Septic System Conditional Certification
The sale of Lot 2, Block 7, South Hills Subdivision is predicated on the construction and
certification of a new septic system. The system was recently placed on the lot under
the permit noted above. All work was completed in accordance with the Municipal
Permit No. OSP161229 and the attached record drawing information. Neither the well
nor the house have yet to be constructed on the lot. No electrical power is yet available
and the new treatment system has yet to be activated. Final grading and revegetation
of the new absorption mound must also be completed.
We are hereby requesting conditional approval of the completed septic system based
on a final inspection report to be completed and submitted once the house is in place,
the well is operational and all grading and vegetation has taken place. Necessary final
inspections will be completed and a revised inspection report will be submitted at that
time. The new owner of the lot will be responsible for the final completion of the well
and septic system and the submittal of the final documentation necessary for the
Certificate of On Site System Approval.

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer
The owner of Lot 2, Block 7, South Hills Subdivision intends to construct a three
bedroom home on the lot. We are therefore requesting a permit be issued to allow the
construction of a new well and septic system to serve the home. The attached Site Plan
and backup documentation identify the location and configuration of the proposed well
and septic system. No existing wells are less than 100' from the proposed septic
system. The well to the south of the property is an unregistered Class C Well and was
recently surveyed to be more than 150' from the south property line. In addition, the
proposed well to serve the lot does not conflict with septic systems in the area. The
surface drainage on the lot is to the northwest as shown by the drainage arrows on the
plan. The drainage pattern will not be changed as a result of this project.
The new absorption bed will be constructed with Engineered Receiving Soils (E.R.S.) in
accordance with proposed AMC 15.65.201.D3c along with an Advantex Treatment
System with an AX -20 Filter Pod and an ultraviolet treatment tube. Horizontal
conveyance will be utilized to transport the effluent through the organic material
surrounding the absorption bed and away from the area. The distribution line in the bed
will be placed at 2.5' above the existing ground to provide 6' of clearance from the in
situ silt below which has been found to be inconsistently permeable throughout the
area. Sections of subsurface soils in the area have been determined through
percolation testing to absorb water at rates faster than 120 minutes per inch, but these
sections are inconsistent and the majority of the subsurface soils are thought to be
impermeable. A minimum of 2' of MOA approved coarse sand will be placed below the
bed and 4' of clean well graded gravel (GW) meeting the requirements of Municipality of
Anchorage Standard Specifications for Type II or Type IIA select backfill will be placed
under the sand and to a depth of 4' below the surface or to the silty in situ layer. In
addition, the organic material beneath the footprint of the aboveground fill will be
removed to 1' below the surface and replaced with MOA approved sand to insure the
transfer of effluent to below the original ground surface.
The test holes placed on the lot in the past and monitored for many months indicated no
groundwater developed on this lot. The engineered fill will be placed in 12" lifts and
compacted to create a stable base for the absorption bed. The fill slopes will be
constructed at a 2:1 angle of repose with a 6" topsoil cover. All disturbed or new
surfaces shall then be seeded to produce vegetation and prevent erosion.

We are proposing to construct a new 450 square foot by .5' effective depth absorption
bed to treat and dispose of the effluent generated in the house. The additional porous
material beneath the bed will provide surge volume and septic treatment and increase
the surface area for water absorption to the surrounding strata. Water will flow
horizontally through the 90' LF perimeter of the bed by 3' effective depth sidewalls or a
total effective area of 270 SF. A conservative percolation rate of 60 minutes per inch
has been utilized for the peat with an application rate of 2 GPD/SF. The design was
completed in accordance with the proposed AMC 15.65.201.D3c.
We hereby request a waiver be issued to allow the use of the engineered receiving soils
in accordance with proposed AMC 15.65.201.D3c. We also request a waiver to allow
for 2:1 fill slopes on the completed mound. As stated earlier the slopes will be covered
with 6" of topsoil and seeded to produce vegetation to prevent erosion.
The ground surface on the lot in the area of the absorption bed slopes to the northwest.
The new bed will be constructed parallel with the contours of the slope as much as
possible in conformance with Municipal requirements. The new septic system will be
constructed a minimum of 100' from all wells and any surface water and 10' from the
proposed and existing water services in the area.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage

