on June 26, 2012.
Elevations were taken of original grade (see attached drawing for elevations for proposed bed). It should
also be noted that the testholes are within the footprint of the proposed bed. The testholes will be
excavated out and be filled with compacted MOA approved sand. At this time we request that your
department issue the well and septic permits for the referenced lot.

width is 20'; at this time we request that your department waive the 15' maximum bed width. It

REPAIR AGREEMENT
MEMORANDUM OF UNDERSTANDING
BETWEEN MUNICIPALITY OF ANCHORAGE

THIS MEMORANDUM mar( -k, UNDERSTANDING made and entered into as of this

herein the "O%ti'NER: and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Memorandum of Understanding agree as follows:
1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality
grants permission to Owner to utilize and operate an Advanced Wastewater
Treatment System (AW WT'S), described as i 1 L.t� t..--r-1L rrjt e._ [_hocated

2. Definitions.
A. Alteration. Any change to the design or function of an AWWTS that
Includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design. Prior to
performing any alterations to an AWWTS the owner must obtain a
Wastewater Disposal System Construction Permit from the Municipality
pursuant to Anchorage Municipal Code (hereinafter, "AMC") I5.65.
B. Certificate of On -Site Systems Approval. An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with AMC 15.65.
'I'liese: approvals certify that the system s are adequate fbr the homes that
they support and meet the codes that were in place at the time of system
construction.
C. Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
D. Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AWWTS that were included in
the original design which would allow the AWWTS to continue to
perform as designed.
E. Permit (Construction) An on -Site Wastewater Disposal System
Construction Permit as defined by AMC: 15.65.
F. Permit (Operating) An Advanced Wastewater Treatment System
Operating Permit. An annual permit, issued by the Municipality, that
allows the Owner to operate an AWWTS, upon meeting all the
requirements of this agreement, the conditions of Operating Permit, the
requirements of the On -Site Wastewater System Construction Permit and
all relevant provisions of AMC 15.65
3. Fee. Owner shall pay to Municipality an annual fee of
($ 0 .00), payable on or before the issuance of the operating permit and annually
thereafter. The annual Ice is due on or before the anniversary date of the approval by the
Municipality of installed system.
4. Term. The term of this Memorandum of Understanding shall be for the lil.'c of the
AWWTS.
The term begins on the date of approval by the Municipality of the installed system and
shall continue while the AWWTS system is in use or operational or until the property is
,sold or title is transferred by owner and a new certificate of On -Site approval is issued to
the new owner or transferee of the property.
5. Alterations, Installation and Removal of Additional Equipment. Owner agrees
not to make any alterations. removal of parts or additions to the AWWTS without a
Construction Permit from the Municipality.
6. Maintenance and Repairs
A. Throughout the term of this Memorandum of Understanding, the Owner shall
maintain AWWTS in good repair. In addition, it shall be the responsibility of the
Owner during the term of this Memorandum of Understanding, and any extensions or
renewals thereof at the owner's sole expense. to pay for any and all: (I) repair(s), (2)
maintenance-, (3) adjustment(s), (4) replacement costs, and (5) inspection costs.
Further. Owner agrees to comply will all applicable ordinance, laws, regulations,
rules and orders for the AWWTS.
B. Owner agrees to provide the Municipality a written schedule of routine
maintenance and repairs which have been performed on the system pursuant to the
terms and conditions contained in the Owner's AW)XTTS Operating Permit. This
schedule shall be submitted to the Municipality annually upon the renewal of the
permit. The schedule of maintenance and repair contained in the Owner's AWW I'S

C. Owner acknowledges that the fine schedule for failing to maintain and repair an
AWWTS are codified in AMC 14.60.
D. C)w-nLr agrees that only maintenance, repair personnel certified by the
Municipality will inspect and make any necessary maintenance, repairs or permitted
alterations to the system.
E. Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
F. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On -Site Systems Approval.
GT. Owner agrees that the relevant provisions of the standard specification guidebook
for AWWTS is the governing professional guidelines for the construction,
maintenance and repair of the Owner's AWwrs.
7. Nonwaiver. The failure of either party at any time to enforce a provision of this
Memorandum of Understanding shall in no way constitute a waiver of the provisions,
nor in any way efibct the validity of the Memorandum of Understanding or any part
hereof, or the right ol'such party thereafter to enforce each and every provision
hereof.
8. Amendment.
A. This Memorandum of Understanding shall only be amended, modified or changed
by a writing, executed by authorized representatives of the parties, with the same
formality of this Memorandum of Understanding was executed and such writing shall
be attached to this Memorandum of Understanding as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of contract, the only authorized representatives of the parties are:

C. Any attempt to amend. modify. or change this contract by either an
unauthorized representative or unauthorized means shall be void.
9. Jurisdiction: Choice of Law. Any civil action arising from this Memorandum of
Understanding shall be brought in the Superior Court for the Third Judicial District of the
State of Alaska at Anchorage. The laws of the State of Alaska shall govern the rights and
obligations of the parties under this Memorandum of Understanding.
10. Severabiliri. Any provisions of this .Memorandum of Understanding decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining provisions
of the Memorandum of understanding.
