1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska Wastewater
Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80).
3. The wastewater code requires inspections during the installation. The engineer must notify the Development Services
Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
Special Provisions: PRIOR TO CONSTRUCTION: A test hole shall be completed to verify the
reserve drainfield design prior to construction. If the results require design revisions, an approved
change order shall be issued prior to construction. The test hole results shall be submitted with
the final inspection report.

If the owner elects to do the septic system installation himself, he shall first submit a signed letter
to On-site stating:
1.His experience installing septic systems,
2.Who owns the equipment used for the installation,
3.That no one will be paid to help with the installation, and
4.That the owner will be present during the installation.

Subject: New Sewer Permit - T15N R1W SEC8 LOT 215
The owner has requested we proceed forward to obtain a septic permit to serve
the proposed 3-bedroom house.
The general slope of this lot is from southeast to northwest at a grade of
approximately 15-20% over the septic area. On June 26, 2014 a testhole was
performed to investigate soils and groundwater. The results of this test are
attached for your review and show adequate soils to support a primary
convenfional field or primary and reserve fields with an advanced wastewater
system. Prior to construction, an additional testhole will be performed for a
conventional or advanced reserve. We propose to install two 5-Wide 66' long
trenches. Groundwater was not encountered at excavation or at monitoring.
The property and adjacent lots are served by private water. There is no surface
water within 100' of the proposed system and there are no known curtain drains
within 50'. We do not expect there to be any adverse effect on adjacent lots by the

Septic System Owner -installer Agreement
A real estate property owner (Owner) may install their own septic system.
The department may issue an approval for a homeowner to perform work on a conventional
(not AWWTS) on-site wastewater disposal system to serve that individual's owner -occupied,
single-family or duplex home if the homeowner satisfies and agrees to the following
requirements.
The property owner and excavation equipment operator may perform work on no more than
one owner installed pr%ect in a 12 -month period.
