All construction must be in accordance with: 1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska Wastewater
Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80).
3. The wastewater code requires inspections during the installation. The engineer must notify the Development Services
Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
Special Provisions:
The Engineer needs to do an additional test hole for the alternate field and southern field prior to
the construction of the septic field. Construction may proceed at your own risk before the 7 day
water monitoring is complete.

Septic System Design & New Water Well
CHANGE ORDER Revision
During trench construction, a large seep developed from the direction of the new house/garage
site. This may have diverted surface water toward the trench. The groundwater was monitored and
found to be ~ 9' during high water month of October.
A Change Order is being requested. A 1' lot line waiver is requested. The same owner owns both
lots along the lot line. Due to the limited space, a CAT III system has been sized for the alternate

A new house is planned with new- septic system and water well. The design is for a 4-bedroom
septic system. A soil test revealed excellent soil, consistent with other soil tests in the area. An
additional soil test will take place prior to construction. The design calls for a deep trench, and a
new septic tank. The entire subdivision is on private water wells and septic systems. The terrain
slopes to the south at about 5 % at the new trench site. A new water well is planned at the proposed
site. A small drainage exists in the south part of the lot. Separation has been maintained with the
design.
Please review the 4 bedroom wastewater system design for the new- 4 bedroom home. I have
included design plans & specs, design guidelines, & soil tests. If there is need for additional

SPECIFICATIONS & DESIGN GUIDELINES
Wastewater System Sizing: Design is for a new 4-bedroom septic system; serving a new
4-bedroom, single family home. This is a mostly developed subdivision. These lots are
near 1 acre and are served by private wells amd septic systems. No adverse impacts are
expected from trench/tank construction and new water well. No conflicts to the other lots
will take place by this septic system construction. The easements are located on the
drawing and are not encroached upon. An application rate of 1.2 GPD/FT2. Trench Length
- 500 FT2/5' x .78 reduction factor -- 78' trench, (2 Trenches, 1.5' effective). A Category
III System for the alternate trench will allow an application rate of 5 GPD/FT2. Length -
120 FT2/5'x .78 - 18.72, say 19' trench. A new 1250 gallon septic tank will be installed.
The proposed well will be placed in the northeast comer. A drainage is present, but the
100' separation will be maintained.
Specification Requirements: All components and work must comply with the
Municipality of Anchorage Specifications (AMC) & State of Alaska Drinking Water
Regulations and Wastewater Regulations.
· Two compartment, New 1250 gallon septic tank. Install new Double Cleanouts.
· 5' minimum between the tank and trench. 10' to property lines.
· 3' of cover or insulation is required for trench; 2" Minimum thickness for insulation
can substitute for 1' cover.
· Tank & solid pipe must be set on well compacted, stable soil.
· 4 inch diameter cleanouts with airtight caps are required 1' to 4' from foundation
wall, prior to any 90 degree bend in 4 inch line, in each tank compartment, and two
adjacent opposing cleanouts between the tank and the absorption field, not more than
10' from the tank positioned tn provide cleanout access towards the tank and towards
the absorption field.
· All cleanouts must extend to at least ground level.
· In solid pipe runs, ASTM D-3034 may be used in lieu of cast iron.
· Trench to be placed level, minimum of 4' to groundwater, 6' to bedrock from
drain-rock.
· Drain rock to be ½ inch to 2 ½ inch screened. Drain rock to be distributed uniformly
throughout the trench.
· Perforated pipe to be installed level with perforations down.
· Silt barrier (flEer fabric) to be installed above the drain rock.
· Smeared trench sides must be raked or scarified before drain rock placement.
· Backfill over drain rock must not be less than 36".
· The finish grade must be mounded to promote drainage away from trench.
· Insulation must be placed over any pipe installed under driveways or parking areas.
· Approved pipe materials include cast iron, PVC ASTM D3034, PVC ASTM F789,
ABS ASTM D2661,
· Sewer Service Line is minimum 2% slope."
· Septic Tank to be pumped every two years or when required.

