As -built survey originally submitted was dated 7/25/19, but was not printed to scale. The survey that was submitted
with the resubmittal was dated 5/31/19 and is missing standpipes. Please provide a to -scale as -built survey with all
standpipes identified to match the record drawing. %Z .$ D L vim_ C)
2. According to the photos provided, the septic tank is not adequately protected from traffic. Either additional barriers
need to be provided or calculations submitted to demonstrate the structural integrity of the tank, accounting for the
potential that a vehicle could be parked over it.
3. Please address the structural stability and protection of cleanouts/monitoring tubes for the drainfield located in the
driveway.

To whom it may concern:
During the installation of the drainfield, the lot was vacant. When our firm was contacted
and informed that the home had been completed, we made another site visit to confirm
separation distances and perform a well adequacy test. During our site visit it had been
discovered that a second driveway had been excavated below the drainfield, creating a
cutbank, and additional fill (approximately 10 feet) had been placed over the drainfield.
This new driveway and imported fill, has created slopes greater than 25% at the
drainfield location. We are now requesting a zero (0) foot slope waiver because of this
development. As can be seen on the attached drawing, at a 25% slope (starting 2.5 feet
above the drainfield) the potential point of daylighting is greater than 50 feet horizontally
from the drainfield.
The orientation of the home and driveway was altered during construction which led to
the tank and drainfield being located underneath the driveway. We are requesting a
waiver for these variances and justifications are as follows; � c e
• The contractor has placed concrete blocks in front of the septic tank to help
prevent any vehicles from driving or parking over the tank.
• It is likely that because of the placement of the concrete blocks, snow removal
over the tank is unlikely which will help prevent any freezing. It should also be
noted that the tank has 6.5 feet of cover.
The drainfield has approximately 14 feet of cover and is unlikely to experience
aes.

To whom it may concern:
The subject lot septic system was designed and installed in 2016. The septic system
was inspected by Garness Engineering and installed by Wilco Excavation in July 2016.
The septic tank was later moved and inspected by GEG. Our field notes for the tank
relocation have been misplaced, so the date of this inspection is unknown. During the
installation, there were no permanent structures to serve as a "Benchmark", so a "Nail in
Tree" was used. During the construction of the home, the benchmark (three with nail)
was removed by the contractor; therefore, the elevation control point for the record
drawings/final grading is no longer present. A benchmark elevation of 100.00 was
establish as the bottom of trim at the house corner and all elevation were back
calculated. It should be noted that GEG still has our field notes for the drainfield
installation and the original tank installation; therefore, we know the system was

To whom it may concern:
The subject lot is vacant. The property owner is proposing to build a four (4) bedroom
home that will be served by a private well and septic system. We are proposing to install
a new 1250 gallon septic tank and deep trench type drainfield. Two testholes were
excavated on the property and the drainfield is designed around the 30 foot radius of
testhole #2. Comments regarding the design are summarized as follows:
1. SOILS: See the attached 2016 logs which show the soil classifications, groundwater
monitoring, and the percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface water concerns.
4. TOPOGRAPHY: See the attached 40 scale design drawing which shows
topography. The average topography around the proposed drainfield is 17%. There is a
cutbank towards the west property line, however, the proposed drainfield is >50 feet
away from the cutbank.
We are unaware of any adverse impacts this installation would have on adjacent wells
