Comments: This investigation was completed in compliance with MOA guidelines, regulations,
and best industry practices / methods. The assessment of the condition of the well and septic
applies only to the conditions as of the day tested. The flow and absorption rates may change
due to subsurface conditions that may not be observed from the surface, changes in land use,
local soil characteristics, groundwater levels that may fluctuate during the year, quality of
construction (workmanship & materials), the water usage of the family being served by the
system and maintenance. The operational life of all well and septic systems are subject to
these various and dynamic characteristics and are outside the control of the evaluator of the
well and septic system. Therefore, any or NO estimate of how long a system will function satisfactory
for current or future occupants or guarantee that no unseen encroachments, deficiencies or

RE: END OF THE ROAD SUBDIVISION LOT 3
Due to wintertime conditions, only the far end septic field pipes could be located. Various
attempts were made along with a metal detector, existing swing tie measurements, etc.… to
locate these pipes but to no avail. The owner said these pipes were at or below grade in the
frozen soils.
The existing 12-year-old steel septic tank will not require a MOA tank advisory, and it will be
pumped this spring by June 1st at the latest once conditions improve. The existing MOA record
documents has a sufficient as-built survey to issue the COSA, but a new one is planned for this
spring when all the pipes can be located.
We respectfully request the COSA to be issued & these items will be completed this spring of
2026 and subsequently submitted to MOA. Please contact us if you have any questions.

1 ALL CONSTRUCTION SHALL BE IN ACCORDANCE WITH CHAPTER 15.65. WASTEWATER DISPOSAL REGULATIONS
OF THE MUNICIPAL CODE, AND MOA STANDARD SPECIFICATIONS FOR STREETS, DRAINAGE, UTILITIES AND PARKS
(MASS) 2009 AS CURRENTLY AMENDED, AND ALL APPLICABLE STATE AND FEDERAL REGULATIONS.
2 CONTRACTOR TO INSTALL 1500 GAL, TWO COMPARTMENT, STEEL SEPTIC TANK
3 SITE TOPOGRAPHIC SURVEY WAS PREPARED BY AERIAL TOPOGRAPHY EXISTING STRUCTURES, WELLS, SEPTIC
SYSTEMS OBSERVED WITHIN 200 FT. OF THE PROPOSED SYSTEM SITE ARE SHOWN ON DESIGN DRAWINGS.
4 ANY DEVIATION FROM THE DESIGN MUST BE AUTHORIZED BY THE ENGINEER PRIOR TO ITS INCORPORATION
INTO THE SYSTEM.
5 CONTRACTOR TO LOCATE ALL UNDERGROUND UTILITIES, PROPERTY LINES, EXISTING WELLS, WATERWAYS,
SURFACE AND SUBSURFACE DRAINAGE FACILITIES, LAKES, PONDS, AND OTHER FACILITIES REQUIRING
SEPARATION DISTANCES FROM THE PROPOSED SEPTIC SYSTEM.
5 CONTRACTOR TO STAKE PROPOSED SYSTEM & NOTIFY THE ENGINEER OF ANY OBSERVED
CONFLICTS PRIOR TO CONSTRUCTION. STAKE LOCATION OF EACH TRENCH END. NOTE DEPTH OF CUT ON LATH.
CONTRACTOR SHALL HAVE SURVEY LEVEL AND ROD SET UP ON SITE DURING CONSTRUCTION.
6 CONTRACTOR TO NOTIFY THE ENGINEER AND DHHS 24 HOURS PRIOR TO COMMENCING WITH CONSTRUCTION.
A MINIMUM OF THREE SITE VISITS ARE REQUIRED. THE FIRST VISIT SHALL BE OF THE SUBGRADE
PRIOR TO PLACING THE SEWER ROCK. THE SECOND VISIT SHALL BE AFTER THE PLACEMENT OF
SEWER ROCK, DISTRIBUTION PIPING, STANDPIPES, AND OTHER COMPONENTS AS SPECIFIED. THE THIRD
VISIT SHALL BE AFTER THE WORK IS COMPLETED.
