The proposed LeBarge Subdivision dedicates 33' of land to supplement the Jerome
Street Right of Way. The new western lot line is only 1.3' from the northerly absorption
trench on the property and 5' from the southerly trench. A waiver is therefore required
to allow the absorption trenches to remain in their current location.
The edge of the trenches border on 66' of the Jerome Right of Way. The soil materials
underlying the trenches is clean sand with some silt (SW/SM) with excellent absorption
properties. We anticipate the majority of the absorption will be downward through the
bottom of the trench with little or no migration. There are no conflicts with wells or
septic systems in the area. We therefore request a waiver be issued to allow the
absorption trenches to remain in their current location.

I am writing to request a separation distance waiver between the existing Private well and an existing
concrete septic tank that is being reused for an upgraded system for the above referenced property
from 100 feet to88.3 feet.
PES was able to locate a well log for this property. The static water level of the well was reported as 114
feet below ground level with a total depth of 163 feet. The ground water in this area typically flows from
the east to the west. The well is located 88.3 feet southwest of the septic tank and has been that way
for over 30 years a recent water test showed no bacteria and a nitrate concentration of 1.6 mg/I .
As outlined under 18 AAC 80.020 and 18 AAC 72.021(a), I have calculated the following points:

was assumed for a water table gradient which is unlikely case, it is in my opinion, this waiver request
meets the above criteria for approval and does not constitute risk to health. I hope the above

Septic System Upgrade Permit Request
Ladies and Gentlemen:
I am writing to request a permit to install an upgrade septic system be issued for this property. The
proposed systems will serve an existing five (5) bedroom house and a one bedroom ADU. The existing
system is undocumented and does not appear to meet code requirements. The proposed system will
utilize a 2000g septic tank and conventional drain field. The surrounding developed lots are served by
private wells. There are no wells on surrounding lots within 200 feet of this system.
Soils. One test hole was performed by Pannone Engineering Services in December of 2013, and
groundwater was monitored for seven days. The test hole monitor tube was dry after the seven day
monitoring period. Bedrock was not encountered in the test hole. It is my opinion, based on the results of
the percolation tests and overall soils appearance; an application rate of 1.2 gallons/day/square feet
should be used for a conventional wastewater system.
1. Soil Absorption System Design.
a. See Sheet 2 of the design package.
2. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain
field. The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage
ditches.
3. Topography: The existing topography slopes from the southeast to the northwest at
approximately 5% in the area of the proposed drain field. There are no steep slopes in the vicinity of the
drain field.
4. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification
that the review is complete and that there are no further comments is received from MoA On -Site

based on the information obtained from the Municipality or Anchorage files and from my investigation and inspection, the on-site water supply

the time of installation.
In conducting an adequacy test, ~ attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA COSA
guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test,
and separation distances measured to readily identifiable features. The operational life ef all wells and septic systems depend on the local eon
condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These cond'itions
are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future
performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provFde any warranty
for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed
