1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska Wastewater
Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80).
3. The wastewater code requires inspections during the installation. The engineer must notify the Development Services
Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
Special Provisions: PRIOR TO CONSTRUCTION: The primary drainfield shall have design
verification test holes (including USC classification, 7 -day groundwater monitoring and percolation
testing) in accordance with the code completed prior to construction. If the test hole results do
not support the permitted design, an approved change order shall be obtained prior to
construction. Test hole results shall be submitted with the final inspection report.

Septic System Permit Request
Ladies and Gentlemen:
I am writing to request a permit to install a new septic system be issued for this property. The proposed
systems will serve a proposed six (6) bedroom house. Currently the has a well installed, but is otherwise
undeveloped. The proposed system will utilize a 20008 septic tank with a conventional drain field. The
surrounding developed lots are served by private wells. There are no wells on surrounding lots within 100
feet of this system and there are no septic systems installed on the surrounding lots within 100 feet of the
proposed well.
Soils. Two test holes were performed on this lot by Michael E. Anderson in May of 2013, and groundwater
was monitored for seven days. Ground water was not observed below the surface in the test hole monitor
tube in May of 2013. Bedrock was not encountered in the test hole. It is my opinion, based on the results
of the percolation tests and overall soils appearance; an application rate of 0.6 gallons/day/square feet
should be used for a conventional wastewater system in the area of these test holes. Due to the
proposed location of the house and the existing test hole's location in the central portion of the lot I
request that the permit for this system be issued with the condition that a permit be issued with the
condition that a test hole and ground water monitoring be performed in the location of the proposed
drain field before construction is begun. The septic system is designed in the location of the septic
reserve area and is based on a conservative application rate of 0.45 GPD/SF. The surrounding test holes in
the area all show similar soil to the holes on this lot and the percolation rate is less than 30 minutes per
inch in all nearby test holes. Additionally if adverse conditions are found in the proposed test holes the
reserve drain field shown in the vicinity of TH-3 can be constructed based on CAT III.

field. The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage
ditches.
3. Topography: The existing topography is relatively flat in the area of the proposed drain field and
slopes from the east to west at approximately 20%-25% in the western portion of the lot. There are no
steep slopes within 50' of the proposed drain field. The proposed drain field will maintain 50' separation
all steep slopes.
4. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification
that the review is complete and that there are no further comments is received from MoA On -Site
Department, the note will be removed and "Issued for Construction" drawings will be issued.
The proposed installation will not affect the future development of the surrounding or existing lots. There
are no surrounding wells within 100 feet of the proposed septic location.

Dear Jay:
Raptors Knoll Subdivision is composed of 6 Lots and is located off of East 142nd Avenue
near Goldenview Drive in Anchorage. It is surrounded bY developed lots served by
private wells and on site septic systems. The lots proposed for the subdivision are in
excess of 40,000 square feet and are proposed to have on site wells and septic
systems.
Three wells are proposed for placement in the subdivision on proposed Lots 2, 5 and 6.
The wells will be placed at sites which will represent standard locations relative to septic
system sites and home sites on typical lots in the subdivision. Each well will be pumped
in accordance with AMC 15.55.060F1 to establish the sustained yield as determined by
the well driller.. Water quality testing from each well will be accomplished in accordance
with AMC 15.55.060F2. A formal report including sustained yield results for each well
will be submitted once all data is gathered and analyzed.
A permit application is attached for placement of the well on Lot 2 in the subdivision.
The location of the well and the proposed reserve area for the septic system on the lot
are shown on the attached Site Plan and will not impact adjacent properties. If the well
and proposed septic system is constructed in accordance with our design the following
statements apply:
The well, if constructed as designed, will have no adverse impact on the wells in
the area or those to be constructed in the future.
The well, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.

space, either surface or subsurface, on any lots located in the area.

5. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures outlined
in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or wastewater
disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated herein. I further verify that
based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply .
and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes, ordinances, and regulations in effect at
the time of installation.
In conducting an adequacy test, 1 attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA COSA
guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test,
and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil
condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions
are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future
performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provide any warranty
for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed
above.
