We are therefore requesting a permit be issued for construction of a new well and septic system to serve the new home. The size of the septic system and the proximity of bedrock to the ground surface in the subdivision limit the available area for the sewage disposal system and make it difficult to place a conventional septic system. We therefore intend to construct an Advantex Treatment System on the lot in conformance with subdivision plat notes to provide the most effective treatment of the septic effluent prior to discharge into the surrounding soil strata. The location of the absorption system is shown on the attached Site Plan. Drainage patterns are also shown on the plan and will not be modified as a result of the proposed construction. The test hole placed on this lot revealed silty sand with gravel soils (SM/SW) underlain by bedrock. The percolation rate in the materials encountered in the area near the new system was 4.1 minutes per inch. No groundwater was found but bedrock was encountered at 3.5'. Our design provides for the bottom of the absorption trench to be 6' above the bedrock layer. We are therefore proposing to place a four-bedroom Advantex Treatment System. The absorption trench will be 30' long x 5' wide with .5' effective depth. The distribution line in the trench will be placed at 3' above the existing ground level. The total depth of the trench will be 1' to remove the organic material. The organic material will be replaced with coarse sand which will also be used to raise the trenches 2.5' above existing ground. A minimum of 2' of fill will be placed above the trench along with 2" of insulation. A minimum of 4' of fill will be placed over the septic tank to meet Municipal Code and prevent freezing. The ground surface of the lot slopes at 25% to 40% to the west, but is less steep near Lot 3, Block 1, Southpointe Page Two May 19, 2016 the location of the primary absorption trench site. Slopes become extreme further to the west. The trench will be constructed parallel with the contours of the slope as much as possible in conformance with Municipal requirements. All components of the new septic system will be constructed a minimum of 100' from any surface water and 100' from the well proposed for the lot. No conflicts exist between the proposed well and septic system and those in the area. If the system is constructed in accordance with our design the following statements apply: 1. The system, if constructed as designed, will have no adverse impact on the wells in the area or those to be constructed in the future. 2. The system, if constructed as designed, will have no adverse impact on existing septic systems in the area or those to be constructed in the future. 3. The system, if constructed as designed, will have no adverse impact on reserve space, either surface or subsurface, on any lots located in the area. 4. The developer of the lot has located the house on the flattest area of the lot further reducing the area available. We have located an area with acceptable soils, but it is situated slightly more than 12' from a break in grade greater than 25%. The alternate site is 17' from the break. We are therefore applying for a waiver allowing placement of the absorption trench at this location. The soil encountered in the test hole is well graded sand with silt (SM/SW) which is very good for the absorption of septic effluent. In addition, an Advantex Nitrate Reducing Treatment System is installed on the lot. No migration of effluent is expected to occur to the steep slope. Fill will be provided fronting the absorption trench. In addition an impermeable membrane will be placed alongside the western edge of the raised absorption trench. The 25% slope when extended downward from 2' above the bottom of the membrane does not daylight for 35'. This is a variance to the engineer's bulletin for excessive slopes, but it has been approved recently. No septic effluent can escape the area of the absorption trench without travelling at least 35' to daylight. A condition which is rarely or never seen. The slope fronting the absorption trench is nearly 2.5:1 at 45' from the edge of the trench. It is difficult to construct a 2:1 fill slope at this location which will meet existing ground without substantial fill. We are therefore proposing to place a 1.5:1 slope at this point to the existing ground catch point. The face of the slope will be covered with jute mesh to stabilize the slope until revegetation has been completed. The slope will be Lot 3, Block 1, Southpointe Subdivision June 27, 2016 Page Two constructed primarily of fractured bedrock which has an angle of repose of 1:1. The 3:1 side slope will extend an incalculable distance down the steep slope to an eventual catch point resulting in an extreme amount of fill. We therefore request a waiver allowing the fill slopes off the raised trench to be 2:1. The slopes will be covered with topsoil and seeded with grass. Any man-made or natural change in a system that would inhibit the system from performing as designed. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AWWTS that were included in the original design which would allow the AWWTS to continue to perform as designed. Permit. An On -Site Wastewater Disposal Permit as required by AMC 15.65 to construct and operate an AWWTS. 3. Term. The term of this Maintenance and Repair Agreement shall begin on the date of approval by the Municipality to operate the installed system or issuance of a COSA, and shall continue while the AWWTS is in use or is operational or until the property is sold or title is transferred by the owner and a new COSA is issued to the new owner or transferee of the property. 