THIS MAINTENANCE AND REPAIR AGREEMENT made and entered into as of this

herein the "OWNER," and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Maintenance and Repair Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. Municipality grants permission to
Owner to utilize and operate an Advanced Wastewater Treatmeht System
(AWWTS), described as
located at (legal description).
2. Definitions.
· Alteration. Any change to the design or function of an AWWTS that
includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design.
Certificate of On-Site Systems Approval (COSA). An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with Anchorage
Municipal Code (hereinafter, "AMC") 15.65. These approvals certify that
H. Owner agrees that the AWWTS installation and maintenance requirements as
provided by the AWWTS vendor/installer and approved by the Municipality are
the governing guidelines for the construction, maintenance and repair of the
Owner's AWWTS.
6. Nonwaiver. The failure of either party at any time to enforce a provision of this
Maintenance and Repair Agreement shall in no way constitute a waiver of the
provisions, nor in any way affect the validity of the Maintenance and Repair
Agreement or any part hereof, or the right of such party thereafter to enforce each and
every provision hereof.
7. Amendment.
A. This Maintenance and Repair Agreement shall only be amended, modified or
changed by a writing, executed by authorized representatives of the parties, with
the same formality that this Maintenance and Repair Agreement was executed
with, and such writing shall be attached to this Maintenance and Repair
Agreement as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this agreement, the only authorized representatives of the parties
are:

manufacturer of the AWWTS for the entire term of the AWWTS. In addition, it
shall be the responsibility of the Owner during the term of this Maintenance and
Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay
for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement
costs, and (5) inspection costs.
B. Owner agrees to comply with ail applicable ordinance, laws, regulations, rules
and orders for the AWWTS.
C. Upon request by the Municipality, the owner agrees to provide the Municipality a
written schedule of routine maintenance m~d repairs which have been performed
on the system. When a record of maintenance is documented and maintained by
the system vendor, the owner agrees to allow the Municipality access to this
information.
D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS
may be assessed in accordance with AMC 14.60 for improper discharge.
E. Owner agrees that only maintenance and repair personnel approved by the
Municipality will inspect and make any necessary maintenance, repairs or
permitted alterations to the system.
F. Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
G. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On-Site Systems Approval.
8. Jurisdiction: Choice of Law. Any civil action arising from tlfis Maintenance
and Repair Agreement shall be brought in the Superior Court for the Third Judicial
District of the State of Alaska at Anchorage. The laws of the State of Alaska shall
govern the rights and obligations of the parties under this Maintenance and Repair
Agreement.
9. Severability. Any provisions ofthls Maintenance and Repair Agreement decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining
provisions of the Maintenance and Repair Agreement.

ADVANCED WASTEWATER.TREATMENT SYSTEM
MAINTENANCE AND REPAIR AGREEMENT
THIS MAINTENANCE AND REPAIR AGREEMENT,herein the"AGREEMENT' made and
entered11into as of this Day of January of 2019 by and between

Anchorage,herein the"MUNICIPALITY", in accordance with Anchorage Municipal Code
(AMC) 15.65.365. In consideration of the mutual covenants contained herein,the parties to this
Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System(AWWTS),
described as Advantex System
located at(legal description)

2. Maintenance. Repairs and Alterations.
(Owner is required to read, understand and initial each section)
Throughout the term of this Agreement,the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AWWTS shall he maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.

repair(s), maintenance, adjustment(s),replacement costs, and costs.inspection p s This
includes an annual maintenance fee(typically$400 to$600).
.* Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance,
qq repairs or permitted alterations to the system.
C ., Owner acknowledges that regular maintenance of an AWWTS reduces the potential
failure of the system,which could include sewage backup and costly repairs or drainfield
replacement.

\I 2. Owner acknowledges that the Municipality may request records of maintenance and
repairs from the manufacturer's representative or maintenance provider.
Owner acknowledges that the fine for failing to maintain and repair an AW4TS may be
assessed in accordance with AMC 14,60.030
L-- Owner agrees to grant the Municipality reasonable access to test and inspect the
AWW-1 S. The Municipality will give at least 24-hour notice.
Owner agrees that any sale or transfer of title of the property will not occur without a new

i( ._,- Owner agrees that the AWWTS installation and maintenance requirement as provided
by the AWW FS vendor installer and approved by the Municipality are the governing
guidelines for the construction, maintenance and repair of the Owner's AWWTS.
\t,./ Owner agrees to maintain remote monitoring of the AWWTS as required by the
AWWTS approval
3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system,or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4. i✓onwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions, nor in any way affect
the validity of the Agreement er any part hereof,or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized mcar.s shall he void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.
