All construction must be in accordance with: 1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska Wastewater
Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80).
3. The wastewater code requires inspections during the installation. The engineer must notify the Development Services
Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.

The entire subsurface disposal field and/or alternate is not within a 30 foot radius of a percolation
test. The Engineer needs to do an additional percolation test prior to the construction of the septic
field. Please submit stamped and signed results with the As-built Inspection Report. If the results
require a design change, construction of the system will stop pending On-Site review and
approval. AMC 15.65.060.B.3

Dear Mr. Crewdson:
We respectfully request your review and approval to construct the subject well and septic system.
Attached to the application sheet, including this letter and sizing and specifications requirements
are the following documents.
1. Well Locations and Septic System Layout stamped and signed by a licensed professional
engineer.
2. Design and calculations stamped and signed by a licensed professional engineer, shown
in Figure 6.
3. Results of soils, percolation, and water table test stamped and signed by a licensed
professional engineer.
This septic system design is for a five-bedroom home to be served by a private well. We propose
installation of a 1500-gallon septic tank, a bed absorption system and new well. The attached
Well Locations and Septic System Layout are provided in Figure 1 and shows existing adjacent
and surrounding property well and septic system approximate locations and the proposed well
for this lot as well as the location of soil and percolation test pit used for this design application.
No other wells and surface water exist within 100 feet of our proposed septic system design.
The included percolation and soils identification logs reflecting exploration to a depth of 16.0
feet and 16.5 feet was performed and stamped by a registered professional engineer in the State
of Alaska. The test pit locations are shown in the attached figures. The test indicates well graded
gravel. The soil percolation rate was faster than one minute per inch. No water or bedrock was
encountered and no water was recorded in a four-month span of monitorh~g. We propose to
complete a test pit and percolation test at the proposed location of the absorption bed prior to
installation which will be submitted to you for approval and confirmation that the proposed
system is properly sized and designed.

Septic System Sizing
The system is for a new construction of a single-family five-bedroom home. Developed
surrom~ding lots are served by on-site septic systems and private wells. Soil percolation rate
faster than one minute per inch requires a two-foot thickness of filter material underlying sewer
rock gravel and 150 square feet of septic area per bedroom. The septic system design area
requirement for five bedrooms is 750 square feet; at 15 feet wide, the required bed length is 72
lineal feet.
The components and materials and the installation shall meet MOA's Handout for Typical Bed
Requirements and Specifications per AMC 15.65, attached.
The topography is generally sloping variably from north to south at approximately 15%. Surfac~e~
waters are more than 100 feet from the proposed and alternate septic field locations. This
proposed installation and well location create no adverse effects or ,limitations on existing or
future adjacent wells or septic systems under current MOA Code.

The purpose of this letter is to request a waiver on behalf of Mike Quinn for allowing the bed to be on a
slope greater than 10%. The bed will meet separation from impermeable layers and will not be close to
groundwater, as seen by the depths of the test holes on the drawings. Excavation will start on the uphill
side of the slope to prevent any problems associated will the depth of the bed on the down/0/ll slope.

