All construction must be in accordance with: 1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska Wastewater
Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80).
3. The wastewater code requires inspections during the installation. The engineer must notify the Development Services
Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
Special Provisions:
1. For the Inspection Report approval, the documentation of the existing field and tank are to have
MOA approval.
2. The proposed trench is too deep for the soils information submitted. An additional test hole is
to be done prior to construction. Construction may proceed at your own risk before the 7 day
water monitoring is complete. Submit results with the Inspection Report.

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The absorption system on the subject lot is undocumented and undersized and must be
upgraded before a COSA can be issued for resale of the property. We are therefore
requesting a permit be issued for the upgrade of the septic system to accommodate the
three-bedroom home on the lot. The attached Site Plan and backup documentation
identify the location and configuration of the existing and proposed septic systems. The
existing septic tank will be exposed and inspected and verified suitable for continued
use. It will be replaced if found defective. A portion of the existing absorption trench
will be decommissioned in accordance with Municipal Code. Also identified on the
plans are the locations of the existing well on this lot and adjacent lots. A waiver to
allow the septic tank and absorption trench to be constructed in the planned location
has been obtained from ADEC and is attached. No conflicts exist between the
proposed septic system and the wells on adjacent lots. Drainage arrows are shown
indicating the current drainage patterns. The ground surface in the area of the
proposed trench is virtually fiat. The drainage patterns will be maintained after
construction.
The test hole placed on the lot indicated Silty Sand with Gravel (SM) in the absorption
zone of the trench. The percolation rate was determined to be between 1 and 5
minutes per inch. No groundwater was encountered during the placement of the test
hole and none has developed to date. We have designed the new trench with an
application rate of 1.2 gallons per day per square foot. We are proposing to verify the
existing 1,000 gallon septic tank and construct an extension to the existing trench 15'

long trench by 3' wide with a 6.5' effective depth. The total depth of the system will be
10.5' with the distribution line at 3.5' below the existing surface. The existing absorption
trench will be decommissioned 10' from the south property line and the new extension
will be constructed parallel to the line.
The ground surface on the lot in the area of the new trench is flat. The trench will be
placed parallel to the contours of the ground as much as possible and in conformance
with design criteria.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

Deer Mr. Anderson,
On July 15, 2016 a request for a Class C public water system locaWxl at 2640 DeArmoun Road was
submitted for review for waivrs to the wastewater systems that eneroaeh near the source well. You are
recoramending these waivers and feel that public health will not be impacted from the existing site
conditions.
A review of the resent samples and the well log and it's soils information along with the placement of the
house between the source well and the wastewater systems of concerns have been evaluated. The
following waivers based upon the engineer's recommendation are herein listed.
Thc separation distance to the existing septic tank on thc same lot as thc source well is established as
85 feet reduced from thc 150 foot requirement. Caution the septic tank is older and may have
reached it's effective live if this unit is in need of replacement it is advised to provide more
separation distance than has been provided.
The separation distance to the existing soil absorption field on the same lot as the source well is
established as 98 feet reduced fi'ora the 150 foot requirement. Caution the older field should it
require replacement would again need to be reviewed to assure the concerns related to public health
are still in place.
The separation'distances to the wastewater system serving the next door property Doan S/D, Lot 1
are an existing 118 feet and ere also reduced from the 150 foot requirement, Should they be fo~md to
be in need of replacement or upgrading a farther separation distance needs to be a part of any new
development or connection to the Municipal utilities.
These waivers are based upon thc engineer's recommendation and the engineering controls established
upon the lot which assured the protection of public health. Should it be found that the site conditions
endanger public health or are a public health concern, the waiver will be rescinded.
This approval does not imply the granting of additional authorizations nor obligate any state, federal or
local regulatory body to grant required authorizations.
Any person who disagrees with this decision may request an adjudicatory hearing in accordance with 18
AAC 15.195- 18 AAC 15.340 or an infmmaal review by the Division Director in accordance with 18 AAC
15.185. Informal review requests must be delivered to the Division Director, 410 Willoughby Avenue,
Suite 303, P.O. Box 111 g00, Juneau, Alaska 99801, within 15 days of receiving the decision. Guidance
information on the informal review process may be found at http://www.dee.state.ak.us/eommish/Review-
Guidanco.htm. Adjudicatory hearings requests must be delivered to the Commissioner of the