4. Alterations, Installation and Removal of Additional Equipment. Prior to performing any alterations to an AWWTS, the owner agrees to obtain an On-site Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65. 5. Maintenance and Repairs. A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall maintain their AWWTS in a satisfactory condition capable of producing treated septic effluent in accordance with the equipment's approval for operation in the Municipality. The owner shall enter into a service aereement with an AWWTS service and maintenance provider approved by the municipality and the manufacturer of the AWWTS for the entire term of the AWWTS. In addition, it shall be the responsibility of the Owner during the term of this Maintenance and Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs. B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules and orders for the AWWTS. C. Upon request by the Municipality, the owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system. When a record of maintenance is documented and maintained by the system vendor, the owner agrees to allow the Municipality access to this information. D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be assessed in accordance with AMC 14.60 for improper discharge. E. Maintenance, Repairs and Alterations. (Owner is required to read, understand and initial each section) 4A4� hroughout the term of this Agreement, the Owner shall enter into a service agreement with an AWWTS service and maintenance provider approved by the Municipality or the manufacturer's representative. The AWWTS shall be maintained in a satisfactory condition capable of performing as designed and producing treated septic effluent in accordance with the equipment's approval for operation in the Municipality. It shall be the responsibility of the Owner during the term of this Agreement to pay for all repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This includes an annual maintenance fee (typically $400 to $600). Owner agrees that only maintenance and repair personnel approved by the Municipality or the manufacturer's representative will inspect and make any necessary maintenance, repairs or permitted alterations to the system. Owner acknowledges that regular maintenance of an AWWTS reduces the potential failure of the system, which could include sewage backup and costly repairs or drainfield replacement. This inspection revealed a nitrate concentration of 7.6 milligrams per liter (mg/L) was reported for the property's well water sample. The Environmental Protection Agency (EPA) has established a maximum contaminant level (MCL) of 10.0 mg/L for public drinking water systems. While private wells are not subject to this regulation, EPA standards are based on existing health information and can therefore be used to gauge the relative quality of water from private wells. Please see the attached "Nitrate Fact Sheet" for important information regarding nitrate. This advisory must be attached to all copies of the subject Certificate of On - Site Systems Approval. Nitrate Fact Sheet From Northern Testing Laboratories, Inc. Nitrate is a negatively charged compound of nitrogen and oxygen, which is very soluble in water. Nitrate is not readily filtered or otherwise removed in the soil and can pass rapidly into ground water wells. SOURCE: Nitrate is a major component of fertilizer and wastewater. Often the nitrate is in the form of ammonia or protein first, which through contact with oxygen and certain bacteria, converts to the oxidized form known as nitrate. Sources of nitrate from wastewater include urea, ammonia cleaners, food solids, and bacterial cells. It may also result from the breakdown of organic matter buried in the soil. TOXICITY: Nitrate is generally not toxic to adults or children over the age of two or three years, but is associated with a potentially fatal infant disease called methemoglobinemia. In the digestive system of young children, nitrate converts to nitrite, which can pass through the intestinal wall into the blood stream. There it combines with the hemoglobin and interferes with the ability of the blood to carry oxygen. For this reason, methemoglobinemia is referred to as "blue baby" disease. The EPA limits the concentration of nitrate in public drinking water supplies to 10 mg/L. The standard has been lowered from a previous level of 45 mg/L set by the US Public Health Service and the World Health Organization. TREATMENT: due to its solubility in water and negative ionic charge, filtration and other common home water treatment systems such as softening or iron filtration does not readily remove nitrate. The best method for limiting nitrate in well water is source control. This can include avoiding overdosing of fertilizer near the well and maintaining good separation distances between septic tank leach fields and the well. A special anion exchange filter that contains a media with a strong affinity for negatively charged ions in water, or by a reverse osmosis treatment system or distillation can remove nitrate. TESTING: Nitrate analysis is usually done by one of the several "wet chemical' methods using a spectrophotometer to read the final color endpoint. Specific ion electrodes also can be used to detect the activity of nitrate in water. This laboratory uses several different wet chemical methods approved under the public water supply laboratory certification program. They also have test kits available, which the laboratory uses to perform an inexpensive "screening test', and with which the homeowner can monitor the change in nitrate levels from their well. They recommend comparing the test kit results against a certified analysis from the lab occasionally to verify the accuracy of the kit.