WATER WELL MAINTENANCE INGRESS AND EGRESS EASMENT
This Water Well Maintenan~ Ingress and Egress Easement (this
"Agreement") is made as of this ~ of -'~',-~ , 2015 by Carmel J. Mclntyre,
whose mailing address is 25250 Mclntyre Rd. Eagle River AK., 99577, as owner of Lot
B-l, Mclntyre Subdivision, according to the officially recorded plat in the Anchorage
Recording District, filed under Plat No. 2015- ~,? , ("GRANTOR") and as owner of
Lot B-2, Mclntyre Subdivision, according to the officially recorded plat in the Anchorage
Recording District, filed under Plat No. 2015- ~,? , ("GRANTEE").
RECITALS:
A. Grantor is the owner of certain real property legally described on
Exhibit A attached hereto and incorporated herein by reference (the "GRANTOR
Property").
B. Grantor is required by Municipality of Anchorage platting
requirements to create said easement on Tract B-1 for the benefit of Tract B-2 to access
and maintain the existing water well.
NOW, THEREFORE, FOR GOOD AND VALUABLE CONSIDERATION, The
receipt and sufficiency of which are hereby acknowledged, the parties to this Agreement
hereby agree as follows:
1. Common Access Easement. Grantor grants to Grantee, its
successors and assigns, for the use and benefit of Grantee, a perpetual non-exclusive
easement upon, over, above that certain area of the Grantor Property labeled as "10'
Water Well Maintenance Ingress and Egress Easement" as shown on Exhibit A and
incorporated herein by reference, for ingress and egress.
2. Indemnification. Grantee hereby defends, indemnifies and holds
Grantor harmless from and against all claims, demands, losses, liabilities, damages or
expenses arising out of or in any way related to the exemise of the rights under this
Agreement.
3. Use of Easement Area by Grantor and Grantee. Grantee shall be
solely responsible for the maintenance and upkeep of the water well maintenance
ingress and egress easement area. Grantor shall not use the area in any manner
inconsistent with the rights granted to grantee hereunder.
4. Running with the Land/Binding Effect. The agreements
contained herein and the easement and other rights granted hereby shall run with the
titles to the Grantor Property and shall bind and inure to the benefit of the parties hereto
and their respective heirs, successors and assigns.

THIS AGREEMENT, made this 26th day of June, 2015, by and between Carmel J. McIntyre
and their heirs, administrators, and assigns, hereinafter called "PERMITTEE", and the Municipality of
Anchorage, a municipal corporation organized and existing under its Charter and the laws of the State
of Alaska, hereinafter called the "PERMITTER".
WITNESSETH:
WHEREAS, PERMITTEE is the owner of the following described real property:
MCINTYRE, TRACT B, according to the official records thereof, on file in the office
of the District Recorder, Anchorage Recording District, Alaska, and;
WHEREAS, PERMITTER owns and/or maintains the real property more particularly
described as follows:
The 30 FOOT UTILITY EASEMENT, located immediately in the center of the
PERMITTEES property as shown on Plat No. 92-134, on file in the office of the
District Recorder, Anchorage Recording District, Alaska, and;
WHEREAS, PERMITTEE has placed a well upon the referenced real property which
encroaches upon the PERMITTER'S 30 foot Utility Easement.
NOW, THEREFORE, it is mutually agreed between the parties hereto that:
The PERMITTER, acting through the Director of Community Development
Department, hereby grants to the PERMITTEE the privilege of allowing a well to
encroach upon the PERMITTER'S 30 foot Utility Easement, as shown on
"Attachment A," included herewith.
The PERMITTEE agrees forever to indemnify, defend, save and hold harmless,
the Municipality, its officers and employees, from any and all lawsuits, claims or
actions brought to any person for or on account of damage to property or injury,
disease, illness or death of persons, including all costs and expenses incident
thereto, arising wholly or in part from or in connection with the existence of,
alterations, maintenance, repair, renewal, reconstruction, operation, use or
removal of the encroaching well, as placed upon the PERMITTER'S 30 foot
Utility Easement.
3. The PERMITTEE shall not assign or transfer any of the rights granted herein to
another individual or company without first notifying and securing the approval
of the Director of Community Development Department.
4. This Agreement and Permit grants PERMITTEE no interest in PERMITTER'S
real property whatsoever, except only the encroachment rights described herein.
5. The PERMITTER reserves the right to revoke this permit upon twenty (20)
days written notice to the PERMITTEE. The PERMITTEE agrees upon such
notice of revocation, to move said encroachment(s) from the Utility Easement
in which it is placed. Should the PERMITTEE refuse or fail to comply with
said written notice, the PERMITTER may, without further notice to the
PERMITTEE, remove or cause to be removed the encroachment(s), and the
PERMITTEE hereby agrees to reimburse the PERMITTER for all costs
incidental to the removal thereof.
In addition to the mutual promises heretofore made, the PERMITTEE has paid the
PERMITTER a one-time permit application fee of $115.00. The PERMITTER hereby waives the
annual fee of $300.00.
IN WITNESS WHEREOF, the parties hereto have hereunto set their hands and seal the day
