All construction must be in accordance with: 1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska Wastewater
Disposal Regulations (18AAC72) and Ddnking Water Regulations (18AAC80).
3. The wastewater code requires inspections during the installation. The engineer must notify the Development Services
Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October '15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
Special Provisions:
The entire subsurface disposal field and/or alternate is not within a 30 foot radius of a percolation
test. The Engineer needs to do an additional percolation test prior to the construction of the septic
field. Please submit stamped and signed results with the As-built Inspection Report. If the results
require a design change, construction of the system will stop pending Qn-Site review and

Well and Septic System Permit Request
Design Narrative
This is a design narrative for a permit to install a new well and septic system to be issued for this property. The
proposed Systems will serve a proposed six (6) bedroom house. Currently the lot is undeveloped. The proposed
system will utilize a 2000g septic tank with a conventional drain field. The surrounding developed lots are served by
private wells. There are no wells on surrounding lots within 100 feet of this system and there are no septic systems
installed on the surrounding lots within 100 feet of the proposed well.
Soils. One test hole was performed in the vicinity of this system by Pannone Engineering Services in May of 2015,
and groundwater was monitored for at least seven days. Ground water was not observed to a depth of 14.0'
below the surface in the test hole monitor tube in May of 2015. Bedrock was not encountered in the test hole. It
is my opinion, based on the results of the percolation tests and overall soils appearance; an application rate of
1.2 gallons/day/square feet should be used for a conventional wastewater system in the a rea of these test holes.
2. Soil Absorption System Design.
a. See Sheet 1 of the design package.
3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain field. The
proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage ditches.
4. Topography: The existing topography slopes from the east to the west at approximately 20% in the area
of the septic system. There are no steep slopes within 50' of the proposed primary drain field. The proposed
primary drain field will maintain 50' separation all steep slopes. The proposed reserve drain field will be located
approximately 38' east of the road cut slope. A waiver for the reserve field will be applied for when lhe system is
upgreaded.
5. Drawing Markings:The Drawings are marked "For MoA Review Only". When writen notification that the review
is complete and that there are no further comments is received from MoA OmSite Department, the note will be
removed and "Issued for Construction" drawings will be issued.
The proposed installation will not affect the future development of the surrounding or existing lots. There are no
surrounding wells within 100 feet of the proposed septic location or septics within 100 feet of the proposed well
location.

Reserve Absorption Field to Slope
Ladies and Gentlemen:
1. Slope Waiver: I request a waiver to the code requiring a minimum distance to a slope
over 25% of fifty feet (50') to a distance of thirty feet (30'). Due to topographical constraints on
the lot, it is not possible to achieve the required separation distance. As shown in the attached
section any effluent moving at a 4:1 slope beginning 2.5' above the pipe invert of the reserve
field at the lower side of the field will not daylight within 38.5' of the system. This is extremely
unlikely though du~ to the well percolating nature of the soil. The primary system will be
located 44.6' from the slope, and an infiltration line at 4:1 will not daylight within 50' of the field.
The proposed installation will not affect the future development of the surrounding or existing
