2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska Wastewater
Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80).
3. The wastewater code requires inspections during the installation. The engineer must notify the Development Services
Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October 15 to Apd115, a subsurface soil absorption system under construction during.freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
Special Provisions:
1. The driveway must be creating a barrier to overland flow of effluent and the swale between the
house and field must be in place prior to Inspection Report approval.
2. The Engineer needs to do 2 test holes prior to the construction of the septic field to satisfy ERS
requirements. Please submit stamped and signed results with the As-built Inspection Report. If
the results require a design change, construction of the system will stop pending On-Site review
and approval.
3. The finished grade around the septic system is to be graded so that water does not pond.

Well antiseptic System Permit Request
Design Narrative
This is a design narrative for a permit to !..r)stall a well and septic system to be issued for this property. The
proposed systems Will serve an prol~0sed three bedroom (3) bedroom house. Currently the lot is
undeveloped. The surrounding developed lots are served by private wells. There are no wetJs within 100' '
of the proposed septic system.
'Soils. One test hole was performed on this lot by PES in June of 2014, and groundwater was monitored
for at least seven days. Ground water was observed at a depth of 2.6' below the surface in the test
hole monitor tube in October of 2014. Bedrock was not encountered in the test hole. Based on the
results of the percolation tests and overall soils appearance; an application rate of 6
gallons/day/square feet was used for a CAT III wastewater system in the area of the test hole.
2. Soil Absorption System Design.
a. See S~heet 1 of the design package.
3. Surface Water: There is surface water in the form of a creek approximately 50 feet from the proposed
septic tank and 72 feet from the proposed drain field. See waiver request.
4. Topography: The existing topography generally slopes from the south to the north in the area
surrounding the septic System at approximately 8%. There are no steep slopes within 50' of the proposed
drain field. The proposed drain field will maintain 50' separation all steep slopes.
5. Drawing Markings:The Drawings are marked "For MoA Review Only". When written notification that
the review is complete and that there are no further comments is received from MoA On-Site
Department, the note will be removed and "issued for Construction" drawings will be issued.
The proposed installation will not affect the future development of th is o} the surrounding lots.

Surface Water Waiver and ERS Wgive'r Request
Surface Water Waiver Request. I am requesting a waiver for the separation distance from the septic
system to surface water of approximately 60'. The surface water is approximately 72.8'_+ from the fiekt
and 60.9'+ from the tank. Construction of an earthen berm system in the form of a raised pad for the
house and driveway will extend the overland sheet flow distance to 100'+ from tee proposed system
to the surface water.
2. Waiver Request for use of Engineered Receiving Soil (ERS). I am requesting a w~iver to the 1992 Code to
use the proposed Code Update Section of Engineered Receiving Soils (ERS). Below I will detail my request
justifying the use of ERS:
a. One test hole was excavated by PES in 2014. The soils log is at~ached. Ground wat~? Was monitored :for
through the fall seasonal h'igh a,~d stabilized at 2.6 feet below g¢ound level in October of 2014.
b. This design proposes to use MoA Type II. This interface will be approximately 2.5 feet in depth and will
be compacted to approximately 80 percent to allow free flow of the effluent through it into the native
soil using MoA Waste Water Code application rates. The infiltration area of the interface between the
ERS and the in-situ soil was based on the 30-60 minute per inch percolation rate of the in-situ receiving
soil. A two foot thick filter sand layer will be placed over the MoA Type II material to act as an interface
between the SAS and ERS.
c. The SAS is sized assuming a percolation rate of the ERS soil, assumed 16-30 MPI, which gives an
application rate of 4 GPDSF.
d. The system will protrude above the ground surface. The cover of the system will produce a mound,
which will have at a minimum, three to one side slopes and six inches of organically rich soil to promote
the growth of vegetation. The mound will be vegetated.
e. The in-situ receiving soils were attempted to be percolated, but due to heavy rains, we were unable to
perform an accurate percolation test.. PES did have a sieve analysis performed on the material bya
Certified Geotechnical Engineer, Crowther and Associates. The Sieve Analysis classified this materiat as a
Silty gravelly SAN D (or SM under the Unified Soils Classification system.) The proposed ERS COde limits
the use of ERS in soils classified as GP, GW, SP, or SV~L. The application rate for an SM soil per 18 AAC
72.035(6) Table B is 275 square feetper bedroom (0.55 gpd/sf- Do the math. See Table C 18AAC72) for
a conventional system (which equates to a percolation rate range of 16-30 mpi ur~der Title J.5.65.350).
PES proposes using a Category Ill advanced wastewater system on this property. Based on the assumed
percolation rate of 16-30 mpi, an application rate can be used up to 4 gpd/sf. PES is proposing using a
conservative rate of 2.0 gpd/sf for a CAT Ill system (percolation rate range of 30 to 60 m~i) and a wide