WHEREAS, Supplying Party owns property located in the Anchorage Recording District, the address

WHEREAS, Supplied Parry owns property located in the Anchorage Recording District, the address

official plat thereof, filed under Plat No. 77-262, in the records of the Anchorage

WHEREAS, Supplying Party currently has a water welt and water treatment, storage and distribution
system on Lot 158-8 capable of and currantty supplying water to Lot 158-A via a water tine
connecting the water systems on both properties (me "water distribution system");
WHEREAS, SGS USA -Anchorage analyzed the welt water and determined itto be safe and potable
water and Anchorage Welt and Pump Service Inc. deemed the watt capacity adequate for two,
single family residences;
WHEREAS, the parties intend for the welt and water distribution system on Lot 158-B to be used
and operated to provide an adequate supply of water for Lots 158A and B for domestic
consumption purposes, and to assure the continuous and satisfactory operation and maintenance
of the well and water distribution system; and
NOW THEREFORE, in consideration of the premises and covenants heremcontained, the parties

1. This Agreement is for the benefit of the present and future owners, their heirs, successors and
assigns of Lots 158A and B, is to run with the land and continue in perpetuity unless and until it is
terminated as provided herein;
2. For the exclusive benefit of Lots 158A and Band use of the households residing. thereon, the
parties agree to withdraw and use water from the well for ordinary domestic use for one single-

4. Supptied Parry shall pay Supplying Party $75 usage fee on the first day of each month for use of
the well and water distribution system. If Supplied Party does not make the monthly payment by
the 511 of the month, a $25.00 Late fee will be charged. If Supplied Party does not timely pay the
usage fee three months in a row, Supplying Party may terminate the Agreement.
5. Each parry shalt pay one-half of alt maintenance, repairs and improvements to the welt and
water distribution system.
6. Any expenses or damages incurred or resulting from the removal or replacement of pre-existing
site improvements necessary for the welt and water distribution system operation, maintenance,

7. Each party, at their own expense, will promptly repair, maintain and replace all water pipes,
pumps, filters, protective devices on their property serving their respective dwellings.

States of America, Rural Housing Services and the costs of such well tests are equally shared like
any other expense.
10. Unless an emergency situation dictates otherwise, the parties shalt first agree and consent to
expenditures for the welt and water distribution system maintenance, replacement or
improvement.
11. If the welt shalt become contaminated and shalt no longer supply water suitable for domestic
consumption, the water is no longer adequate for the needs two single family residences as
provided for herein, another source of water becomes available to the respective parcels, or the
owners of both properties agree, the Agreement shall terminate and the rights and obligations of
the parties created by this Agreement shalt cease In accordance with the terms and conditions
herein.
12. That upon the availability of such other source of water, it is contemplated that a reasonable
time shall be allowed to effectuate the necessary connections to the new source.
13. Upon termination of the Agreement, Supplied Party shalt have no further right to the use of the

1d. That the parties may amend this Agreementto assure equitable distribution of shared hosts
and responsibilities and as they may agree in writing.
15. Should a dispute concerning the Agreement arise bemoan the parties that they cannot resolve,
the parties shall submit the matter to mediation. If the parties are unable to resolve the dispute

4. Supplied Parry shalt pay Supplying Party $75 usage fee on the first day of each month for use of
the well and water distribution system. If Supplied Party does not make the monthly payment by
the 5`" of the month, a $25.00 late fee will be charged. If Supplied Party does not timely pay the
usage fee three months in a row, Supplying Party may terminate the Agreement.
5. Each parry shall pay one-half of all maintenance, repairs and improvements to the well and
water distribution system.
6. Any expenses or damages incurred or resulting from the removal orreplacement of pre-existing
site improvements necessary for the well and water distribution system operation, maintenance,
7. Each party, at their own expense, will promptly repair, maintain and replace alt water pipes,
pumps, fitters, protective devices on their property serving their respective dwellings.
8. The water shalt be annually sampled and tested by a certified water testing laboratory and the
uvea shalt he inspected every five years at minimum, by a licensed well contactor as part of a
regular maintenance plan with each party equatly sharing the costs thereof.
9. That the undersigned parties shall permit welt water sampling and testing by a responsible
authority at the request of an undersigned parry, state or local authority, mortgagee, or the United
States of America, Rural Housing Services and the costs of such well tests are equally shared like
any other expense.
10. Unless an emergency situation dictates otherwise, the parties shalt first agree and consent to
expenditures for the well and water distribution system maintenance, replacement or
improvement.
11. If the welt shalt become contaminated and shall no longer supply water suitable for domestic
consumption, the water is no longer adequate for the needs two single family residences as
provided for herein, another source of water becomes available to therespective parcels, or the
owners of both properties agree, the Agreement shall terminate and the rights and obligations of
the parties created by this Agreement shalt cease in accordance with the terms and conditions
herein.
12. That upon the availability of such other source of water, it is contemplated that a reasonable
time shall be allowed to effectuate the necessary connections to the new source.
13. Upon termination of the Agreement, Supplied Party shalt have no further right to the use of the
well and water distribution system and no further financial obligations with respect thereto. The
Supplied Party shall disconnect from the water distribution system at their own expense.
