The owner has requested we proceed forward to obtain an extension of time for
the well and septic permit OSP161088 to serve the proposed 4 -bedroom house.
There have been no significant changes to the property or soil conditions
observed since the original permit was issued and the owner is now able to move
forward with the installation of the systems per the approved design.
The property and adjacent lots are served by private water. There is no surface
water within 100' of the proposed system and there are no known curtain drains
within 50'. We do not expect there to be any adverse effect on adjacent lots by the

The owner has requested we proceed forward to obtain a well and septic permit
to serve the proposed 4-bedroom house.
The general slope of this lot is from east to west at a grade of approximately 0-
5%. On June 6, 2014 a testhole was performed to investigate soils and
groundwater. The results of this test are attached for your review. We propose
to install one 42' long deep trench. Groundwater was not encountered at
excavation or at monitoring.
The property and adjacent lots are served by private water. There is no surface
water within 100' of the proposed system and there are no known curtain drains
within 50'. We do not expect there to be any adverse effect on adjacent lots by the