GRANT OF EASEMENT
This easement agreement is made between Claire Steffens, whose address P.O.Box
101847, Anchorage, Alaska (Grantor), owner of the land described as Lot lA, Block 6H,
Rabbit Creek View and Rabbit Creek Heights Subdivision, (herein Property) according to the
Plat No. 2004-91, records of the Anchorage recording District, Third Judicial District, State of
Alaska, for good and valuable consideration, the receipt whereof is hereby acknowledged,
and does hereby grant unto Claire Steffens, whose address P.O.Box 101847, Anchorage,
Alaska (Grantor), owner of the land described as Lot 2A, Block 6H, Rabbit Creek View and
Rabbit Creek Heights' Subdivision, according to the Plat No. 2004-91, records of the
Anchorage recording .District, Third Judicial District, State of Alaska, the following grant of
easement: described as:
, A perpetual, non-exclusive right of way, to provide access by a trail not to exceed ten
(10) feet in width for the construction, maintenance, repair, removal and inspection of a
water well serving exclusively Lot 2A, Block 6H, Rabbit Creek View and Rabbit Creek
Heights Subdivision, Anchorage: and
A perpetual right to construct, install, operate, maintain, repair, inspect, use and
remove a water well, including all facilities ( eg. casing, pumps, water line and electric line)
needed to operate said well, in, under, upon, about, over and through Lot lA, Block 6H,
Rabbit Creek View and' Rabbit Creek Heights Subdivision, Anchorage, Alaska,said well to
serve only Lot 2A, Block 6H, Rabbit Creek View and Rabbit Creek Subdivision.
Said easement more particularly described as:
An easement depicted on the attached survey prepared by Lee Karabelnikof, Reg.
Land Surveyor No. 3143-S, dated July 21, 2016, having the dimensions as indicated by the
line data thereon, an area of approximately 770 square feet.
1. The easement granted hereby shall not in any manner be dedicated for, or subject
to, use by the public. The use of this easement shall be exclusive to Lot 2A, Block 6H,
Rabbit Creek View and Rabbit Creek Heights Subdivision. Within this easement, rock, soil,
sand, gravel, and vegetation may be excavated and removed to construct, improve, maintain
and repair the trail area.
2. Grantee may maintain, at Grantee's sole expense, the well access trail descibed with
particularity above, for the purpose of effectuating the Grant. Grantor may not place
permanent structures over such trail or otherwise make access to the well impracticable.
The trail may not exceed 10 feet in width and may be permanently
surfaced only with vegetative material.
3. The Easement granted and created is intended to and shall run with the land, with
the benefit of the easement appurtenant to the respective dominant parcel and the burden of
the easement appurtenant to the servient parcel. This Easement shall be binding upon and
inure to the benefit of the parties successors and assigns, heirs, beneficiaries and personal
representatives and be perpetual.
4. This Easeme.~t shall be governed by, and construed and enforced in accordance
with the laws of the State of Alaska.