7 CONTRACTOR SHALL DELIVER TO THE ENGINEER A SET OF RED -LINED, AS -BUILT DRAWINGS SHOWING ALL DATA
AS SPECIFIED IN SEC. 15.65.F.2, 3, 4, 5, 6 OF MUNICIPAL WASTEWATER DISPOSAL CODE WITHIN FIVE DAYS OF
THE FINAL VISIT. INVERTS AND SWING TIE LOCATES SHALL BE TO THE NEAREST TENTH OF A FOOT.
THESE AS-BUILTS SHALL BE USED BY THE ENGINEER TO PREPARE AND SUBMIT RECORD DRAWINGS TO DHHS.
8 ALL EXCAVATION DEPTHS ARE ADVISORY. THEY ARE TO BE VERIFIED AND MAY BE MODIFIED BY THE
ENGINEER IF ACTUAL FIELD CONDITIONS VARY FROM THOSE USED TO PREPARE THE DESIGN.
9 BOTTOM OF TRENCH EXCAVATION SHALL BE LEVEL. SIDEWALLS SHALL BE SCARIFIED. RECORD
ELEVATIONS AT BEGINNING, MIDDLE, AND END OF TRENCH BOTTOM.
10 PLACE THE SEWER ROCK TO THE SPECIFIED DEPTHS. DO NOT CONTAMINATE SEWER ROCK.
LEVEL ROCK SURFACE TO t 0.1' PRIOR TO INSTALLING THE PERFORATED PIPE.
11 ALL PIPE SHALL BE PVC CONFORMING TO ASTM D3034. ALL JOINTS SHALL BE SEALED WITH APPROVED GLUE.
CLEANOUTS SHALL BE INSTALLED AT THE END(S) OF THE TRENCH(S). 4" MONITORING TUBE(S) SHALL BE
INSTALLED AT THE END OF TRENCH, AS SHOWN ON PLANS.
12 COVER THE DISTRIBUTION PIPE WITH MIN 2" SEWER ROCK, BEFORE PLACING INSULATION AND/OR GEOTEXTILE,
FILL, AND TOPSOIL.
13 MINIMUM SOIL COVER SHALL BE 4 FEET OVER THE PIPE AND 4 FEET OVER THE SEPTIC TANK. ONE INCH OF
INSULATION MAY BE SUBSTITUTED FOR 1 FOOT OF SOIL COVER, BUT THERE MUST BE A MINIMUM OF 2' OF SOIL
COVER OVER THE INSULATION. ALL INSULATION BOARD SHALL BE 2" THICK POLYSTYRENE, DOW HI -35
OR ENGINEER -APPROVED EQUAL. CENTER BOARD WIDTH OVER PIPE.
14 AN APPROVED GEOTEXTILE FABRIC SHALL BE INSTALLED BETWEEN THE SEWER ROCK AND THE
BACKFILL.
15 MATERIAL USED AS FILL SHALL BE CLEAN AND FREE OF ORGANICS, TRASH AND CONSTRUCTION DEBRIS.
16 SLOPE ALL FILL MATERIAL AND TOPSOIL TO DRAIN AWAY FROM IMPROVEMENT AT 2% MIN. SLOPE, AND IN SUCH A
MANNER THAT PONDING AT OR NEAR THE DRAIN FIELD & WELL DOES NOT OCCUR. THE FILL SHALL BE LEFT 6"
HIGHER THAN SHOWN TO ALLOW FOR SETTLEMENT. THE TOP 4" OF COVER SHALL BE TOPSOIL. SEED SURFACE
AFTER COMPLETING INSTALLATION AS PER MUNICIPAL SPECS.
17 CONTRACTOR TO BE RESPONSIBLE FOR THE REMOVAL AND DISPOSAL OF CONTAMINATED MATERIAL FROM THE
EXISTING TRENCHS, IF ENCOUNTERED.