ADVANCED WASTEWATER TREATMENT SYSTEM
MAINTENANCE AND REPAIR AGREEMENT
BETWEEN MUNICIPALITY OF ANCHORAGE
THIS MAINTENANCE AND REPAIR AGREEMENT made and entered into as of this .

herein the "OWNER," and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration ofthe mutual covenants contained herein, the
parties to this Maintenance and Repair Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. Municipality grants permission to
Owner to utilize and operate an Advanced Wastewater Treatment System
(AWWTS), described as
located at (legal description).
2. Definitions.
Alteration. Any change to the design or function of an AWWTS that
includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design.
Certificate of On-Site Systems Approval (COSA). An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with Anchorage
Municipal Code(hereinafter, "AMC") 15.65. These approvals certify that
the systems are adequate for the homes that they support and meet the
codes that were in place at the time of system construction.
Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AWWTS that were included
in the original design which would allow the AWWTS to continue to
perform as designed.
Permit. An On-Site Wastewater Disposal Permit as required by AMC
15.65 to construct and operate an AWWTS.
3. Term. The term of this Maintenance and Repair Agreement shall begin on the
date of approval by the Municipality to operate the installed system or issuance ora
COSA, and shall continue ~vhile the AWWTS is in use or is operational or until the
property is sold or title is transferred by the owner and a new COSA is issued to the
new owner or transferee of the property.
4. Alterations, Installation and Removal of Additional Equipment. Prior to
performing any alterations to an AWWTS, the owner agrees to obtain an On-site
· Wastewater Disposal Permit fi:om the Municipality in accordance with AMC 15.65.
5. Maintenance and Repairs.
A. Throughout the term of this Maintenance and Repair Agreement, the OWner shall
maintain their AWWTS in a satisfaCtOry condition capable of producing treated
septic effluent in accordance with the equipment's approval for operation in the
Municipality. The owner shall enter into a service a~reement with an AW~VTS
service and maintenance provider approved by the muincipality and the
manufacturer of the AWWTS for the entire term of the AWWTS. In addition, it
shall be the responsibility of the Owner during the term of tins Maintenance and
Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay
for any and ali: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement
costs, and (5) inspection costs.
B. Owner agrees to comply with all applicable ordinance, laws, regulations, roles
and orders for the AWWTS.
C. Upon request by the Municipality, the owner agrees to provide the Municipality a
written schedule of routine maintenance and repairs which have been performed
on the system. When a record of maintenance is documented and maintained by
the system vendor, the owner agrees to allow the Municipality access to tins
information.
D. Owner acknowledges that the frae for failing to maintain and repair an AWWTS
may be assessed in accordance with AMC 14.60 for improper discharge.
E. Owner agrees that only maintenance and repair personnel approved by the
Municipality will inspect and make any necessary maintenance, repairs or
permitted alterations to the system.
F. Owner age_es to gran~ the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
G. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On-Site Systems Approval.
H. Owner agrees that the AWWTS installation and maintenance requirements as
provided by the AWWTS vendor/installer and approved by the Murficipality are
the governing guidelines for the construction, maintenance and repair of the
Owner's AWWTS.
I. Owner agrees to provide and maintain a telephone connection to the AWWTS as
required by the AWWTS approval.
6. Nonwalver. The failure of either party at any time to enfome a provision of th/s
Maintenance and Repair Agreement shall in no way constitute a waiver of the
provisions, nor in any way affect the validity of the Maintenance and Repair
Agreement or any part hereof, or the right of such party thereafter to ~nforce each and
every provision hereof.
7, Amendment.
A. This Maintenance and Repair Agreement shall only be amended, modified or
changed by a writing, executed by authorized representatives of the parties, with
the same formality that this Maintenance and Repair Agreement was executed
with, and such writing shall be attached to this Maintenance and Repair
Agreement as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this agreement, the only authorized representatives of the parties
are:

C. Any attempt to amend, modify, or change this contract by either an unauthorized
representative or unauthorized means shall be void.
8. Jurisdiction: Choice of Law. Any civil action arising fi:om this Maintenance
and Repair Agreement shall be brought in the Superior Court for the Third Judicial
District of the State of Alaska at Anch6mge. The laws of the State of Alaska shall
govern the rights and obligatinns of the parties under this Mainte~}ance and Repair
Agreement.
9. Severabilit¥. Any provisions of this Maintenance and Repair Agreement decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining
provisions of the Maintenance and Repair Agreement.
